Functional orthosis in shoulder joint subluxation after ischaemic brain stroke to avoid post-hemiplegic shoulder-hand syndrome: a randomized clinical trial.

OBJECTIVE: To examine whether the use of a shoulder joint functional orthosis over four weeks can mitigate the development or progression of the shoulder-hand syndrome in patients with shoulder joint subluxation after stroke. DESIGN: Two-armed randomized controlled trial. SETTING: Rehabilitation unit of a neurological hospital, single centre. SUBJECTS: Forty-one patients with caudal subluxation of the glenohumeral joint and hemiparesis of the upper extremity after ischaemic brain stroke. INTERVENTIONS: Support by functional orthosis Neuro-Lux (Sporlastic, Nürtingen, Germany) on top of usua

End-stage ankle arthritis: arthrodiastasis, supramalleolar osteotomy, or arthrodesis?

End-stage ankle arthritis is a debilitating condition that leads to pain and swelling in the ankle joint, with symptoms aggravated by standing and ambulation. Ankle arthritis commonly results from a history of trauma, or a series of recurrent injuries to the ankle. However, it may develop from other causes such as uneven loading of the ankle joint caused by an alignment deformity or from inflammatory arthritis such as rheumatoid arthritis, gout, or secondary to a serious joint infection. Patients with severe ankle arthritis often have limited ankle motion with an antalgic gait.

Feasibility of a hybrid-FES system for gait restoration in paraplegics.

This paper proposes a new configuration for a hybrid-FES gait restoration system, and presents a combination of simulation and experiment that support the feasibility of the proposed approach. Gait simulation results are presented that indicate the majority of load bearing and the majority of power for gait is provided by the legs (i.e., quadriceps muscle stimulation). Based on these simulations, experiments on healthy subjects indicate that the gait restoration approach should be capable of providing long periods of locomotion unimpeded by quadriceps muscle fatigue.

Surgery can reduce the nonoperative care associated with an equinovarus foot deformity.

UNLABELLED: Equinovarus is the most common lower extremity deformity seen after a stroke. Despite its frequency, there are no specific guidelines in determining when surgery should be considered and for which patients it is appropriate. We evaluated the charges of nonsurgical and surgical treatments for equinovarus foot in 29 consecutive patients who underwent surgery for a unilateral equinovarus deformity after stroke. Twenty-six patients (seven males, 19 females) were available for followup. Mean patient age at the time of stroke was 48.2 years (range, 3-66 years). The average age at surgery

Gymnastic wrist injuries.

During gymnastic activities, the wrist is exposed to many different types of stresses, including repetitive motion, high impact loading, axial compression, torsional forces, and distraction in varying degrees of ulnar or radial deviation and hyperextension. Many of these stresses are increased during upper extremity weight-bearing and predispose the wrist to high rates of injury during gymnastics. Distal radius stress injuries are the most common and most documented gymnastic wrist conditions. Other conditions include scaphoid impaction syndrome, dorsal impingement, scaphoid fractures, scaphoi

Development of a "Neuro-orthosis" for the control of wrist movements in patients with carpal tunnel syndrome: preliminary results.

Carpal tunnel syndrome (CTS) is a common, painful condition. Treatment is indicated when the symptoms of the disease interferes with the usual daily activities. The use of orthoses is advocated in the treatment of mild and moderate CTS. The rationale for using neutral wrist orthoses lies on the relation of the disease with the increased intracarpal pressure. It was demonstrated that the intracarpal pressure is minimum when the wrist is in neutral position. In spite of their proven therapeutic effects, long-term or improper use of static orthoses may lead to unwanted side effects such as muscle

Carpal tunnel syndrome during pregnancy.

Carpal tunnel syndrome is common during pregnancy. The symptoms usually disappear after delivery, but how soon has not been established. Thirty pregnant women with the syndrome reported to us the degree of pain in their hands every week before, and daily after, delivery. Pain was graded on a scale from 0 (none) to 10 (worst imaginable). The prevalence of the syndrome during pregnancy was about 2%. The mean pain score was 5.6 (SD 2.0) at referral and this was reduced by 1.2 (SD 2.2) after wearing an orthosis for a week. After this, it stayed almost unchanged until delivery. The score fell by ha

The effect of serial casting on gait in children with cerebral palsy: preliminary results from a crossover trial.

