Superior Semicircular Canal Dehiscence Revision Surgery Outcomes: A Single Institution's Experience.

BACKGROUND: Superior semicircular canal dehiscence (SSCD) is an abnormality of the otic capsule, which normally overlies the superior semicircular canal. Surgical management is indicated in patients with persistent and debilitating symptoms. Given the complexity of the disease, there are patients who experience less favorable surgical outcomes and require revision surgery. The purpose of this study was to report to the rate of postoperative symptomatic improvement in patients who required revision surgery. METHODS: A retrospective analysis of patients undergoing SSCD surgical repair at a sing

Earlier recovery of lingual dysfunction after middle ear surgery in pediatric versus adult patients.

OBJECTIVES: It is generally thought that the recovery of damaged chorda tympani nerve (CTN) function after middle ear surgery is different in pediatric patients from that in adult patients. The purpose of this study was to investigate the changes and the progress of taste and somatosensory functions of the tongue after middle ear surgery in pediatric patients compared with those of adult patients. STUDY DESIGN: Prospective study. METHODS: Fifty-nine pediatric patients and 106 adult patients underwent middle ear surgery. Taste and somatosensory functions of the anterior tongue, the so-called

Hearing aid silicone impression material as a foreign body in the middle ear.

We report an extremely rare case of hearing aid silicone impression material as a foreign body in the middle ear. Symptoms of the patient were otorrhea and vertigo after taking of a mold impression on his only hearing ear, and the symptoms mimicked chronic otitis media. A temporal bone CT scan revealed foreign body material in the middle ear and Eustachian tube. An intact canal wall mastoidectomy with a facial recess approach and type IV tympanoplasty was performed to remove the silicone impression material. In addition to the case report, we review the literature regarding impression material

Balloon dilatation of the Eustachian tube: postoperative validation of patient satisfaction.

OBJECTIVES: The Eustachian tube is responsible for ventilation, protection and drainage of the middle ear. Dysfunction of the Eustachian tube can lead to impairments ranging from inadequate pressure equalisation in the middle ear and pneumatised mastoid process to cholesteatoma. Conventional surgical interventions for chronic tube dysfunction have not brought resounding clinical success. However, the 'Bielefelder Balloon dilatation' constitutes a new and, judging from early results, very effective treatment for chronic tube dysfunction. Proof of the efficacy of the surgical procedures is provi

Comparison of different computed tomography post-processing modalities in assessment of various middle ear disorders.

Several anatomic structures of the middle ear are not optimally depicted in the standard axial and coronal planes. Several 2D and 3D image-processing modalities are currently available for CT examinations in clinical radiology departments. Till now 3D reconstructions of the temporal bone have not been widely used yet, and attracted only academic interest. The aim of this study was to compare axial (source images), 2D and 3DCT post-processing modalities, and to evaluate the value of 3D reconstructed images/virtual endoscopy (VE) in assessment of various middle ear disorders for identification o

A clinical experience of 'STAMP' plate-guided Bonebridge implantation.

CONCLUSION: The surface template-assisted marker positioning (STAMP) method is useful for successful Bonebridge™ (BB) implantation on a planned site while avoiding dangerous positions. OBJECTIVES: To confirm the usefulness of the STAMP method for the safe operation of BB. METHODS: From a patient's temporal bone CT data, a guide plate and confirmation plate were generated by the STAMP method. The guide plate is used to mark the correct place for implantation, while the confirmation plate lets us know the correct angle and depth of the hole. RESULTS: With the guide plate, the correct place fo

Neuroendocrine adenoma of the middle ear (NAME) mimicking as chronic otitis media with an episode of facial nerve palsy.

OBJECTIVES: To increase awareness of neuroendocrine adenomas of the middle ear (NAME), rare lesions often mistaken for other entities or chronic otitis media. Histogenesis remains controversial, although the consensus tends toward a pluripotent stem cell of the middle ear mucosa as the origin of the lesion. The tumour is characterised by dual differentiation with exocrine and endocrine components. The most common symptoms are conductive hearing loss, tinnitus and vertigo. The treatment of choice is complete surgical removal of the tumour with no adjuvant radiotherapy being required. CASE REPO

Self reported hearing difficulties following excision of vestibular schwannoma.

A new questionnaire, developed to investigate self-reported hearing difficulties in unilateral deafness, was completed by 221 post-surgical vestibular schwannoma patients. Results were compared to a group of 51 normal hearers and 12 patients with severe unilateral deafness from other causes. The vestibular schwannoma patients rated their post surgical hearing as profoundly deaf in the operated ear, and worse than pre-surgical in the other. Factor analysis of the results revealed a general hearing factor and five factors, indirect listening, direct listening, face-to-face listening, noise and d

Long-term evolution of subjective visual vertical after vestibular neurectomy and labyrinthectomy.

Subjective visual vertical (SVV) tilt, observed after vestibular neurectomy and labyrinthectomy, is considered to be due to the deafferentation of the otolithic organs. The aim of this study was to determine the long-term evolution of the SVV up to 4 years after surgery. Between 1993 and 1998 the SVV was measured in 35 patients (18 men, 17 women) aged from 21 to 71 years (mean 36 years). Vestibular neurectomy was performed in 30 patients and labyrinthectomy in 6. SVV was measured with a binocular test (vertical frame) and a monocular test (Maddox rod). Immediately after operation, the SVV show

A Comparison of Otolaryngology Training in Five English-Speaking Countries.

Otolaryngology was conceived at the turn of the twentieth century as a product of the amalgamation of the separate disciplines of the primarily surgeon-led otology and physician-led laryngology. Since its conception, otolaryngology has flourished and continues to, due to the many advancements in medical technologies. We aim in this review to provide a comparison between the postgraduate training pathway for otolaryngology in five English-speaking countries, highlighting the main differences, strengths, and drawbacks of each pathway. This, we hope, will be able to guide future changes in the tr

Diode laser-assisted transcanal endoscopic removal of an aural polyp in the external auditory canal of a dog.

BACKGROUND: In humans, aural polyps comprise fibrovascular tissue covered by the respiratory epithelium. Aural polyps with ciliated epithelium are common in cats but are rarely reported in dogs. In a previous case, a mass filled the tympanic cavity alone, and it was surgically removed. OBJECTIVES: To report a case of a canine aural polyp with ciliated epithelium extending from the dorsal tympanic cavity to the external auditory canal with detailed otological features and to demonstrate the usefulness of the transcanal endoscopic procedure (TEP) with a diode laser as a less-invasive therapy.
