Maternal Infections, Antibiotics, Steroid Use, and Diabetes Mellitus Increase Risk of Early-Onset Sepsis in Preterm Neonates: A Nationwide Population-Based Study.

The global evolution of pathogens causing early-onset sepsis (EOS), a critical condition in preterm infants, necessitates a re-evaluation of risk factors to develop updated prevention and treatment strategies. This nationwide case-control study in Taiwan analyzed data from the National Health Insurance Research Database, Birth Reporting Database, and Maternal and Child Health Database from 2010 to 2019. The study included 176,681 mother-child pairs with preterm births. We identified 2942 clinical EOS cases from 5535 diagnosed sepsis cases, excluding unlikely cases. A control group of 14,710 pr

Kaiser Permanente early-onset sepsis calculator as a safe tool for reducing antibiotic use among chorioamnionitis-exposed term neonates: Qatar experience.

Being an important cause of early-onset neonatal sepsis, clinical chorioamnionitis in the mother results in frequent laboratory workup and antibiotic use for the neonate. Neonatal intensive care units (NICUs) in Qatar follow the categorical approach recommended by the Centers for Disease Control and Prevention, USA, and all chorioamnionitis-exposed neonates receive antibiotics. Our project aimed to reduce antibiotic use among chorioamnionitis-exposed, asymptomatic term babies by adopting the early-onset sepsis calculator (EOSCAL). Reduction of blood culture and NICU stay duration were added as …

A cross-sectional study of clinical features of bacterial meningitis among neonates presumed to have sepsis in a tertiary hospital, Dar es Salaam, Tanzania.

Identifying meningitis among neonates is usually challenging given the non-specific presentation and overlap with neonatal sepsis. This study was aimed at determining the pertinent clinical features that would suggest bacterial meningitis among infants with signs of possible serious bacterial infection and their outcomes. This hospital-based cross-sectional study was conducted among neonates presenting with clinical features of sepsis admitted at Muhimbili National Hospital (MNH) between May and December 2015. Detailed clinical features, blood cultures, and cerebrospinal fluid were obtained. T…

Association of maternal infections, antibiotic use, and cesarean delivery with the risk of early-onset sepsis: a nationwide population-based study in full-term neonates.

BACKGROUND: Neonatal sepsis remains a significant cause of morbidity and mortality among newborns worldwide. Although the implementation of intrapartum antibiotic prophylaxis (IAP) has led to changes in the microbiological landscape of early-onset sepsis (EOS), the incidence among full-term neonates has not declined as expected. This underscores the ongoing need to identify and understand maternal and neonatal risk factors to inform more effective prevention strategies. METHODS: We conducted a nationwide, population-based matched case-control study using data from January 1, 2010, to December…

Elizabethkingia Infections - An Important Emerging Cause of Sepsis and Meningitis in Neonates and Infants: Case Series from North Batinah Region, Oman, and literature review.

Elizabethkingia species, particularly E. anophelis and E. meningoseptica, are gram negative bacilli known to be associated with neonatal infections. Although these bacteria are rare, they are highly concerning pathogens associated with severe sepsis, bacteraemia and meningitis in neonates and immunocompromised individuals. Managing Elizabethkingia infections presents significant challenges because of their multidrug resistance, including resistance to most β-lactam agents. They have been implicated in both nosocomial and community outbreaks worldwide, often with high mortality rates. In Oman,

Prophylactic therapy with human amniotic fluid stem cells improved survival in a rat model of lipopolysaccharide-induced neonatal sepsis through immunomodulation via aggregates with peritoneal macrophages.

BACKGROUND: Despite recent advances in neonatal care, sepsis remains a leading cause of mortality in neonates. Mesenchymal stem cells derived from various tissues, such as bone marrow, umbilical cord, and adipose tissue, have beneficial effects on adult sepsis. Although human amniotic fluid stem cells (hAFSCs) have mesenchymal stem cell properties, the efficacy of hAFSCs on neonatal sepsis is yet to be elucidated. This study aimed to investigate the therapeutic potential of hAFSCs on neonatal sepsis using a rat model of lipopolysaccharide (LPS)-induced sepsis. METHODS: hAFSCs were isolated as…

Clinical Relevance of Pathogens Detected by Multiplex PCR in Blood of Very-Low-Birth Weight Infants with Suspected Sepsis - Multicentre Study of the German Neonatal Network.

INTRODUCTION: In the German Neonatal Network (GNN) 10% of very-low-birth weight infants (VLBWI) suffer from blood-culture confirmed sepsis, while 30% of VLBWI develop clinical sepsis. Diagnosis of sepsis is a difficult task leading to potential over-treatment with antibiotics. This study aims to investigate whether the results of blood multiplex-PCR (SeptiFast®) for common sepsis pathogens are relevant for clinical decision making when sepsis is suspected in VLBWI. METHODS: We performed a prospective, multi-centre study within the GNN including 133 VLBWI with 214 episodes of suspected late on

Results of a two-center, before and after study of piperacillin-tazobactam versus ampicillin and gentamicin as empiric therapy for suspected sepsis at birth in neonates ≤ 1500 g.

OBJECTIVE: We changed from ampicillin and gentamicin (AG) to piperacillin-tazobactam (PT) for routine treatment of suspected early-onset sepsis. The rationale for this change included ototoxic and renal toxic effects of gentamicin, resistance to gentamicin in late-onset infections and emergence of ampicillin resistant Escherichia coli. A before and after study was designed before the start of PT administration to monitor whether PT was associated with altered outcomes within the 501 to 1500 g birth weight (Very Low Birth Weight) population. METHOD: Both unmatched and matched comparisons of AG

Contribution of nosocomial transmission to Klebsiella pneumoniae neonatal sepsis in Africa and South Asia: An observational study of infection clusters inferred from pathogen genomics and temporal data.

BACKGROUND: Klebsiella pneumoniae is the leading cause of sepsis among neonates in low- and middle-income countries (LMICs) in Africa and Asia, contributing substantially to the overall burden of antimicrobial-resistant infections and mortality among neonates globally. Pathogen sequencing has been used to investigate case clusters and confirm nosocomial transmission in a small number of neonatal units. Here we utilise pathogen sequence data to estimate the fraction of K. pneumoniae neonatal sepsis attributable to nosocomial transmission in African and South Asian countries. METHODS AND FINDIN…

Diagnostic Role of Complete Blood Count (CBC) and C-reactive Protein (CRP) in Suspected Early-Onset Sepsis and Antibiotic Stewardship in the Neonatal Intensive Care Unit (NICU).

BACKGROUND: Early-onset sepsis (EOS) is a major cause of neonatal morbidity and mortality, but its diagnosis remains challenging because early clinical manifestations are nonspecific and blood culture results are not immediately available. Consequently, many neonates receive empirical antibiotics despite subsequently having negative blood cultures, increasing unnecessary antibiotic exposure and prolonging neonatal intensive care unit (NICU) stay. Readily available laboratory markers such as complete blood count (CBC) parameters and C-reactive protein (CRP) are commonly used to support clinical
