Comparison of cadaveric and isomorphic three-dimensional printed models in temporal bone education.

OBJECTIVES/HYPOTHESIS: Current three-dimensional (3D) printed simulations are complicated by insufficient void spaces and inconsistent densi … factory (range, 4.9-6.2). The simulation was considered a beneficial training tool for all types of mastoidectomy (range, 5.9-6.6), posterior tympanotomy (6.5 ± 0.71), and skull base approaches (range, 6-6.5). Participants believed the model to be an effective training instrument (6.7 ± 0.68), which should be incorporated into the temporal bone lab (7.0 ± 0.0). …

Successful Systemic and Topical Treatment of Mycobacterium abscessus Otomastoiditis.

Mycobacterium abscessus is an extensively drug-resistant opportunistic pathogen that can cause chronic otomastoiditis. There are no evidence-based treatment regimens for this severe infection. We treated four children with M. abscessus otomastoiditis with a structured regimen of topical imipenem and tigecycline, intravenous imipenem and tigecycline, and oral clofazimine and azithromycin and adjunctive surgery. This structured approach led to cure, with 1 year of follow-up after treatment. Adverse events were frequent, mostly caused by tigecycline.

The impact of middle ear aeration on surgical outcome after intact canal wall tympanoplasty for cholesteatoma.

OBJECTIVE: The aeration status of the middle ear is presumed to be one of the factors affecting the outcome in acquired cholesteatoma. The present study investigated the impact of postoperative middle ear aeration on hearing and recurrence after intact canal wall tympanoplasty. METHODS: We conducted a retrospective chart review of 127 pars flaccida cholesteatoma patients who underwent primary intact canal wall tympanoplasty at a tertiary academic medical center. We collected data on clinical characteristics, including extent of cholesteatoma, surgical procedure, pre- and postoperative middle

Characteristics of laryngopharyngeal reflux in patients with chronic otitis media.

OBJECTIVE: To summarize the characteristics of laryngopharyngeal reflux (LPR) in patients with chronic otitis media. METHODS: This was a prospective study in which 31 patients with chronic otitis media were enrolled. General patient information, reflux symptom index (RSI), reflux finding scores (RFSs), and Ryan scores were summarized. RESULTS: Most (29/31, 93.5%) patients had a negative RSI (RSI ≤ 13). The most common symptoms of these patients were throat clearing (22/31, 71.0%), symptoms of the stomach and esophagus (19/31, 61.3%), and excess throat mucus or postnasal drip (14/31, 45.2%).

Mastoid cavity obliteration in revision tympanomastoidectomy with ossiculoplasty for chronic cholesteatomatous otitis media: A retrospective case series on audiological and Patient-reported outcomes.

OBJECTIVE: Mastoid cavity obliteration is a surgical strategy rather than an intervention per se, that has been proposed as an effective approach to reduce post-operative complications and long-term morbidity associated with canal wall down (CWD) tympanomastoidectomies. The present study aims to evaluate, in a retrospective case series, audiological, functional, and quality-of-life (QoL) outcomes in adult patients undergoing revision tympanomastoidectomy and ossicular chain reconstruction with mastoid obliteration. MATERIALS AND METHODS: A retrospective analysis was performed on a cohort of p…

Challenges and outcomes of cholesteatoma management in children with Down syndrome.

INTRODUCTION: The high incidence of chronic otitis media with effusion and Eustachian tube dysfunction in children with Down syndrome (DS) may predispose them to cholesteatoma formation. Establishing the diagnosis, choosing the appropriate operative intervention, and post-operative care can be challenging. OBJECTIVE: To describe management strategies for cholesteatoma diagnosis, surgical treatment, and post-operative management in children with Down syndrome. METHODS: Retrospective case series of 14 patients (17 total ears) with Down syndrome diagnosed with cholesteatoma over a 9-year period

Is it useful to use computerized tomography image-based artificial intelligence modelling in the differential diagnosis of chronic otitis media with and without cholesteatoma?

OBJECTIVE: Cholesteatoma is an aggressive form of chronic otitis media (COM). For this reason, it is important to distinguish between COM with and without cholesteatoma. In this study, the role of artificial intelligence modelling in differentiating COM with and without cholesteatoma on computed tomography images was evaluated. METHODS: The files of 200 patients who underwent mastoidectomy and/or tympanoplasty for COM in our clinic between January 2016 and January 2021 were retrospectively reviewed. According to the presence of cholesteatoma, the patients were divided into two groups as chron…

A survey of the burden of management of chronic suppurative otitis media in a developing country.

BACKGROUND: Although the prevalence of chronic suppurative otitis media (CSOM) is much higher in developing counties, most surgical treatmen … facilities and/or expertise. Among institutions that offer surgery, 40% (4/10) offer only cortical mastoidectomy and only 30% (3/10) offer type 1 tympanoplasty. Achievements of permanent dry ears were reported more in institutions that offer surgery. Unaffordable cost of hearing aid and lack of expertise for tympanoplasty are the major challenging factors for …

Complications of homograft tympanoplasty.

The use of a homograft tympanic membrane with an attached malleus has provided an excellent alternative for reconstructing the severely damaged middle ear. I have limited homograft tympanoplasty to four specific indications: previous failure of standard tympanoplasty techniques, high risk of failure (total perforation, absent malleus, slag burns), reconstruction of radical mastoidectomy, and congenital aural atresia. Homograft tympanoplasty has provided a 94 per cent incidence of anatomic success in these severely damaged middle ears. Closure of the air-bone gap to within 25 dB. was accomplish…
