The rendu-osler-weber disease revealed by a refractory hypoxemia and severe cerebral fat embolism.

The Rendu-Osler-Weber disease is a genetic disease which may lead to severe hemorrhage and less frequently to severe organ dysfunction. We report the case of a 22-year-old patient with no personal medical history who was involved in a motorcycle accident and exhibited severe complications related to large arteriovenous pulmonary shunts during his ICU stay. The patient developed an unexplained severe hypoxemia which was attributed to several arteriovenous shunts of the pulmonary vasculature by a contrast study during a transesophageal echocardiographic examination. The course was subsequently c

Cadaveric position of unilateral vocal cord: a case of cricoid fracture with ipsilateral arytenoid dislocation.

We report a case of cricoid cartilage fracture with unilateral arytenoid dislocation following a motorcycle accident. This 25 year old male sustained blunt injury to the head, face and neck. He presented late to the hospital with one week history of dysphonia. Laryngoscopy revealed cadaveric position of the non-functioning left vocal cord. CT and MRI showed laterally displaced left vocal cord. Displaced fractures were noted in the cricoid at the junction of lamina with the anterior arch on the left side and at the right side of the anterior arch, along with dislocated left arytenoid resulting

Laparoscopic resection of isolated hepatic splenosis mimicking liver tumors: case report with a literature review.

Isolated hepatic splenosis is a rare condition in hepatobiliary surgery. In this study, we report a case of this condition managed by laparoscopic surgery. A 38-year-old male hepatitis B virus carrier, who had a motorcycle accident and splenectomy 14 years before the current incident, was hospitalized due to a hepatic mass. His laboratory tests were consistent with a hyposplenic state, whereas radiologic images revealed a benign tumor in the left liver lobe located in a site difficult to access with preoperative biopsy. Therefore, we performed a laparoscopic exploration and total resection, wh

New maneuver for fixation of pediatric nasal bone fracture.

Surgical treatment of nasal bone fractures in children is a complex and challenging task not only to maxillofacial surgeons but exceeding to ears, nose, and throat and plastic surgeons. Twelve child patients including 9 boys (75%) and 3 girls (25%) with nasal bone fractures were seen at the Maxillofacial Surgery Department at Ramadi Teaching Hospital during the period of December 2009 to December 2010. Fractured nasal bone is reduced to its anatomic position with Walsham forceps, and the dislocated or disrupted nasal septum with Asch forceps. The most common cause of the injury was road traffi

Central retinal artery occlusion following severe blow-out fracture in young adult.

A 20-year-old woman was involved in a traffic accident while riding a motorcycle. The vision in her right eye was severely reduced. At the first examination, the eyelids of her right eye were severely swollen, and the eye could barely be seen. The fundus was not visible. She had no light perception in her vision. Computed tomography revealed a severe blow-out fracture in her right eye. Surgery was immediately performed to correct the fracture and the eye globe was replaced in the orbit. On the fourth postoperative day, the right fundus was visible and a cherry-red spot and milky-white edema we

Risk factors and musculoskeletal injuries associated with all-terrain vehicle accidents.

Accidents, injuries, and deaths sustained via all-terrain vehicle (ATV) use are on the rise. In addition to safe and proper ATV operation, accident-related risk factors include operator controllable behaviors such as helmet use, alcohol use, and deciding whether or not to carry a passenger. What the operator has little or no control over, however, is the inherently unstable ATV design with its narrow wheelbase, short turning radius, and high center of gravity, in addition to common use of low tire pressure to maximize maneuverability. These factors lead to musculoskeletal injuries that consist

Peripheral nerve injuries: a retrospective survey of 456 cases.

This 16-year retrospective study reports the data on 456 consecutive patients with 557 peripheral nerve injuries (PNIs) between 1989 and 2004. Most patients were men (74%) and the mean age was 32.4 years. In 83% of cases the PNIs were isolated; combined lesions most commonly involved the ulnar and median nerves. Upper-limb PNIs occurred in 73.5% of cases; the ulnar nerve was most often injured, either singly or in combination. Vehicle accidents affecting the brachial plexus or radial, sciatic, facial, and peroneal nerves were the most common cause of injury. Penetrating trauma commonly affecte

[Cranial epidural abscesses: a case report].

