Acute Charcot neuro-osteoarthropathy.

Charcot neuro-osteoarthropathy (CN) is one of the most challenging foot complications in diabetes. Common predisposing and precipitating factors include neuropathy and increased mechanical forces, fracture and bone resorption, trauma and inflammation. In the last 15 years, considerable progress has been made in the early recognition of the acute Charcot foot when the X ray is still negative (stage 0 or incipient Charcot foot). Recent advances in imaging modalities have enabled the detection of initial signs of inflammation and underlying bone damage before overt bone and joint destruction has …

[Refractory status epilepticus presenting as shaken baby syndrome].

INTRODUCTION: A convulsive status in infants is usually triggered by a febrile syndrome secondary to an intercurrent infection or an infection affecting the central nervous system. Shaken baby syndrome is characterised by its association with bilateral or multifocal haemorrhage, retinal haemorrhage and encephalopathy. Children under one year old are the group with the highest risk, with a maximum incidence reaching a peak between 10 and 16 weeks of age. Intercurrent processes, such as baby colic or febrile syndromes that tend to step up crying, are usually precipitating factors of shaking. CA…

Redefining urological history taking - anal intercourse as the cause of unexplained symptoms in heterosexuals.

INTRODUCTION: Symptoms suggestive of genito-urinary infection are frequent reasons for visits to general practitioners and account for appro … mportance of careful history taking. Whilst some questions may be difficult to ask, they may reveal precipitating factors that the patient will be reluctant to volunteer. The repetitive nature of the behaviour may explain the chronicity of symptoms experienced by the patients, and avoidance of this activity may be the only management needed to improve them. For those w …

[Nonallergic rhinitis with eosinophilia syndrome: state of knowledge].

The aim of our article was to present the actual state of knowledge of the non-allergic rhinitis with eosinophilia syndrome (NARES). NARES is characterized by perennial rhinitis and nasal eosinophilia in the absence of allergy in patient's history, negative skin tests and normal IgE. This syndrome was described 20 years ago, but studies on it are scarce. NARES is more frequent in females at mean age. Family history is positive in part of cases. The clinical symptoms are sneezing, watery rhinorrhea, nasal obturation and nasal itching. The precipitating factors are often nonspecific irritants, w…

The central role of magnesium deficiency in Tourette's syndrome: causal relationships between magnesium deficiency, altered biochemical pathways and symptoms relating to Tourette's syndrome and several reported comorbid conditions.

Prior studies have suggested a common etiology involved in Tourette's syndrome and several comorbid conditions and symptomatology. Reportedly, current medications used in Tourette's syndrome have intolerable side-effects or are ineffective for many patients. After thoroughly researching the literature, I hypothesize that magnesium deficiency may be the central precipitating event and common pathway for the subsequent biochemical effects on substance P, kynurenine, NMDA receptors, and vitamin B6 that may result in the symptomatology of Tourette's syndrome and several reported comorbid condition

Suicidal intent in non-fatal illicit drug overdose.

AIM: To explore suicidal intent among drug users experiencing non-fatal overdose. DESIGN: Semi-structured interviews. SETTING AND PARTICIPANTS: Seventy-seven drug users experiencing non-fatal overdose and attending six hospital accident and emergency departments in two Scottish cities during 1997 and 1998. MEASUREMENTS: The extent of suicidal intent and motivations for intentional overdosing were examined. FINDINGS: The incidence of suicidal intent was high, with 38 respondents (49%) reporting suicidal thoughts or feelings before overdosing. Suicidal actions were significantly associated w

Tardive blepharospasm.

I report on five patients with tardive blepharospasm seen in a movement disorders clinic, out of 25 tardive dystonia patients. They were young (aged 25-50 yrs); four were men and three had a schizophrenic disorder. The onset was gradual while on maintenance neuroleptics in four and on withdrawal in the fifth. There were no significant antecedent events precipitating the disorder. The disorder was bilateral but asymmetric in two cases. Dyskinetic blinking was often an initial feature and tended to persist after the resolution of the blepharospasm. Orolingual dyskinesia was present in one case a

Hyperlipidemia among Saudi diabetic patients - pattern and clinical characteristics.

Premature development of atherosclerosis and increased rate of coronary artery and peripheral vascular diseases are characteristic features of patients with diabetes mellitus. Hyperlipidemia is an important precipitating factor for these diseases. Several patterns of lipid alteration among diabetics were described; the most common is hypertriglyceridemia. This study was constructed to look at the pattern of lipid alteration among Saudi diabetic patients. Forty-three percent of diabetic patients were dyslipidemic with an equal distribution of dyslipidemic patients with respect to type of dyslip

["Spontaneous" ischemic colitis: infectious or medicamentous colitis? 25 cases].

The diagnosis of non-gangrenous ischaemic colitis is difficult to assert when histological findings are not specific and when no precipitating cardiovascular event can be found. The constant absence of relapse after the initial episode suggests that an extraneous triggering event is involved. We have studied retrospectively 25 cases of spontaneous ischaemic colitis, looking for a non-haemodynamic triggering event. At the onset of colitis 9 patients had been taking non-steroidal anti-inflammatory drugs or antibiotics for 2 weeks or less. In 3 other patients colitis was associated with Escherich

Spontaneous occurrence of the induced cardioinhibitory vasovagal reflex.

