Premorbid personality and psychopathological dimensions in first-episode psychosis.

BACKGROUND: Broadening our knowledge of the relationship between personality and psychopathological dimensions in psychosis would provide insights into the nature of their underlying pathophysiology. Research, to date, has been carried out in chronic samples and the possibility that the personality assessment may have been contaminated by the psychotic episode cannot be ruled out. The aim of this study was to examine the relationships between personality and psychopathology in a 'first-episode psychosis' sample using a dimensional approach. METHOD: Premorbid personality dimensions of 94 conse…

Personality profiles of youngsters with velo-cardio-facial syndrome.

The personality profile of 48 youngsters (24 males and 24 females, mean age 8 years, 5 months) with Velo-Cardio-Facial Syndrome (VCFS) was compared with a group of 240 non VCFS control youngsters (matched on age and gender), and, in addition, with groups of youngsters with Prader-Willi (PWS), Fragile X (FXS), and Williams Syndromes (WS). Personality characteristics of each youngster were rated by both parents, using the California Child Q-set (CCQ). The scores on eight personality dimensions were compared, i.e., Extraversion, Agreeableness, Conscientiousness, Emotional Stability, Openness, Mot…

Prefrontal structural and functional deficits in schizotypal personality disorder.

Structural prefrontal deficits have been reported in patients with schizophrenia, but it is unclear if they are also found in patients with schizophrenia spectrum personality disorders. The hypothesis that a spectrum group will be characterized by prefrontal structural deficits was tested by assessing prefrontal gray and white volumes using magnetic resonance imaging in a community sample of 16 individuals with schizotypal/paranoid personality disorder, 27 comparisons, and 26 psychiatric controls. Frontal neurocognitive functioning was also assessed using the Wisconsin Card Sorting Test and th…

Object loss and object relations in depressive personality analogues.

The author investigated the extent to which self-reported early object loss and Thematic Apperception Test (TAT) measures of object relations differentiated individuals with a depressive personality from dysthymic persons and highly dependent individuals. In an undergraduate analogue sample of 54 students, those with a depressive personality reported a nonsignificantly higher proportion of object loss than did dysthymic and highly dependent participants. Those in the depressive personality and dependent groups had less complex representations of others than did those with dysthymia, and those …

Skills training as an adjunctive treatment for personality disorders.

Personality disorders are usually treated with either psychodynamic or supportive psychotherapy, with psychotropic medication often used as an adjunctive treatment. However, patients with personality disorders pose special treatment issues, because their problems are pervasive and long-standing and they have entrenched deficits in many areas of functioning. In this article, the authors consider the role of skills training in the treatment of personality disorders. They describe a two-pronged approach to the treatment of personality disorders based on a model of deficit compensation. Because sk…

Identity disturbance in borderline personality disorder: an empirical investigation.

OBJECTIVE: Identity disturbance is one of the nine criteria for borderline personality disorder in DSM-IV, yet the precise nature of this disturbance has received little empirical attention. This study examines 1) the extent to which identity disturbance is a single construct, 2) the extent to which it distinguishes patients with borderline personality disorder, and 3) the role of sexual abuse in identity disturbance in patients with borderline personality disorder. METHOD: The authors constructed an instrument that consisted of 35 indicators of identity disturbance culled from relevant clini…

Convergent validity of alternative MMPI-2 personality disorder scales.

The Morey, Waugh, and Blashfield (1985) MMPI (Hathaway et al., 1989) personality disorder scales provided a significant contribution to personality disorder research and assessment. However, the subsequent revisions to the MMPI and the multiple revisions to the diagnostic criteria sets that have since occurred may have justified comparable revisions to these scales. Somwaru and Ben-Porath (1995) selected a substantially different set of items from the MMPI-2 (Butcher, Dahlstrom, Graham, Tellegen, & Kaemmer, 1989) to assess Diagnostic and Statistical Manual of Mental Disorders (4th ed.; America…

Personality profiles of mothers of children with mitochondrial disorders.

