Impact of maternal death reviews at a rural hospital in Zambia: a mixed methods study.

BACKGROUND: Maternal mortality in sub-Saharan Africa remains high despite programmatic efforts to improve maternal health. In 2007, the Zambian Ministry of Health mandated facility-based maternal death review (MDR) programs in line with World Health Organization recommendations. We assessed the impact of an [MDR program] at a district-level hospital in rural Zambia. METHODS: We conducted a mixed methods convergent study using hospital data on maternal mortality and audit reports of 106 maternal deaths from 2007 to 2011. To evaluate the overall impact of MDR on maternal mortality, we compared …

Maternal near miss and predictive ability of potentially life-threatening conditions at selected maternity hospitals in Latin America.

BACKGROUND: Every year millions of women around the world suffer from pregnancy, childbirth and postpartum complications. Women who survive the most serious clinical conditions are regarded as to have experienced a severe acute maternal complication called maternal near miss (MNM). Information about MNM cases may complement the data collected through the analysis of maternal death, and was proposed as a helpful tool to identify strengths and weaknesses of health systems in relation to maternal health care. The purpose of this study is to evaluate the performance of a systematized form to detec

Pregnancy-related mortality in Guatemala, 1993-1996.

To select the proper interventions that could prevent maternal mortality, adequate and appropriate maternal mortality data are needed. Nevertheless, the quality and quantity of information and the scope of maternal health- and death-related data are inadequate in many countries, particularly in the developing world. From January 1993 to December 1996 a surveillance program in maternal mortality was developed to conduct surveillance studies in the department of Guatemala, Guatemala. With an active surveillance system, our approach gave a more complete picture of maternal death and produced info…

Beyond Infection: Targeting Maternal Sepsis.

Although sepsis only occurs in 1 to 2.5 per 1,000 births, it represents 11-23% of maternal deaths-many of which are preventable. Due to the high morbidity and mortality associated with sepsis, the Surviving Sepsis Campaign (SSC) recommends that every hospital develop a process to screen for sepsis and a standard operating procedures for treatment. The purpose of this report is to review the most recent SSC guidelines with particular reference to maternal physiology, highlight maternal-specific screening tools, and recommendations for treatment, including the need for multidisciplinary collabor

A Delphi process to build consensus on revised Emergency Obstetric and Newborn Care (EmONC) signal functions and levels of care.

The emergency obstetric care (EmOC) monitoring framework has been used for decades to monitor the availability and use of EmOC services in low- and middle-income countries (LMICs). EmOC monitoring is based around eight signal functions, a shortlist of key clinical interventions capable of averting deaths from the main direct causes of maternal mortality, categorised between two levels of care: basic and comprehensive, with a newborn resuscitation signal function added in 2009. The Re-Visioning Emergency Obstetric and Newborn Care (EmONC) Project (2020-2024) aimed to update the EmOC approach to…

Adapting the 2022 WHO verbal autopsy tool for use in Lagos State, Nigeria: insights from the LVASA-SRS project.

OBJECTIVE: Accurate and routine cause-of-death data are essential for health planning, yet many low- and middle-income countries lack comprehensive systems for such data, resulting in undocumented maternal and perinatal deaths. This article details the adaptation of the 2022 WHO Verbal Autopsy (VA) tool for maternal mortality surveillance in a linguistically diverse urban setting with high rates of out-of-facility deaths as part of the Lagos Verbal and Social Autopsy Sample Registration System project in Nigeria. RESULTS: The adaptation process enhanced the tool’s cultural sensitivity, languag…

Why Abortion is Illegal? Comparison of Legal and Illegal Abortion: A Critical Review.

Abortion is the termination of pregnancy that occurs spontaneously or purposely. In the most developed world, abortion is legally allowed for women seeking safe termination of pregnancies. Particularly, when access to legal abortion is restricted, abortion is the resort to unsafe methods. The aim of this review is to necessitate safe abortion and to accentuate the consequences of illegal abortion in case of legal prohibition. We used Pubmed, MedLine and Scopus databases to review previous literatures of safe, unsafe, legal and illegal abortions. Research work and reports from organizations suc

Understanding and managing HELLP syndrome: the integral role of aggressive glucocorticoids for mother and child.

Antepartum or postpartum HELLP syndrome constitutes an obstetric emergency that requires expert knowledge and management skills. The insidious and variable nature of disease presentation and progression challenges the clinician and complicates consensus on universally accepted diagnostic and classification criteria. A critical review of published research about this variant form of severe preeclampsia, focused primarily on what is known about the pathogenesis of this disorder as it relates to patient experience with corticosteroids for its management, leads to the conclusion that there is mate

[Audits and confidential enquiry: thoughts on the technique used by the French National Committee for the reduction of maternal deaths].

This study demonstrates the need for a confidential approach to medial information when the objective is to improve medical practices, as was the goal of the French National Committee for the reduction of maternal deaths. This approach is the exact opposite of a litigation where the goal is to determine "guilt". Confidential analysis of maternal deaths is not designed to find a guilty person, but rather to identify the mechanisms leading to a maternal death in order to correct the organizational or medical practice leading to failure. We review here similar actions taken to reduce other types

The bad old days are still here: abortion mortality in developing countries.

Each year, complications related to unsafe abortion account for at least one in seven maternal deaths worldwide. Nearly all of these deaths occur in developing countries and most are preventable. The authors argue that to prevent abortion-related deaths, all women must have access to safe abortion and contraceptive services, and they discuss the strategies used to prevent abortion-related deaths--preventing unwanted pregnancy, preventing abortion, preventing unsafe abortion, and treating abortion complications. Some of these strategies have been effective, while others have failed. Preventing