ECG recording of spontaneous, neurally-mediated syncope is rare. We have observed ten patients who sustained 70 syncopal episodes in whom: (1) ECG monitoring recorded syncope caused by ventricular asystole (AV block, three patients; sinus arrest; seven patients); (2) syncope and the spontaneously observed arrhythmias were reproducible by carotid sinus massage, upright tilt test, or eyeball pressure; and (3) no discernable cause of precipitating factors were detected. Two patients had a history of cardiac disease and four patients had only mild nonclinical ECG or echocardiographic abnormalities…

[Factors increasing life threat in patients with lesions of the spinal cord].

Mortality of spinal cord trauma has been analysed. An analysis included 3,486 patients treated early after spinal trauma in 1965-1989. Life threat in patients, who underwent trauma to the spinal cord embraces several factors. The most important is are: the level and degree of the lesion to the spinal cord, certain causes precipitating severe lesions to the nervous system, advanced age, and--to some degree--accompanying lesions, way of therapy, advances in medical technology, and intensive treatment technics. Available data indicate that the life of patients admitted to the hospital with sympto

Recurrent classic migraine attacks following transdermal scopolamine intoxication.

A 20 year old male naval crew-member suffering from sea sickness was treated with transdermal scopolamine (TS). After 5 months of continuous … p examination, he continued to suffer from the attacks once every 10 to 14 days, with no identified precipitating factors. Psychiatric contraindications to pregnancy, with reference to therapeutic abortion and sterilization.

Pregnancy and parturition are not considered the basic causes of associated mental disorders, but as precipitating stresses. Pregnancy and parturition are thought to involve conflicts relating to dependency needs, homosexuality and hostility toward the child and husband. The incidence of psychoses related to pregnancy and childbirth appears to be declining. Therapeutic abortion and/or sterilization are indicated when continuance of pregnancy or future pregnancies would be likely to precipitate severe mental disease. Among the important factors to be weighed in considering a recommendation for

Amoxicillin Induced Fever, Rash, and Catatonia - A Case Study.

INTRODUCTION: Adverse drug reactions (ADR) are defined as any harmful or unpleasant events or injuries resulting from the use of any particular drug. Among those antibiotics that cause adverse reactions, amoxicillin is one of them. Catatonia and vasculitic rash are its rare adverse effects. CASE PRESENTATION: A 23-year-old postpartum female, with a history of taking empirical Amoxiclav (amoxicillin-clavulanic acid 625 mg) injection and oral tablets for episiotomy wound, presented with altered sensorium and fever followed by maculopapular rash. The use of dissolvable layered double hydroxide components in an in situ solid-phase extraction for chromatographic determination of tetracyclines in water and milk samples.

This research presents a simple and green in situ solid phase extraction (is-SPE) combined with high-performance liquid chromatography (HPLC) for the simultaneous analysis of tetracyclines (TCs) including tetracycline, oxytetracycline, and chlortetracycline. In is-SPE, TCs were efficiently extracted through the precipitation formation of dissolvable layered double hydroxides (LDHs) by mixing the LDH components such as magnesium and aluminum ions (both in metal chloride salts) thoroughly in an alkaline sample solution. After the centrifugation, the precipitate was completely dissolved with trif

Celiac crisis in children in Serbia.

BACKGROUND: To assess the prevalence and risk factors of celiac crisis (CC) in children with classical celiac disease (CD). METHODS: This retrospective study comprised 367 children with classical CD diagnosed from 1994 to 2015. The diagnosis of CD was based on the revised ESPGHAN criteria and CC on acute worsening and rapid progression of chronic diarrhea and vomiting followed by severe dehydration, multiple metabolic derangements and a marked decrease of body weight. RESULTS: Celiac crisis was confirmed in six (1.63 %) children, five in the first and one in the second year of life. In three

Exploring recent spatial patterns of cutaneous leishmaniasis and their associations with climate in some countries of the Middle East using geographical information systems.

This study explores the spatial trends of cutaneous leishmaniasis (CL) and characterises the relationships between the observed spatial patterns and climate in Jordan, Syria, Iraq and Saudi Arabia in 2009. Areal interpolation revealed the presence of four major hotspots of relatively high incidence rates covering most parts of Syria, central parts of Iraq, and north-western, central, south-eastern and south-western parts of Saudi Arabia. The severity of these hotspots was seen to decrease from high to low latitudes. The spatial patterns could be partly linked to precipitation (the higher the p

Gender differences in the cardiovascular responses to morphine and naloxone in spinal rats.

Putative gender differences in opiate cardiovascular effects were evaluated in spinal rats. After a 4-h exposure to a single dose of morphine (30 mg/kg, i.v.), abstinence was precipitated by naloxone (0.03-3 mg/kg, i.v.). Morphine produced a long-lasting bradycardia and a transient increase in arterial pressure that was similar in both genders. Thereafter, blood pressure decreased both in males and females. Naloxone precipitated a similar dose-dependent heart rate increase in both sexes and a gender-dependent increase in blood pressure. This sex difference appeared in the shape of the response
