Cardiac tamponade most commonly results from accumulation of blood, or other fluids, within the pericardial sac. However, rarely air tamponade can lead to tension pneumopericardium and death. Positive pressure ventilation in infants carries a risk of barotrauma and presents as pneumothorax, pneumomediastinum or rarely pneumopericardium. We present an infant with cardiac tamponade due to pneumopericardium to illustrate the classical signs and symptoms, and the management of this rare condition.

Between 1974 and 1986, inclusive, over 400 newborns with clinical, radiological, and/or pathological evidence of necrotizing enterocolitis (NEC) were treated at the Hospital for Sick Children, Toronto, Ontario. Within this group were 37 babies who had a bowel perforation that was treated with peritoneal drainage under local anesthesia. This report describes two cases of severe cardiovascular collapse occurring during laparoscopy using carbon dioxide insufflation under general anesthesia. Both patients made eventual complete recovery. The possible causes, gas embolism being the most likely, are discussed. A list of precautions for the safe conduct of laparoscopy is presented. Since these precautions have been followed there has been no recurrence of cardiovascular collapse in over 300 subsequent cases.

A 630-g male infant developed presumed necrotizing enterocolitis (NEC) after fungal enteritis that resulted in subsequent fungal septicemia associated with pneumoperitoneum on the 9th day of life. The authors report their experience of 610 cases of laparoscopic cholecystectomy (VLC) and focus attention on the specific complications of this operation, in other words those not linked to the access phase (pneumoperitoneum and introduction of the trocars). Five patients (four males, and one female, median age of 60 years) with colonoscopy-related bowel perforations were managed between January 1993 and June 1994. One patient had a difficult colonoscopy whereas four patients had polypectomies or biopsies. Abdominal pain was the main presenting symptom at a median of 3 h (range 2 to 12 h) following colonoscopy.
