Active surveillance for intermediate risk prostate cancer: the Manitoba Prostate Center experience.

OBJECTIVES: To report long-term oncological outcomes of men with intermediate-risk prostate cancer managed with active surveillance at a single centre. SUBJECTS/PATIENTS AND METHODS: We retrospectively analysed a prospectively maintained Manitoba Prostate Centre active surveillance database. Included were men with disease classed per National Comprehensive Cancer Network risk categories: very low-, low-, favourable intermediate-, and unfavourable intermediate-risk prostate cancer managed with surveillance (2004-2023). Active surveillance involved serial prostate-specific antigen testing, digi…

Activation of cytotoxic cells by syngeneic prostate antigens in experimental autoimmune vesiculo-prostatitis.

In our experimental model of autoimmune vesiculo-prostatitis, obtained by immunization with syngeneic male accessory glands (MAG) and complete Freund's adjuvant, the presence of specific autoreactive cells with cytotoxic activity against prostate antigens was studied. The specific cytotoxicity generated in MAG immunized rats was tested using 51Cr labelled syngeneic prostate cells or labelled chicken erythrocytes coated with specific antigens (MAG homogenate or chromatographic fractions from prostate homogenate) which were used as target cells in a medium free of complement. The addition of spl

Building a multi-model fusion prostate cancer risk prediction model based on clinical data and machine learning.

To provide a reference for the initial diagnosis of clinical prostate cancer, we identified predictors and established a risk prediction model by analyzing a national Chinese prostate tumor dataset. Average value was used to interpolate the data from a prostate cancer dataset provided by the National Population Health Data Center of China. Factor screening was performed using the Kruskal-Wallis test and binary logistic regression. A multicollinearity analysis was performed on the variables. Cleaned data were divided into training and test datasets. Seven machine learning models and the 3 tradi

Prostatic epithelial polyp of the prostatic urethra.

Ten cases of prostatic epithelial polyps of the prostatic urethra are reported. The ages of the patients were from 27 to 69 years, with a mean of 44 years. The most common clinical signs were gross hematuria, microhematuria, hematospermia, and dysuria. All specimens, obtained by transurethral resection, had a papillary or polypoid configuration. The tissue had a glandular structure that resembled that of the prostate. Prostate-specific antigen and prostatic acid phosphatase were found in the epithelial cells of the polyps by indirect immunoperoxidase staining. The polyps seemed to be hyperplas…

Prostate Cancer Specificity of PCA3 Gene Testing: Examples from Clinical Practice.

A specific marker for early prostate cancer would fill an important void. In initial evaluations of the prostate cancer antigen 3 (PCA3) gene vis-à-vis serum prostate-specific antigen (PSA) levels, the gene offers great promise. At the cellular level, PCA3 specificity for cancer is nearly perfect because of the gross overexpression of the gene by cancer cells. As a clinical test for early prostate cancer, heightened specificity is also seen in urine containing prostate cells from men with the disease. PCA3 gene testing holds valuable potential in PSA quandary situations: (1) men with elevated …

Cost-Effectiveness Analysis of Prostate Cancer Screening in the UK: A Decision Model Analysis Based on the CAP Trial.

BACKGROUND AND OBJECTIVE: Most guidelines in the UK, Europe and North America do not recommend organised population-wide screening for prostate cancer. Prostate-specific antigen-based screening can reduce prostate cancer-specific mortality, but there are concerns about overdiagnosis, overtreatment and economic value. The aim was therefore to assess the cost effectiveness of eight potential screening strategies in the UK. METHODS: We used a cost-utility analysis with an individual-based simulation model. The model was calibrated to data from the 10-year follow-up of the Cluster Randomised Tria…

Prostate carcinoma mimicking rectal cancer: a case report.