OBJECTIVE AND IMPORTANCE: Cranial epidural abscess mostly follows sinusitis or mastoiditis, also open cranial injury or cranial surgery with iatrogenic infection. In our case the abscess possibly evolved from epidural haematoma and the process invaded frontal sinus and subgaleal space in the frontal region. CLINICAL PRESENTATION: A 22-year-old male after motorcycle accident 4 years ago, with signs of sinusitis frontalis and seizures, with CT confirmed epidural abscess. INTERVENTION: Craniotomy was performed and the abscess totally removed. Homogeneous bone was replaced at the operation. Good

[Horizontal deceleration trauma with diffuse decollement bleeding--a casuistry].

Treatment of severe haemorrhage caused by multiple trauma is a serious challenge to preclinical as well as clinical management. This is a case report of a motorcycle accident in which a patient sustained total amputation of both legs. Following adequate preclinical care, vital indication led to the patient's immediate surgical treatment. After initially successful haemodynamic stabilisation, the patient developed a horizontal deceleration trauma which resulted in an extended decollement of the muscles of the back and buttock. During the further clinical course, soft tissue bleeding occurred th

Long-term follow-up of coronary artery dissection due to blunt chest trauma with spontaneous healing in a young woman.

We report a previously healthy 17-year-old woman who experienced coronary artery dissection with an acute transmural anterior myocardial infarction and myocardial contusion following blunt chest trauma in a motorcycle accident. A chest roentgenogram on admission was normal, and an electrocardiogram showed an acute transmural anterior myocardial infarction with complete right-bundle-branch block. A 2D echocardiogram revealed an akinesis of the anterior wall and a hypokinesis of the posterior wall in the left ventricle. Initial coronary angiography demonstrated severe stenosis with delayed anteg

A review of 100 closed head injuries associated with facial fractures.

One hundred closed head injuries associated with facial fractures treated over a 78-month period at a level I trauma center in Northeast Ohio were reviewed. The incidence of closed head injury in patients with facial fractures was 17.5%. Males suffered closed head injuries four times more often than females, and sustained severe intracranial injuries eight times as often. The 16- to 30-year age group predominated (59%). Although motor vehicle accidents were the most frequent cause of injury (61%), motorcycle accidents were associated with the most severe head injury. The mandible/midface fract

Posterolateral bone graft of the tibia.

A series of 30 high-energy tibial diaphyseal fractures in 30 patients were treated with posterolateral bone grafting (1984-1987). All of the fractures were open, a majority being Gustillo Type III with significant soft-tissue injury, comminution, or segmental bone loss. The common mechanisms of injury included motorcycle, pedestrian versus automobile, and motor vehicle accidents. Nineteen of 30 fractures were treated with a posterolateral bone graft within six months of the initial injury, and 11 of 30 were grafted after established nonunion. Union was obtained in 29 of 30 fractures (97%). Hea

Differences in head injuries of pedal cyclist and motorcyclist casualties in Victoria.

There being no legal requirement in Victoria for pedal cyclists to wear crash helmets (and only few do so), we tested the hypothesis that pedal cyclists are subject to significantly greater risk of head injury than motorcyclists, who are required by law to wear them. Injury data of 1975 to 1980 showed that there were three times more fatalities and reported non-fatal casualities among motorcyclists than among pedal cyclists. Head injuries, including fractured vault of skull, fractured base of skull, concussion and intracranial injury, were significantly more frequent among pedal cyclist casual

Alcohol and road safety: Geelong experience 1967 to 1978.

