We describe a versatile apparatus for optical observations of experimental processes at temperatures down to 0.1 K. The cooling is achieved by a wet cryostat with a 3He-4He dilution refrigerator on a vibrationally isolated platform, capable of continuous rotation at an angular velocity of up to 3 rad s-1. The illumination light beam from lasers on a non-rotating optical table at room temperature is introduced via an optical fiber. The images are transferred to the intensified camera at room temperature through a coherent bundle of 105 optical fibers, limiting the spatial resolution to ∼30μm, d…

Glaucoma is known as an optic neuropathy prone to progression that determines characteristic not only structural (loss of the ganglion cells as well as their axons) but also functional defects (visual field loss). Objective: To evaluate the possibility of applying ganglion cell complex analysis (GCC) in patients who associate ocular hypertension with tilted disc and marked peripapillary atrophy. Methods: In order to evaluate its components, GCC can be investigated using the Optical Coherence Tomography (OCT) revealing: ganglion cell layer (cells bodies), inner plexiform layer (dendrites and sy

Optical coherence tomography is a simple, high-resolution technique to quantify the thickness of retinal nerve fiber layer (RNFL). Previous studies have shown that degenerative changes occur in optic nerve fibers and are manifested as thinning of RNLF in patients with Alzheimer's disease (AD). However, there are no studies on the thickness of the RNLF in other types of dementia, such as dementia with Lewy bodies and dementia associated with Parkinson's disease. In this study, patients fulfilling diagnostic for AD (n = 10), dementia with Lewy bodies (n = 10), dementia associated with Parkinson'

Reorganization involving residual visual pathways with unilateral damage to the primary visual cortex was previously described. Using diffusion tensor imaging, we measured water diffusion-related changes in the optic radiation contralateral to occipital lobe ablation in children with intractable epilepsy. We studied 10 children who had undergone a resection of the unilateral occipital cortex and 13 control subjects. Diffusion tensor imaging was acquired using a 1.5 Tesla magnetic resonance scanner. Fiber bundles representing optic radiation were tracked. Diffusion parameters included mean frac

Infrared confocal scanning laser photography of a patient with long-standing optic tract lesion revealed a homonymous hemianopic hyporeflective image contralateral to the visual field defect. Spectral domain optical coherence tomography showed thinning of the retinal nerve fiber and retinal ganglion cell layer and thickening of the inner nuclear layer (with microcystic degeneration) in the macular area, matching the infrared image. Hyporeflective image on infrared laser photography is associated with retinal degeneration secondary to anterior visual pathway disease and, when located in homonym

A 24-year-old patient with Friedreich's ataxia presented with advanced visual loss due to optic nerve atrophy. After interdisciplinary consultation and after obtaining informed consent, an off-label therapy with the N-methyl-D-aspartate (NMDA) antagonist memantine was initiated. In a 1-year follow-up no further loss of the nerve fiber layer could be detected by optical coherence tomography (OCT) and visual acuity remained stable. Despite the limitations of this single and time limited case observational study, memantine should be discussed as an option for treatment of acute optic nerve atroph

A 48-year-old woman was struck on the right inferior orbital rim by a gardening device and immediately developed complete visual loss in the right eye. Clinical and imaging evaluations failed to disclose any damage to the globe or optic nerve or to their nearby soft tissues and bones. Within months, ipsilateral optic disc pallor, a nerve fiber bundle visual field defect, and a persistently subnormal visual acuity developed. We attribute the visual loss to indirect optic nerve injury. To our knowledge, blunt injury to the inferior orbital rim has not been reported as a cause of this phenomenon.