Serial casting aims to improve an equinus gait pattern in children with spastic cerebral palsy (SCP). We evaluated the effect of short-term stretch casting on gait in children with SCP, compared to the natural history. A crossover trial, consisting of a control phase and a casting phase, was conducted with children randomised into two groups. Both groups were assessed clinically, and using 3D gait analysis, at 0, 5 and 12 weeks. Subjects in one group had the 3 month casting phase first and in the other had the 3 month control period first. Casts were changed weekly and set at maximum available

Development of an ankle-foot orthosis with an oil damper.

The purpose of the present study was to develop an ankle-foot orthosis (AFO) that satisfies the requirements for an AFO for patients with hemiplegia as determined in a previous study. An oil damper has been introduced as an assistive device. The oil damper provides a resistive moment to plantar flexion of the ankle joint during initial stance on the paretic side. This function improves the insufficient eccentric contraction of the dorsiflexors. The magnitude of the resistive moment generated by this newly developed AFO can be changed easily to adjust its properties in accordance with the requi

[Ultrasound monitoring of the orthopaedic treatment of congenital hip dysplasia carried out within the first 6 months of life].

BACKGROUND: The use of ultrasound scanning in the early diagnosis of congenital hip dysplasia in infants within the first six months of life has become part of the routine in preventive medicine examinations. Ultrasound scanning makes it possible to monitor the development of the neonatal dysplasic hip at any moment and it also tells us whether or not it is responding to treatment and, if it is not, to modify the treatment in order to achieve cure. METHODS: The present study considers pathological hips studied echographically over 12 years in order to assess to what extent ultrasound controls

Current practice patterns in the treatment of Charcot foot.

Treatment of Charcot foot osteoarthropathy has emerged as a major component of the American Orthopaedic Foot and Ankle Society (AOFAS) Diabetes 2000 Initiative. A two-part survey described treatment patterns and current footwear use of patients with Charcot osteoarthropathy of the foot and ankle. In the first part, 94 consecutive patients with a history of Charcot foot and ankle presenting for care were questioned on their foot-specific treatment and current footwear use. A history of diabetic foot ulcer was given by 39 (41%) patients, and an infection had been present in a foot of 20 (21%) pa

Biomedical gait evaluation of the immediate effect of orthotic treatment for flexible flat foot.

Flexible flat foot subjects attending the prosthetics and orthotics units come with prescriptions from orthopaedic surgeons for arch supports. Usually a pair of thermoformed plastic inserts are fabricated and fitted to treat the patients. However the effect of the orthotic treatment is not yet clear. A motion analysis system with two video cameras placed on the lateral and rear sides of the subject together with one force platform was used to investigate the immediate effects of the orthotic treatment. The force platform collected force data and the two cameras captured two-dimensional displac

A comparison of gait with solid, dynamic, and no ankle-foot orthoses in children with spastic cerebral palsy.

BACKGROUND AND PURPOSE: This study compared the effects of dynamic ankle-foot orthoses (DAFOs) with a plantar-flexion stop, polypropylene solid ankle-foot orthoses (AFOs), and no AFOs on the gait of children with cerebral palsy (CP). These orthoses were used to reduce the excessive ankle plantar flexion during the stance phase of gait. SUBJECTS AND METHODS: Ten children with spastic CP (6 with diplegia and 4 with hemiplegia) were tested after wearing no AFOs for an initial 2-week period, solid AFOs for 1 month, no AFOs for an additional 2 weeks, and DAFOs for 1 month. The effects of the two o

Gait electromyography in children with myelomeningocele at the sacral level.

OBJECTIVE: Patients with sacral level myelomeningocele can be expected to maintain a high level of ambulatory status long into adulthood. Gait deterioration and knee pain reported in this population may be attributed to compensatory movements and increased recruitment of less affected muscle groups to achieve this desired level of ambulation. The objective of this study was to analyze the effect of the solid ankle-foot-orthoses (AFOs) on the muscular activity of selected muscles during walking. DESIGN: Cohort/outcome. SETTING: Laboratory. PATIENTS: Twenty four patients with sacral level mye
