Transcrestal sinus floor elevation with osteotomes: simplified technique and management of various scenarios.

In order to place implants subantrally when there is a reduced amount of bone, some form of bone augmentation is required. A transcrestal sinus floor elevation using osteotomes can increase bone levels and is a highly successful procedure. However, malleting osteotomes can be disconcerting for a patient. Therefore, a simplified technique is presented that minimizes the use of a mallet when executing a sinus floor elevation in medium dense bone. In addition, issues related to performing a transcrestal sinus floor elevation are addressed.

Inadequate vertical bone dimension managed by bone-added osteotome sinus floor elevation (BAOSFE): a literature review and case report.

AIM: The purpose of this report is to provide a brief literature review of the bone-added osteotome sinus floor elevation (BAOSFE) procedure and to present a case utilizing this technique. BACKGROUND: Implant placement in the posterior maxillary region is often compromised due to anatomic limitations. In clinical situations where there is inadequate vertical bone dimension, the BAOSFE technique can be employed to increase bone height and allow placement of implants with an appropriate length dimension. CASE REPORT: A case of a 60-year-old female with inadequate vertical bone dimension in the

Correction of facial asymmetry and maxillary canting with corticotomy and 1-jaw orthognathic surgery.

Although 2-jaw orthognathic surgery is a typical recommendation for the treatment of facial asymmetry, another good treatment alternative is maxillary corticotomy with temporary skeletal anchorage devices followed by mandibular orthognathic surgery. The corticotomy procedure described here can achieve unilateral molar intrusion and occlusal plane canting correction with potentially fewer complications than 2-jaw orthognathic surgery. The approach allows movement of dentoalveolar segments in less time than with conventional dental intrusion using temporary skeletal anchorage devices. A 2-jaw as

Interdisciplinary treatment of a patient with amelogenesis imperfecta, a skeletal Class III relationship, and an anterior open bite.

An adult woman with amelogenesis imperfecta, a skeletal Class III relationship, long face syndrome, and a severe anterior open bite received interdisciplinary treatment (orthodontics, orthognathic surgery, and prosthodontics). Presurgical orthodontic treatment was followed by a maxillary posterior impaction with anterior advancement and a mandibular setback. After surgery, the patient received ceramic crowns. Function and esthetics were successfully re-established.

Osteotome sinus floor elevation technique without grafting: a 5-year prospective study.

AIM: To evaluate the long-term stability of peri-implant bone formation following implant placement without grafting into resorbed posterior maxillae. MATERIALS AND METHODS: Twenty-five implants of 10 mm were placed in 17 patients to rehabilitate atrophic maxillae by means of an osteotome sinus floor elevation (OSFE) procedure without grafting. Mean residual bone height was 5.4±2.3mm. Bone levels were evaluated at 1, 3 and 5 years using periapical radiographs. RESULTS: All implants fulfilled survival criteria and gained peri-implant bone (mean increase 3.2±1.3mm). Implant protrusion into the

Comparative evaluation of upper airway dimensions following isolated mandibular setback versus bimaxillary surgery in the surgery-first approach: A 1-year follow-up study.

OBJECTIVES: This study aimed to quantitatively compare alterations in upper airway dimensions-specifically volume and cross-sectional area-following isolated mandibular setback versus bimaxillary surgery using the surgery-first approach (SFA) in patients with skeletal Class III malocclusion. To the best of our knowledge, this is the first study to directly compare these two surgical protocols exclusively under the SFA paradigm with acoustic pharyngometry and per-millimetre analysis over a 1-year follow-up. MATERIALS AND METHODS: A retrospective cohort investigation was conducted on 40 skeleta

Implant over implant: an alternative method for solving malpositioned osseointegrated implants at the sinus floor.

An implant was malpositioned and osseointegrated at the maxillary sinus floor with no complications. However, unexpected bone formation over the implant made the implant nonfunctional. Because the patient rejected the removal of the implant, it was left in place while another short implant was placed into the newly formed bone in approximately the same position as the old implant to restore the posterior maxillary tooth. Within 6 months, the new implant over the old asymptomatic implant was functional. Radiographs revealed osseointegration and stable periimplant marginal bone level with no sig

New bone formation in the maxillary sinus without bone grafts.

PURPOSE: Various sinus augmentation procedures, using bone substitutes, have been used to place dental implants in the atrophic posterior maxilla. The aim of this article is to evaluate the possibility of new bone formation in the maxillary sinus without a bone graft. MATERIALS: Ten patients without significant sinus pathosis were selected for this study. The bony window was prepared in the lateral wall using the ultrasonic piezoelectric bone surgery device (Surgybone, Silfradent srl, Sofia, Italy). The sinus membrane was elevated superiorly and implants were placed simultaneously to maintain

A new technique for the transcrestal sinus floor elevation and alveolar ridge augmentation with press-fit bone cylinders: a technical note.

BACKGROUND: The minimally invasive sinus floor elevation as first described by Summers is limited in the volume of augmentation that is possible. In contrast, the more invasive approach is the sinuslift of Tatum which is indicated for greater bone deficiencies. Therefore, a new technique was developed for transcrestal elevation of the sinus floor and alveolar ridge augmentation with bone dowels in press-fit technique. MATERIALS & METHODS: The crestal cortical bone is cut with a hollow grinder followed by an indirect sinus floor elevation with a plunger. The cylindrical defect is then filled w

Safe sinus lift: use of acrylic stone trimmer to avoid sinus lining perforation.

Iatrogenic injury to the maxillary sinus membrane is a common complication during direct sinus lift procedures. The most common cause is perforation of the Schneiderian membrane using a tungsten-carbide round bur no.6. We propose a safe technique in which an acrylic stone trimmer is used to create a window in the maxillary antrum thereby minimizing the risk of injury to the delicate sinus membrane.

Minimally invasive sinus floor elevation procedures for simultaneous and staged implant placement.

Osteotome techniques are used as a less invasive alternative to the lateral window osteotomy to increase the volume of bone in the posterior maxilla. Implants may be placed simultaneously for four to six months after performing osteotome-mediated sinus floor elevation, depending upon the residual crestal bone remaining beneath the sinus floor. This article describes the clinical indications and techniques for localized internal sinus elevation using osteotomes. The incorporation of this minimally invasive crestal approach to sinus augmentation into the clinical practice results in increased ca

Cytological Examination of Secretions From the Paranasal Sinuses in Horses.

The objective of this study was to determine whether the assessment of cytological features of secretions from the paranasal sinuses represents a useful diagnostic tool in equine sinusitis to distinguish between different etiologies. Secretion samples from 50 horses with sinusitis and 10 healthy horses were taken transendoscopically from the drainage angle of the nasomaxillary aperture using a Swing Tip catheter. An additional direct sample from the caudal maxillary sinus was taken from all healthy horses after trephination. A direct sample was obtained from the affected sinus in 19 diseased h
