Innovations in mental health care delivery.

In serious mental illness (SMI) even good community care does not usually make a major impact on clinical or social function, but patients and relatives prefer community to hospital care, and it tends to be cheaper. Any gains are lost if the required community services are not resourced, coordinated, and maintained indefinitely. A few SMI patients continue to need asylum under one roof. CPNs see more anxiety/depression than SMI. Their patients come increasingly from GPs, and they tend to work in practices with less need. Their cost-effectiveness is uncertain, although nurse behaviour therapist

Community drug abuse prevention in a Hmong village in Thailand.

The goal of this project was to implement a model of drug abuse prevention, drawing on health care, psychosocial and legal principles, in a selected village community in Thailand. Primary, secondary, and tertiary prevention strategies were used within a nursing conceptual framework that included the interactions among the person, drugs, and environment. An initial community survey revealed 112 addicted individuals and identified many atrisk groups, such as children of addicted parents, adolescents, and women. Primary health care and dental health services were provided in the community, as wel

Exploring Nursing Care Coordination Through Swanson's Theory of Caring: Perspectives of Families of Children and Youth With Special Health Care Needs.

PURPOSE OF STUDY: Caring for children and youth with special health care needs can be overwhelming for families. Higher levels of need often require more extensive and coordinated support. The burden of caring for children with complex health care needs can result in negative effects for parents and caregivers over time, thus increasing the risk of poor physical, emotional, and social well-being. Nursing care coordination is an evidence-based intervention that provides families with guidance and support by delivering safe and effective care. The quality of coordination, however, can profoundly

An Introduction to the Semantics and Statistics Behind the Firearm Policy Debates.

It is impossible to fully understand why the United States has consistently failed to protect its citizens from firearm violence until one understands some of the key discrepancies that exist at the center of the firearm policy debate. Differences in language, data categorization, and research related to firearms and their impacts in the United States contribute to confusion and debate between firearm policy advocates and opponents, ultimately stalling progress toward some common goals. As frontline health professionals, emergency nurses must be aware of these nuances in order to be informed a

Use of a hydrogel dressing for management of a painful leg ulcer.

This case study is of an 82-year-old lady who was widowed and lives alone in a council house. A left lateral leg ulcer had developed over the gaiter area, and the community team were asked to assess in 2004. The main issues were her inability to tolerate compression (even when reduced) because of the pain. Nevertheless, the community nurses had tried very hard with compression, using different compression techniques at different times to try to encourage her to persevere. The nurses felt they were running out of ideas and therefore the tissue viability nurse (TVN) was asked to assess the wound

The Lived Experiences of Patients and Health Care Providers During ED Triage in Saudi Arabia.

METHODS: This qualitative descriptive study explored triage experiences in a tertiary emergency department in Riyadh, Saudi Arabia. Purposive sampling recruited 42 participants: 21 nurses, 11 physicians, and 10 patients. Data were generated through 35 semistructured individual interviews and 1 nurse focus grou

Depressive Symptom Patterns and Digital Screen Exposure Among Adults in Qatar: A Cross-Sectional Study With Implications for Psychiatric and Mental Health Nursing Practice.

BACKGROUND: Depression is a major global health concern, yet population-based evidence on depressive symptom patterns and associated lifestyle factors in Qatar remains limited. Understanding symptom distribution is important for informing nurse-led prevention and early identification strategies. METHODS: A cross-sectional secondary analysis was conducted using data from 1098 adults enrolled in the Qatar Biobank. Depressive symptoms were assessed using the nine-item Patient Health Questionnaire. Descriptive statistics summarized total scores, severity categories and individual symptoms. Bivari

How Recovery Attitudes Relate to Views on Coercive Interventions and Interpersonal Characteristics in Psychiatric Nurses: A Quantitative Cross-Sectional Study.

AIMS: To examine the associations among psychiatric nurses' attitudes toward mental health recovery, attitudes toward coercive interventions, and interpersonal care characteristics, particularly the dimensions of dominance and coldness. DESIGN: Cross-sectional study. Intergroup differences were analysed using t-tests and analysis of covariance, with Bonferroni corrections applied, whereas str

An estimation of the effect of finding nurses helpful on early breastfeeding exclusivity mediated via breastfeeding self-efficacy, using a counterfactual approach and G-computation.

BACKGROUND: Exclusive breastfeeding is recommended for at least the first six months of life, however in South Africa only 32% of mothers are exclusively breastfeeding to six months and discontinuation happens early. Nurses are a key resource for promoting breastfeeding self-efficacy and thus exclusivity, yet mothers often report nurses to have poor attitudes, which deter health seeking behaviour. This study aimed to infer whether perceived support from nurses immediately postpartum was causally related to breastfeeding exclusivity, and specifically whether this effect was mediated via breastf

The impact of self-efficacy on health-related quality of life among community-dwelling older adults with chronic diseases: the mediating role of self-management behaviour.

OBJECTIVE: To examine the relationships among self-efficacy, self-management behaviour and quality of life in community-dwelling older adults with chronic conditions, to test the mediating effect of self-management behaviour on the relationship between self-efficacy and quality of life, and to provide a theoretical basis for the development of a community-based chronic disease management nursing model led by specialist nurses. METHODS: A convenience sampling method was used to recruit 189 older patients with chronic diseases from a community health service centre in Jiange County, Guangyuan C

Guidelines in Practice: Prevention of Perioperative Pressure Injury.

Immobility and lack of sensation during perioperative procedures increase the risk of patients developing pressure injuries (PIs). Such injuries can cause pain and result in serious infections, thereby leading to increased health care costs. The recently developed AORN "Guideline for prevention of perioperative pressure injury" provides perioperative nurses and leaders with applicable recommendations for preventing these injuries. In addition to a brief overview of a health care facility's interdisciplinary perioperative PI prevention program, this article discusses a variety of concepts relat

Developing community mental health services for children in South Africa.

As a result of South Africa's Apartheid history, mental health care for black people, especially in rural areas, has been grossly inadequate and even non-existent in many areas. Children have been severely neglected in this regard. This paper describes an attempt by clinical psychologists to develop a community intervention programme for children with emotional problems. From their hospital base the authors set out, on a monthly basis, to outlying areas up to 250 km away to (1) train primary care nurses and other personnel in the basic techniques of identifying and dealing with uncomplicated p

Stress--nurses dealing with family members.

Family caregivers directly care for the older family member, and network with other family members and health and community agencies. Factors contributing to caregivers' stress include impaired family relationships, depression and chronic health problems, lack of preparation, lack of personal time, and moderate support and respite services. The role of the nurse in health promotion includes values clarification, goal setting, decision making, planning behavior change, and social support building. Support programs may include the conceptualization of support as varying in duration and intensity
