Lisfranc Injuries in the Athlete.

LisFranc injuries in the active, athletic, and military populations can be devastating injuries. There has been much debate over open reduction and internal fixation versus arthrodesis as primary treatment in these injuries. This article aims to present the existing evidence-based medicine to help guide appropriate treatment in this population. With the introduction of flexible fixation, the variety of ways to address these injuries surgically has expanded.

Resorbable Versus Titanium Hardware for Rigid Fixation of Pediatric Upper and Midfacial Fractures: Which Carries a Lower Risk Profile?

PURPOSE: Titanium associated risks have led to interest in resorbable hardware for open reduction and internal fixation (ORIF) of pediatric facial fractures. This study aims to systematically review and compare the outcomes of titanium/resorbable hardware used for ORIF of upper/midfacial fractures to determine which hardware carries a higher complication rate in the pediatric patient. METHODS: Studies published between 1990 and 2020 on the ORIF of pediatric upper/midfacial fractures were systematically reviewed. A retrospective institutional review was also conducted, and both arms were compi

Surgical Management of a Displaced Posterior Column Pelvic Fracture Involving the Triradiate Cartilage in a 15-Year-Old Adolescent Boy: A Case Report.

CASE: A 15-year-old adolescent boy sustained a displaced acetabular fracture involving the ilioischial physis of the triradiate cartilage after a ground-level fall. The patient was managed operatively with open reduction and internal fixation and was followed for 9 months postoperatively. At 4 months postoperatively, the patient had resumed full weight bearing on the injured extremity and was participating in sports with no difficulties. CONCLUSION: Pediatric pelvic fractures through the triradiate cartilage are rare, and no consensus exists on the treatment of these injuries. Previous author

Treatment of Navicular Stress Fractures With an Algorithmic Approach.

BACKGROUND: Navicular stress fractures are becoming increasingly more common. There is no universal consensus on treatment. We provide an algorithm that we feel will be useful in determining treatment. METHODS: A retrospective study was performed on all patients having operative treatment of navicular stress fractures during a 10-year period. Acute fractures were treated with open reduction internal fixation. Chronic fractures greater than 3 months were treated with open reduction and internal fixation (ORIF) and iliac crest bone grafting. Chronic fractures with evidence of sclerosis, avascul

Condylar remodeling after osteosynthesis of fractures of the condylar head or close to the temporomandibular joint.

PURPOSE: Condylar head fractures (CHFs) are increasingly treated by open reduction and internal fixation (ORIF). However, there are no reports on the three-dimensional postoperative volumetric transformation of the condyle, especially with regard to fragmented cases. Protruding hardware can lead to severe complications, so the goal of this study was to examine the amount of condylar bony resorption occurring after ORIF. METHODS: Included were surgically treated CHFs with eligible cone beam computer tomography (CBCT) datasets immediately after ORIF (T1) and after implant removal (T2), plus fra

Key points in surgical management of mandibular condylar fractures.

Mandibular condylar fractures are among the most common facial fractures and some of the most difficult to manage. Opinions about the management of mandibular condylar fractures differ among surgeons. With the implementation of new technology, an increased understanding of fracture management, and better functional and morphological outcomes reported in the literature, open reduction and internal fixation is becoming many surgeons' preferred choice for the treatment of condylar fractures. Because surgical treatment of such fractures is complex, certain factors must be considered to achieve sat

A Novel Technique for Correcting Radial Length and Translation in Distal Radius Fractures.

We describe a novel technique in correcting distal radius fractures deformed with significant shortening or coronal plane translation in both the acute or chronic setting. The technique involves using a modified push-pull device to assist the surgeon in correcting length and/or translation of the articular block without the use additional hardware outside of the volar plate.

Low interobserver and intraobserver reliability using the Matta radiographic system for intraoperative assessment of reduction following acetabular ORIF.

INTRODUCTION: The system described by Matta for rating acetabular fracture quality of reduction following ORIF has been used extensively throughout the literature. However, the reliability of this system remains to be validated. We sought to determine the interobserver and intraobserver reliability of this system when used by fellowship-trained pelvic and acetabular surgeons to evaluate intraoperative fluoroscopy. METHODS: This is a retrospective evaluation of a prospectively collected acetabular fracture database at an academic level I trauma center. The quality of reduction of all acetabula

Unplanned Return to the Operating Room in Upper-Extremity Surgery: Incidence and Reason for Return.

PURPOSE: Complications after upper-extremity surgery are generally infrequent. The purpose of this study was to assess the rate of early unplanned return to the operating room (URTO) within 3 months after surgery) in upper-extremity surgical procedures. Our hypotheses were that the rate of URTO in upper-extremity surgery would be low and that surgically treated fractures would be at greatest risk for complications. METHODS: We performed a retrospective review of all upper-extremity surgical procedures performed at a large academic practice of fellowship-trained hand surgeons over a 5-year per

Accessory Anterolateral Talar Facet Syndrome After Treatment of Neglected Fracture of the Lateral Process of the Talus: A Case Report.