The optic nerve head (ONH) region at the posterior pole of the eye is supported by a fibrous structure of collagen fiber bundles. Discerning how the fibrous structure determines the region biomechanics is crucial to understand normal physiology, and the roles of biomechanics on vision loss. The fiber bundles within the ONH structure exhibit complex three-dimensional (3D) organization and continuity across the various tissue components. Computational models of the ONH, however, usually represent collagen fibers in a homogenized fashion without accounting for their continuity across tissues, fib

PURPOSE: To investigate the reflectivity of the structural optical coherence tomography images of the inner choroid as it relates to potential structural composition.
METHODS: The reflectivity of slab images 29 to 49 µm and 45 to 65 µm below the retinal pigment epithelium, obtained with the Zeiss Plex Elite 9000, was evaluated. The mean and SD of a group of subjects with no ocular disorders were determined. Binarization of the images was performed and a threshold level established at the mean plus one SD for each slab depth. The proportion of area binarized was evaluated with generalized esti

We obtained quantitative measurements of capillary numbers, areas, and diameters in atrophic (pale) and normal primate optic nerve heads. The number of capillaries per square millimeter in pale optic disks was not significantly different from that in normal optic disks. Because the loss of all nerve fibers leads to a 50% decrease in nerve head substance, capillaries must atrophy to maintain a constant relationship between capillary number and tissue volume. The mean size of individual capillaries in atrophic nerve heads was smaller than normal, leading to a decrease of more than 27% in the per

A 55-year-old man with pulmonary Mycobacterium avium intracellulare infection developed decreased vision to 3/200 in the right eye, and 20/200 in the left eye, 11 months after starting ethambutol, rifampin, and isoniazid. A diagnosis of presumed ethambutol optic neuropathy was made, and the medications were discontinued. Visual acuity gradually improved to 20/30 and 20/70 over a period of 34 months. Despite improved central vision and visual field, the patient developed progressive bilateral optic disc cupping, disc pallor, and diffuse nerve fiber layer loss on optical coherence tomography. Th

Partial optic nerve avulsion (ONA) secondary to finger gouging is an uncommon but devastating injury. A 21-year-old man who had an acute vision loss after accidentally getting poked by himself in his right eye when he fell down during jogging is reported. The patient was diagnosed with partial ONA. Magnetic resonance imaging revealed intact optic nerve. Optical coherence tomography (OCT) revealed deep cavity at the inferior-temporal half of the optic disc. Retinal nerve fiber layer thickness was also thin at the inferior quadrant with circumpapillary OCT scan. Visual field test and electrophys

We show that myelin, the insulation wrap of nerve fibers, can couple laser light, thus behaving as a single-cell optical device. The effect was employed to map distinct myelin regions based on the coupling efficiency. Raman spectra acquisition allowed us to simultaneously understand the underlying microscopic differences in the membrane lipid ordering degree. The described method potentially provides new capabilities in myelin-associated disease studies and can be used as a handy tool for myelin structure investigation in combination with other methods.

A rationale is presented for designing screening-type eye examinations to document visual capabilities for specific jobs or changes in visual function following exposure to specific ocular hazards. Possible applications to clinical situations are also discussed. A battery of tests that includes contrast (glare) sensitivity, distortions in macular imaging, and color vision is detailed for use with three populations: automobile driver's license applicants, visual display operators, and laser users, including military as well as nonmilitary installers and repairers of optical fibers for communica…

PURPOSE: To investigate if a congenital anomaly of the head of the optic nerve like such as tilted disc can be a risk factor for the development of optic disc drusen.
METHODS: The study was performed retrospectively on the files of 47 patients with optic disc drusen. The diagnosis was confirmed by fluorescein angiography and B-scan ultrasonography. The authors examined the fundus photographs and the fluorescein angiographies of these patients looking for the presence of tilted discs.
RESULTS: Two of the 47 patients with optic nerve drusen had tilted discs as well, about twice the expected ra

Standardized perimetry and nerve fiber layer and color fundus photography were performed annually on 1344 eyes with elevated intraocular pressures. In 83 eyes, glaucomatous field defects developed that met rigid criteria on manual kinetic and suprathreshold static perimetry. Individual nerve fiber layer photographs were read by two masked observers. The more sensitive of the two identified nerve fiber layer defects in 88% of readable photographs at the time field loss first occurred; 60% (6/10) of eyes already had nerve fiber layer defects 6 years before field loss. In contrast, the nerve fibe