Mothers of children with mitochondrial disorders, inherited neurodegenerative diseases, are faced with a frightening diagnosis and numerous demands associated with caring for these children. The psychological profile of mothers whose children carry a mitochondrial disorder is unknown. Forty-two mothers of children with mitochondrial disorders were interviewed and administered the Minnesota Multiphasic Personality Inventory--Second Edition (MMPI-2). Fifty-six per cent of the mothers had scores in the pathological range on three or more scales, most notably on Hypochondriasis, Hysteria and Paran…

A profile analysis of personality disorders: beyond multiple diagnoses.

The authors applied cluster analysis and multidimensional scaling to the analyses of 59 patients with personality pathology. Cluster analysis yielded eight typologies of patients: detached, anankastic, phobic, dramatic, erratic, emotional, milder emotional, and masochistic negativistic. Multidimensional scaling identified the dimensions of classifying patients: anxious rumination versus behavioural acting out, overall severity of personality pathology, and assertiveness versus withdrawal. Considering the distinction between personality disorder (dysfunctional personality) and abnormal personal…

[Differential diagnosis of dissociative identity disorder (multiple personality disorder)].

Recently the concept of dissociative identity disorder (formerly known as multiple personality disorder) has attracted increasing public and scientific interest. However, it is rarely diagnosed in the clinical setting. the reported case of a 47-year-old woman with a history of child abuse demonstrates the problems of differential diagnosis. A number of psychopathologic symptoms pointed to a multiple personality disorder, but in the follow-up psychotic symptoms such as delusions, possible hallucinations and bizarre behavior clearly emerged. The differential diagnosis of dissociative identity di…

Personality patterns and outcome in depressive and bipolar disorders.

Personality traits and disorders have a strong influence on the course and outcome of depressive and bipolar disorders. Studies of the influence of personality disorders (PD) and some PD clusters on outcome of mood disorders are controversial and suggest that more specific assessment of underlying traits or dimensions is needed. Utilizing the Munich Personality test (MP-T) scales of von Zerssen, this study tries to identify specific personality traits that may influence the outcome and clinical course of unipolar endogenous depression and bipolar disorder. Six unipolar depressives and 6 bipola…

Repetition of parasuicide--ICD-10 personality disorders and adversity.

Patients with a history of previous parasuicide were compared to those who had made their first attempt. A scale for suicidal ideation derived from the Scaled Version of the General Health Questionnaire was completed by patients. ICD-10 personality disorder diagnoses were derived from the Standardized Assessment of Personality which was administered to knowledgeable informants. Logistic regression showed that unemployment, increasing severity of suicidal ideation, previous psychiatric treatment and borderline personality disorder increased the risk of reports of previous parasuicide. Anankasti…

[The personality characteristics of patients with duodenal peptic ulcer].

Using MMPI test, 196 patients with duodenal ulcer were studied for their personal traits. The patients' personality was characterized by a high level of anxiety, mental tension, emotional immaturity, affective rigidity, suppressed aggression. The most apparent in the young adults were deviations from the norm of psychopathic nature; as patients grew older, their illness of longer duration, their personal profile tended to be dominated by a problem of suppressed aggression and emotional immaturity, with hypochondriac and depressive tendencies to be on the increase. The personal traits did not a…

Reliability and convergence of three concepts of narcissistic personality.

UNTIL recent years, the personality disorders have been relatively unexplored compared to other psychiatric diagnoses. Over 15 years ago, there was little agreement on the diagnosis of borderline personality disorder (Perry and Klerman 1978), but efforts to specify the constructs and respective criteria for the borderline diagnosis spurred a plethora of systematic research. The result is that, next to antisocial personality disorder, borderline has become one of the best-documented and validated personality disorders (Perry and Vaillant 1989). One important shift has been that good descriptive…

Acute and transient psychotic disorder: comorbidity with personality disorder.

A study sample of 51 patients with acute and transient psychotic disorder (ATPD) (ICD-10) is presented. The findings suggest that, in hospital settings, ATPD is a non-frequent condition with onset in early adult life and most often associated with female sex, good premorbid social functioning and no or minor/moderate psychosocial stressors. The DSM-IV criteria distribute the patients into three diagnostic categories: schizophreniform disorder (41%), brief psychotic disorder (33%) and psychotic disorder not otherwise classified (25%). A high prevalence (63%) of personality disorders (PD) is rev…