This paper reviews the part which prior consumption of alcohol played in motor vehicular fatalities in Geelong and district between January, 1967, and June, 1978. There were 344 deaths of persons aged 17 years and older within four hours after the accident. Of 147 such victims who were aged 17 to 50 years and who were drivers of motor vehicles. 54% had a blood alcohol concentration (BAC) greater than 0.1 g/100 mL (22 mmol/L) at autopsy. Of 35 male pedestrians, 60% had a BAC greater than 0.15 g/100 mL (33 mmol/L) at autopsy, and 80% of such accidents occurred between 6 p.m. and 10 p.m. A high d

Rupture of the thoracic esophagus from blunt trauma.

A young man with severe multiple injuries following a motorcycle accident was admitted with head and mandible fractures, coma, fracture dislocation at C5-C6 resulting in total leg paralysis, partial paralysis of the right arm and intercostal muscles, and closed chest injury with possible pulmonary contusion. On the fourth day he developed fulminating mediastinitis and massive empyema, and was found to have a ruptured esophagus. Recovery became possible with surgical drainage of the pleural cavity and mediastinum, proximal and distal decompression of the esophagus, antimicrobial therapy, irriga

Central slip defect reconstruction utilizing partial ulnar side of flexor digitorum superficial tendon for chronic boutonniere deformity: A case report.

BACKGROUND: Post-traumatic central slip injuries, resulting in boutonniere deformity, are a complex issue that can significantly impact hand function and quality of life. The deformity should be easily reducible in the acute phase, but if left untreated, it shortens the oblique retinacular ligament, leading to chronic contracture. This is a challenging issue in hand surgery, as chronic central slip defects cannot be sutured like other tendon. Various methods for reconstructing central slips have been discussed, but the best method for subacute or chronic injuries remains unclear. This case pre

Trauma mechanisms and patterns of blunt cervical vascular injury: A descriptive study using a nationwide trauma registry.

OBJECTIVE: Blunt cervical vascular injury (BCVI) is a non-penetrating trauma to the carotid and/or vertebral vessels following a direct injury to the neck or by the shearing of the cervical vessels. Despite its potentially life-threatening nature, important clinical features of BCVI such as typical patterns of co-occurring injuries for each trauma mechanism are not well known. To address this knowledge gap, we described the characteristics of patients with BCVI to identify the pattern of co-occurring injuries by common trauma mechanisms. METHODS: This is a descriptive study using a Japanese n

Comparison of the RTS and ISS scores on prediction of survival chances in multiple trauma patients.

PURPOSE OF THE STUDY: Trauma represents the third cause of death after cardio vascular disease and tumors. Also in Iran, road accidents are one of the leading causes of death. Rapid evaluation of trauma severity and prediction of prognosis and mortality rate and probability of survival and rapid treatment of patients is necessary. One of the useful instruments for this is ISS and RTS scoring systems. This study evaluated 70 multi trauma patients in Fatemi trauma center affiliated to Ardabil University of medical science. MATERIALS AND METHODS: This study was prospective study populations were

Rib Season: Temporal Variation in Chest Wall Injuries.

INTRODUCTION: Trauma to the chest wall is one of the most common injuries suffered. Knowing whether there are regular and reproducible changes in frequency or severity of certain injury types may help resource allocation and improve prevention efforts or outcomes; however, no prior studies have evaluated seasonal variation in chest wall injuries (CWIs). We aimed to determine if CWIs vary annually in a consistent distinct temporal variation. METHODS: Using an established traumatic blunt CWI database at a single urban level 1 trauma center, patients with a moderate-to-severe (chest wall Abbrevi

Traumatic hangman's fracture after cervical disk arthroplasty with device in-tact: A case report and literature review.

BACKGROUND CONTEXT: Cervical disk arthroplasty (CDA) has been demonstrated to be a safe and effective method to treat myelopathy with the added benefit of preserving neck mobility compared to anterior cervical discectomy and fusion (ACDF). Few studies describe complications of trauma after CDA, and to our knowledge this is the only study describing a grossly intact artificial cervical disk (ACD) without extrusion after high energy trauma. Based on our case and a review of literature, we hypothesize that, given adequate osseous integration (OI), CDA may be a safe intervention despite their risk
