Vertebroplasty for metastasis.

Vertebral metastases are frequently asymptomatic; however, the occurrence of a pathological (micro-)fracture may be associated with unremitting pain, instability, and even kyphoscoliotic deformity and require prompt and effective treatment. In symptomatic patients, the beneficial effect of conservative therapies requires too much time. Vertebroplasty (VP) may be an additional or even alternative local treatment modality for such patients. Currently, up to 80% of VP patients report important relief of metastasis-related pain. Ongoing deformity of the vertebral body is avoided. Morbidity rates a

Pediatric supracondylar humerus fractures: the anterior approach.

OBJECTIVES: To evaluate the efficiency of the anterior approach for displaced supracondylar pediatric humeral fractures. DESIGN: A retrospective analysis. SETTING: Clinical and outpatient clinic care. PATIENTS: The anterior approach group consisted of twenty-six patients. The historical group consisted of thirty-two patients. All had a completely displaced extension-type supracondylar humerus fracture (Gartland Type III). All were operated within six hours of admission. All patients were contacted; twenty-five of the twenty-six and twenty-nine of the thirty-two patients were re-examined at

[Classification of results of treatment of congenital clubfeet].

The paper presents the comparison of late results of surgical treatment of congenital clubfeet using a number of assessment classifications. 61 feet in 46 patients treated between 1979-1990 with a mean follow-up time of 15.7 years. Strong correlations were noted between the various classifications, but none was found "ideal" or particularly useful in everyday clinical practice.

[Anterior hyperostosis of the cervical spine: diagnostic studies and surgical therapy].

BACKGROUND: Dysphagia due external compression by anterior hyperostosis of the cervical spine is rare. The diagnosis may be established by conventional X-ray of the spine, esophagogram, and CT. PATIENTS: We operated on three patients with large anterior osteophytes from C3 to C7. In two cases morphologic changes of the cervical spine were the main cause of dysphagia. One patient with progressive hypopharynx cancer had hyperostosis of cervical spine as secondary findings. RESULTS: The patients were asymptomatic, post-operatively. CONCLUSIONS: Cervical osteophytes can be detected in 20-30% of

Can You Stall a Baclofen Pump During a Magnetic Rod Lengthening?

INTRODUCTION: Baclofen pumps are susceptible to magnetic forces because of the internal drug delivery mechanism. Magnetically controlled growing rods, sometimes implanted in the patients who have pumps, are lengthened using a device that generates a magnetic field. No previous study has determined if the external remote controller (ERC) can stall a baclofen pump during a rod lengthening. METHODS: Two baclofen pumps were placed in proximity to the ERC while it generated a magnetic field. This process was repeated with variations in the distance between pump and ERC, speed (dose) of the pump, l

The Surgical Management of Osteochondritis Dissecans of the Knee in the Skeletally Immature: A Survey of the Pediatric Orthopaedic Society of North America (POSNA) Membership.

BACKGROUND: While the characteristics of osteochondritis dissecans (OCD) of the knee that require surgery to heal have been described, several surgical techniques/procedures exist with no consensus established regarding timing of treatment and specific surgical intervention. In this study, we aim to determine current trends in surgical treatment for OCD lesions in the skeletally immature who have failed 6 months of nonoperative management by surveying a large cohort of orthopaedic surgeons. METHODS: An electronic survey designed using REDCap to capture surgeon treatment preferences for OCD le

Is PLC repair of a peel-off femoral lesion an effective option in a multiligament setting?

PURPOSE: Treatment of posterolateral corner (PLC) injury in the multiligament-injured knee is still controversial. The aim of this study was to investigate the clinical outcome of acute or sub-acute surgical repair of Type 1 and 2 PLC peel-off lesions in a multiligament setting. METHODS: Mini-open direct repair of the PLC was performed in 13 patients. Combined PCL, ACL and MCL injuries were simultaneously managed. Telos valgus and varus stress radiographs at 30° of flexion with 150 N load were used to investigate medial and lateral joint opening. Posterior stress radiograph with 150 N load wa

The Ponseti method of treatment for clubfoot in Brazil: barriers to bracing compliance.

BACKGROUND: Clubfoot is the most common extremity birth defect. It causes the feet of affected individuals to point inward and downward, preventing them from walking normally. Neglected clubfoot causes disabilities that result in a lack of social integration, creating a psychological and financial burden for the family and community. Clubfoot has been effectively treated through the Ponseti method, a treatment utilizing serial casts to correct the deformity followed by use of an abduction brace for approximately 2-4 years. sustained use of the brace is necessary to prevent relapse and ensure a

Little-known Swiss contributions to the description, diagnosis, and surgery of lumbar disc disease before the Mixter and Barr era.

The understanding of lumbar spine pathologies made substantial progress at the turn of the twentieth century. The authors review the original publication of Otto Veraguth in 1929 reporting on the successful resection of a herniated lumbar disc, published exclusively in the German language. His early report is put into the historical context, and its impact on the understanding of pathologies of the intervertebral disc (IVD) is estimated. The Swiss surgeon and Nobel Prize laureate Emil Theodor Kocher was among the first physicians to describe the traumatic rupture of the IVD in 1896. As early a

Separating the chicken from the egg: an attempt to discern between clubfoot recurrences and incomplete corrections.

PURPOSE: To better delineate between incomplete clubfoot correction and true clubfoot recurrence based on the time at which the deformity reappears and the treatment necessary to correct the foot. METHODS: A chart review of all idiopathic clubfoot at a single institution treated by either the Ponseti method or short leg casting and surgery were reviewed for recurrent deformity involving the tibia, ankle, or foot. Comparisons of treatment required to correct deformities were made between those noticed within six months of initial treatment and those noticed after six months. Similar comparison

Comparison of radiolucent and fracture tables in the treatment of slipped capital femoral epiphysis.

A retrospective study was undertaken to see whether there was any difference in the time required and the accuracy of pin placement between slips pinned on a fracture table and those pinned on a radiolucent table. All patients were treated by single screw fixation of stable slipped capital femoral epiphysis (SCFE), 36 on a fracture table and 29 on a radiolucent table. Mean operating room time on the fracture table (63 minutes) was greater than that on the radiolucent table (51.2 minutes) (P <0.05). Mean surgery time for the fracture table (38.55 minutes) was greater than that on the radiolucen