In the pre-prostate specific antigen era, patients with prostate cancer (PC) commonly presented with symptoms. Currently, most PC are diagnosed at an asymptomatic stage with abnormal digital rectal examination or raised prostate specific antigen. In rare cases, PC may infiltrate the rectum and cause symptoms mimicking rectal cancer. It is difficult to differentiate between the two based on clinical features alone. We here report our experience in managing an 86-year-old male, with no significant personal pathological history, who presented with diarrhea and occasional rectal bleeding without a

18F-DCFPyL Uptake in an Incidentally Detected Follicular Lymphoma by PET/CT Performed for Biochemically Recurrent Prostate Cancer.

A 75-year-old man, treated with curative intent for histopathologically proven prostate cancer (initial prostate-specific antigen, 27 ng/mL; Gleason 4 + 5 = 9) through external beam radiation therapy in 2010 in combination with 3 years of androgen deprivation therapy (leuprorelin), underwent F-DCFPyL PET/CT for biochemical recurrence with a prostate-specific antigen of 4.1 ng/mL in February 2019. Multiple pelvic and some para-aortic lymph nodes showed highly increased F-DCFPyL uptake, suspicious for metastases. Incidentally, a solid mesenteric mass and mesenteric lymph nodes with moderately in…

A sensitive proximity ligation assay for active PSA.

Prostate-specific antigen (PSA) is a widely used marker for prostate cancer. The utility of PSA tests is limited by their inability to differentiate prostate cancer from non-malignant conditions such as benign prostatic hyperplasia and prostatitis. In circulation, PSA occurs in various complexed and free forms, and specific determination of some of these can be used to improve the diagnostic accuracy of PSA tests. We have previously identified peptides that specifically bind to enzymatically active PSA and using such a peptide we have developed an immunopeptidometric assay for this form of PSA…

Cell-surface targets for prostate cancer therapeutics.

This review highlights recent data regarding antigens on the cell surface of prostate cancer cells or cells in the prostate cancer tumor microenvironment which may serve as targets for novel therapeutics. Emphasis is placed on clinical data involving human subjects. The biological role of each antigen discussed is described, as are the expression patterns of the antigen in relation to histologic subtype of prostate cancer. Most descriptions are based on histological or genomic data, and quantitative assessment via targeted immuno-PET when available. Some antigens such as prostate-specific memb

Increased Petrous Bone Uptake on 18 F-PSMA-1007 PET/CT Due to Otospongiosis.

We report the case of a 74-year-old man who had undergone radical prostatectomy for prostatic cancer 6 months earlier. Elevated prostate-specific antigen during follow-up prompted 18 F-prostate-specific membrane antigen (PSMA) ligand PET/CT ( 18 F-PSMA-1007 PET/CT) to search for new manifestations of prostate cancer, revealing an increased focal uptake (SUV max , 5.9) in the left cochlear/pericochlear temporal bone and equivocal PSMA-RADS-3a external iliac nodes. Comparison with cone-beam CT and MRI showed that the focal temporal bone uptake corresponded to the typical morphological features o…

An immunohistological study of granulomatous prostatitis.

Granulomatous prostatitis may result from tuberculosis and fungal infection and has been described following prostatic surgery. In most cases, however, the aetiology is unknown, although it may be due to a reaction to extravasated or altered prostatic secretions. We have investigated cells (macrophages, lymphocytes), serum proteins (fibrinogen, alpha 1-antitrypsin) and prostatic epithelial products (prostatic-specific antigen and prostatic acid phosphatase) in diffuse granulomatous prostatitis (3 cases), focal periacinar prostatic granulomas (9) and focal prostatic infarcts (5), using an immun

Clinical approach to the prostate: an update.

Prostatic disease continues to present clinicians with challenges. Although giant strides have been made in the medical and surgical management of benign prostatic hyperplasia, many fundamental questions about its pathogenesis, progression, and treatment efficacy remain unanswered. Prostate cancer also continues to be an area in which progress is needed despite major recent advancements. Numerous debates that include the value of prostate-specific antigen screening and appropriate roles for each of the numerous therapeutic modalities await resolution. For millions of patients who suffer from p…
