[Anesthetic management for a patient with Huntington disease].

A 41-year-old man (169 cm, 48 kg), having a 10 year history of Huntington disease, was presented for percutaneous endoscopic gastrostomy because of repeated aspiration episodes. He had suffered from choreiform movements, misswallowing and progressive mental deterioration. Midazolam 2 mg i.v. was given on transferring the patient to the operating room. On arriving, the patient was somnolent but responded to call. BIS index was 55. Anesthesia was induced with thiopental 120 mg i.v. and fentanyl 100 microg i.v. followed by vecuronium 4 mg i.v. After tracheal intubation, anesthesia was maintained

Calcium sensitivity of neurotransmitter release differs at phasic and tonic synapses.

The efficacy of synaptic transmission varies greatly among synaptic contacts. We have explored the origins of differences between phasic and tonic crustacean neuromuscular junctions. Synaptic boutons of a phasic motor neuron release three orders of magnitude more quanta to a single action potential and show strong depression to a train, whereas tonic synapses are nearly unresponsive to single action potentials and display an immense facilitation. Phasic and tonic synapses display a similar nonlinear dependence on extracellular [Ca2+]. We imposed similar spatially uniform intracellular [Ca2+] (

The British Journal of Anaesthesia. An informal history of the first 25 years.

In 1961, some 7 months after starting anaesthesia in the Leeds General Infirmary, I took out a subscription to the British Journal of Anaesthesia. It cost Pound Sterling3.15s.0d (Pound Sterling3.75) a year. The publishers John Sherratt and Son of Altringham) sent me the back numbers from the start of that year. I first had a paper published in the journal in 1965; first refereed a paper in 1969; joined the editorial board in 1975; and lasted there until 1998. The following account of the early years of the journal derives from the journal itself, and from records, letters and minutes of meetin

[Paraplegia after surgery for aneurysm of the descending thoracic aorta].

Postoperative paraplegia caused by ischemic injury of the spinal cord is the most disabling complication of thoracoabdominal surgery, particularly when repair of the descending thoracic aorta is involved. We describe the case of a 59-year-old man who underwent emergency surgery for placement of a Dacron prosthesis to repair a ruptured descending thoracic aorta aneurysm, using an aortic cross-clamping technique plus aortic-femoral partial bypass with normothermia and an ischemic time of 165 minutes. The early postoperative course included complete spinal syndrome with motor and sensory loss bel

Rescue of exocytosis in botulinum toxin A-poisoned chromaffin cells by expression of cleavage-resistant SNAP-25. Identification of the minimal essential C-terminal residues.

Botulinum neurotoxin (BoNT) types A and B selectively block exocytosis by cleavage of SNAP-25 and synaptobrevin, respectively; in humans, many months are required for full recovery from the resultant neuromuscular paralysis. To decipher the molecular basis for such prolonged poisoning, intoxication in adreno-chromaffin cells was monitored over 2 months. Exocytosis from BoNT/B-treated cells resumed after 56 days because of the appearance of intact synaptobrevin. However, inhibition continued in BoNT/A-treated cells, throughout the same interval, with a continued predominance of cleaved SNAP-25-

[Supplementation of DL-alpha-tocopherol acetate in children with chronic cholestasis and vitamin E deficiency].

OBJECTIVE: To determine the oral supplement doses of dl-alpha-tocopheryl-acetate to maintain normal serum alpha-tocopherol concentrations in children with chronic cholestasis and vitamin-E deficient. BACKGROUND: Malabsorption and deficiency of vitamin E in children with chronic cholestasis causes a progressive, neuromuscular degeneration at approximately 18-24 months of life, if left untreated. Using prompt treatment, it can be completely prevented and reversed to normal. METHOD: Longitudinal, prospective and comparative study was performed on consecutive sixty vitamin E deficient children w

Management of hypophosphatemia.

The etiology, clinical presentation, and management of hypophosphatemia are reviewed. Phosphorus is a major intracellular anion and plays an important role in many biochemical pathways relating to normal physiologic functions. Approximately 60 to 90% of the 1 to 1.5 g of daily dietary phosphorus intake is absorbed, and of that amount, about two thirds is excreted in the urine. The overall incidence of hypophosphatemia is about 2 to 3% of all hospitalized patients. Factors associated with hypophosphatemia include phosphate-binding antacid therapy, nasogastric suction, liver disease, sepsis, alc

Outpatient versus inpatient initiation of home mechanical ventilation in patients with amyotrophic lateral sclerosis - a protocol for a randomised trial.

INTRODUCTION: In-home non-invasive ventilation (NIV) is associated with prolonged life and improved quality of life in patients with hypoventilation due to amyotrophic lateral sclerosis (ALS). The initiation of NIV is scheduled for a 1-2-night hospital stay. Telemedicine enables remote monitoring and adjustment of respiratory treatment. These possibilities should be examined to improve patients' experiences and adherence to treatment while freeing up resources in the healthcare system. We hypothesise that outpatient initiation of NIV combined with close telemonitoring in patients with ALS is n

Management of spinal muscular atrophy: nursing clinical commentary and best practices update.