Fracture of the lateral process of the talus has often been missed or confused with lateral ankle sprain, and this can lead to long-term untreated cases of nonunion, malunion, and subtalar joint osteoarthritis. In a review of the published data, accessory anterolateral talar facet syndrome was not found after treatment of nonunion of the lateral process of the talus fracture. This report presents the case of a 40-year-old male snowboard instructor who suffered from a neglected fracture of the lateral process of the talus and was treated with open reduction using an iliac autogenous bone graft.

Increased Lateral Calcaneal Body and Subtalar Joint Visualization Utilizing a Modified Sinus Tarsi Approach.

BACKGROUND: The traditional sinus tarsi approach (TST) for open reduction and internal fixation (ORIF) of calcaneus fractures has gained popularity because of adequate fracture exposure with lower wound complication rates. Visualization, as compared with more extensile approaches, can be limited, however. This study introduces a modified sinus tarsi approach (MST) that extends the TST incision approximately 2 cm posterodorsally as well as 2 cm anteroplantarly, to enhance exposure. The aim of this study is to quantify the amount of exposure obtained with the traditional sinus tarsi approach and

Medial Hoffa Fracture: A Case Report and Literature Review of Approach and Management.

BACKGROUND Hoffa fractures are an uncommon form of coronal fracture that impact the femoral condyle. As a result, they are not very prevalent. It is necessary to perform anatomical reduction and rigorous fixation on these fractures; however, there is no consensus among medical professionals on the surgical procedure and implant that would be the most successful in treating these fractures. CASE REPORT A 50-year-old woman who had poliomyelitis in her right lower limb presented with a displaced medial Hoffa fracture of her left knee. She had fallen and was suffering from poliomyelitis. The traum

The Operative Treatment of Scapula Fractures: An Analysis of 10,097 Patients.

BACKGROUND: The indications for operative treatment of scapula fractures have been debated over the past decade. Our purpose was to determine 1) the incidence and trends in the operative treatment of scapula fractures, 2) the incidence of conversion from operative fixation to total or hemi-shoulder arthroplasty (THSA) and 3) rates of associated injuries in scapula fractures. We hypothesized that the operative treatment of scapula fractures is increasing over time and that scapula fractures treated with open reduction and internal fixation (ORIF) would have increased risk for conversion to THSA

Open reduction and internal fixation of the posterior malleolus fragment via the posterolateral approach is radiologically superior to 'A to P' screw fixation.

PURPOSE: Functional outcome in trimalleolar fractures is largely correlated to the reduction of the posterior fragment. Until recently, fixation was mainly performed for large fragments, by percutaneous anterior to posterior ('A to P') screw placement after closed reduction. Nowadays, ORIF via a posterolateral approach seems to gain in popularity. The aim of this study was to compare the postoperative photographs of operated trimalleolar fractures after either fracture treatment method, for fracture diastasis and step-off. MATERIAL AND METHODS: All consecutive patients with trimalleolar fract

New Minimally Invasive Intraoral Procedure for Condylar Fractures: Clinical Presentation and Considerations on Current Techniques.

Mandibular fractures are the third most prevalent maxillofacial traumatic events. Surgical approaches to the condyle are a debated topic. This study describes a mini-invasive technique for condylar fracture reduction. The patient of this study suffered multiple traumatic injuries including a carotid artery dissecting aneurysm, which contraindicated the standard open reduction and internal fixation technique. The novel minimally invasive technique involves intraoral access and fracture fragment realignment using a periosteal elevator, a molar occlusal splint, and intermaxillary fixation after i

Management of Missed Type-2 Monteggia Fracture Equivalent in a 9-Year-Old Child: A Case Report.

CASE: A 9-year-old girl presented with a painful stiff elbow consequent to an injury she had sustained 4 months ago. The radiographs were suggestive of a type 2 Monteggia equivalent injury. She underwent an open reduction of the radial neck fracture and proximal ulna extension osteotomy to restore radiocapitellar alignment. At one-year follow-up, the child had a painless elbow with a good functional outcome. CONCLUSION: Monteggia lesions can be frequently missed and result in significant morbidity. Obtaining appropriate radiographs for a comprehensive understanding of the injury and proper su

Minimum Clinically Important Difference in Short Musculoskeletal Function Assessment: What Change Matters in Ankle Fractures.

Accurate interpretation of the clinical significance of patient-reported outcome (PRO) research requires determination of the threshold where a difference in PRO score represents a clinical benefit to patients, termed the minimum clinically important difference (MCID). The Short Musculoskeletal Function Assessment (SMFA) is one of the most commonly utilized PRO tools in orthopaedics. However, to date, no MCID has been determined. The purpose of this study was to define the MCID for the SMFA. A prospectively collected ankle fracture outcomes registry was reviewed between 2014 and 2016. Inclusio
