A 45 year old male was admitted to the hospital for unsteady gait and dizziness. The brain CT showed bilateral cerebellar ischemic stroke. The MRI corroborated the stroke in the cerebellum with a third acute ischemic stroke in the pons. The laboratory work up showed a positive anti beta2-glycoprotein I antibodies. A 66-year-old man, with a history of pulmonary tuberculosis 40 years before admission, complained of headache and dysarthria that lasted for 2 weeks and was followed by diplopia. MRI revealed an isolated nodular lesion in the pons with a marked enhancement mimicking brain tumor and other diseases. Antituberculous drugs were started under the presumptive diagnosis of tuberculoma. The lesion was completely resolved after 9 months of treatment and thus, the final diagnosis was confirmed. This report describes a patient with sudden sensorineural hearing loss who was found to have a megadolichoectasia vertebrobasilar system that appeared to be causing compression of the ipsilateral facial and vestibulocochlear nerves. The patient was treated conservatively for 4 months, during which time no hearing returned. He then underwent microvascular decompression of the affected nerves. We report 36 cases of post-traumatic "primary brain stem haemorrhage" visualized by the CT scan and confirmed at autopsy. Clinical experience shows that many technical factors influence the inability to visualize brain stem haemorrhages.