BACKGROUND: Spinal muscular atrophy (SMA) is a rare neuromuscular disorder that has seen a paradigm shift in its management due to early diagnostic strategies, disease-modifying therapies, and integrated care models. Despite these advances, the nursing role - essential across the care continuum - remains underrepresented in clinical protocols and guidelines. MAIN BODY: This Clinical commentary, authored by Italian nurses with direct expertise in SMA care, outlines the key responsibilities, practices, and procedural frameworks that define specialized nursing contributions. Nurses are involved

An Extensible Platform for Measurement and Modification of Muscle Engagement During Upper-Limb Robot-Facilitated Rehabilitation.

Populations with upper-limb neuromotor disabilities could greatly benefit from robot-mediated therapy, but the heterogeneity of these disorders, combined with unresolved questions around neuroplasticity mechanisms, hinders the standardized prescription of effective rehabilitation tasks and corresponding robot behaviors. Building on consensus that active neuromuscular engagement is vital for therapy, we introduce a novel, extensible rehabilitation robot platform to directly measure (and, ultimately, modify) user engagement at the muscle level. The system integrates surface electromyography to m

A study on adverse events of botulinum toxin type A in real-world applications: A FAERS database analysis.

AIMS: Botulinum toxin type A (BoNT-A) is widely used for its efficacy in treating a variety of neuromuscular disorders. However, data on its safety and adverse events (AEs) in real-world settings remain limited. The Food and Drug Administration Adverse Event Reporting System (FAERS) database provides an invaluable resource for evaluating the safety of BoNT-A. METHODS: We used four methods to evaluate the disproportionality of AEs associated with BoNT-A: the reporting odds ratio (ROR), the proportional reporting ratio (PRR), the Bayesian confidence propagation neural network (BCPNN), and the m

A Comprehensive Approach to Myasthenia Gravis With Thymoma: A Case Report Highlighting Management of Myasthenic Crisis and Post-Crisis Care.

Myasthenia gravis (MG) is a chronic autoimmune neuromuscular disorder that leads to muscle weakness and rapid fatigue, often exacerbated by thymoma, a tumor of the thymus gland. This case report presents the clinical management of a 40-year-old female with MG complicated by thymoma, who presented with a severe myasthenic crisis. The patient required intensive care, including respiratory support, plasmapheresis, and immunosuppressive therapy. Initial evaluations confirmed the diagnosis of MG through the detection of high levels of anti-acetylcholine receptor antibodies, and a CT scan revealed a

Management of a 25-Year-Old Female Patient With Limb-Girdle Muscular Dystrophy With Physiotherapy: A Case Report.

Limb-girdle muscular dystrophy (LGMD) is a collection of neuromuscular diseases that develop gradually and are rare, genetically, and clinically diverse. The weakness in muscles affecting the shoulder and pelvic girdles is a defining feature of LGMD. Calpainopathy is another name for limb-girdle muscular dystrophy type 2A (LGMD2A). Limb-girdle muscular dystrophy type 2A results from alterations in the calpain-3 (CAPN3) gene, which results in a CAPN3 protein shortage. Gower's sign is most commonly found in LGMD2A. The prevalence ranges from one person in every 14,500 to one in every 123,000. We

Revisiting the compound muscle action potential (CMAP).

The compound muscle action potential (CMAP) is among the first recorded waveforms in clinical neurography and one of the most common in clinical use. It is derived from the summated muscle fiber action potentials recorded from a surface electrode overlying the studied muscle following stimulation of the relevant motor nerve fibres innervating the muscle. Surface recorded motor unit potentials (SMUPs) are the fundamental units comprising the CMAP. Because it is considered a basic, if not banal signal, what it represents is often underappreciated. In this review we discuss current concepts in th

Changes in countermovement jump force-time characteristic in elite male basketball players: A season-long analyses.

Basketball is a sport that is characterized by various physical performance parameters and motor abilities such as speed, strength, and endurance, which are all underpinned by an athlete's efficient use of the stretch-shortening cycle (SSC). A common assessment to measure SSC efficiency is the countermovement jump (CMJ). When performed on a force plate, a plethora of different force-time metrics may be gleaned from the jump task, reflecting neuromuscular performance characteristics. The aim of this study was to investigate how different CMJ force-time characteristics change across different pa

[Contribution of a hospital pharmacy team to critical care of patients infected with SARS-CoV-2].

With regard to the hospital drug supply chain, the safest system is the individual automated drug dispensing one provided by the pharmacy. For several years we have been trying to convince hospital decision-makers to set it up. In the meantime, to mitigate the risks of medication errors incurred by patients and caregivers, we have set up several work teams within the care units. These teams, made up of one pharmacist and one or two hospital pharmacy technicians, who notably manage the medicine cabinets in care units. The close collaboration with doctors and nurses developed over the years was
