Safety and efficacy of femoral-based hemodialysis access grafts.

OBJECTIVE: Arteriovenous fistulae traditionally have been placed in the upper extremity. Experience with groin hemodialysis access has been discouraging because of high infection rates and associated limb amputation. We reviewed our experience with angioaccess grafts in the groin to assess safety and viability in our hemodialysis patient population. METHODS: A retrospective review was performed of all groin hemodialysis access grafts placed at a single tertiary care center between June 1990 and February 1998. Demographics, complications, and subsequent treatment were recorded. Graft patency a

[Chronic low grade endophthalmitis. A growing indication for explantation of intraocular lenses].

The aim of this study was to evaluate the effect of progress made in cataract surgery regarding the indications and results of intraocular lens (IOL) removal. For this purpose, the charts of all patients who had undergone IOL explantation at our institution between January 1990 and December 1992 were reviewed. Indications were divided into six groups: (a) chronic low-grade endophthalmitis; (b) pseudophakic bullous keratopathy; (c) luxation; (d) traumatic expulsion; (e) high-degree aniseikonia; (f) acute endophthalmitis. Visual acuities were grouped according to a good (1.0 to 0.5), acceptable

Complications of hip arthroplasty causing periprosthetic radiolucency on plain radiographs.

The technique of hip arthroplasty is at least 30 years old and currently is performed 120,000 times per year in the United States [1]. Changes in materials and surgical techniques have markedly decreased the prevalence of infection and aseptic loosening. At the same time, other complications such as stress shielding and aggressive granulomatosis have become increasingly important complications of joint replacement [2]. All four of these entities manifest radiographically as periprosthetic lucency (Fig. 1.). The purpose of this essay is to illustrate the plain film findings of each of these com

Infective endocarditis treated surgically: correlation between imaging, and surgical findings.

Echocardiography is crucial in the diagnosis of infectious endocarditis (IE). We assessed the correlation between transesophageal echocardiography (TEE) and surgical findings. A retrospective study of IE patients who underwent surgery. Discrepancies between TEE and surgical findings were defined as ≥ 1 infected valves, vegetations and abscess, and vegetations size ≥5 mm. Descriptive statistics and regression analysis were performed. Eighty patients were included. Mean age was 55.03 years, 26.3% had prosthetic valves. S. aureus and S. viridans spp were identified in 22.5% and 20% of cases, resp

The Role of Biofilms and Material Surface Characteristics in Microbial Adhesion to Maxillary Obturator Materials: A Literature Review.

BACKGROUND: Maxillofacial prosthetics includes restoration of maxillary defects resulting from resection of palate and nasosinus neoplasms with obturator prostheses which may be colonized by microorganisms and function as a reservoir of infection. Patients with neoplasms commonly also require radiotherapy that can result in changes in saliva quality and quantity and changes in the oral microbial flora. The altered flora, in individuals immunocompromised from cancer therapy, increases their risk of prosthesis-related infections. OBJECTIVES: In this review article, we explore microbial biofilms…

Fulminant ruptured septic aneurysm complicating the catheter related blood stream infection in a patient on maintenance hemodialysis: A case report.

Metastatic infections can complicate catheter-related blood stream infections (CRBSI) in dialysis dependent patients. However, an infected/septic aneurysm involving the aorta or its branches as a direct complication of CRBSI without an underlying infective endocarditis is not reported so far in the literature. We report a 43-year female, who presented with CRBSI 2 weeks following a tunneled dialysis catheter (TDC) insertion. Due to the lack of defervescence after 72 h of antibiotics given as per the culture sensitivity reports, the TDC was removed. Blood cultures grew Pseudomonas aeruginosa. A

Superior Mesenteric Artery Aneurysm Caused by Aortic Valve Endocarditis: The Case Report and Review of the Literature.

Infectious aneurysms of the superior mesenteric artery are a rare but life-threatening condition due to the risk of visceral ischemia, sepsis, or hemorrhage. In this study, we report the case of a superior mesenteric artery aneurysm (SMAA) secondary to a bioprosthetic valve endocarditis, successfully managed with aneurysm resection and saphenous vein interposition graft. We performed an extensive PubMed-based rewiew of the literature of the last 10 years on SMAA, which include the detection of 38 articles quoting 41 SMAAs. The case histories were divided in 2 groups: 18 cases belonged to nonmy

A Systematic Review and Meta-Analysis on the Use of Omniflow for Peripheral Arterial Bypass.

BACKGROUND: Omniflow IIⓇ is a biosynthetic material specifically marketed for arterial bypass procedures at high risk of graft infection. This meta-analysis aimed to combine current evidence for the use of Omniflow IIⓇ in peripheral arterial bypass and compare postoperative outcomes. METHODS: Seven databases were searched for the term "Omniflow." Studies were reviewed and included if: they were a randomized control trial, cohort study or case series; some patients in the study received Omniflow graft for peripheral arterial bypass. Primary outcomes were early and late postoperative infection;

Conventional single articulation constrained liners in revision hip arthroplasty: risk factors for failure and their combinations.

BACKGROUND: Constrained liners (CLs) have been used in revision total hip arthroplasty (rTHA) with varying results. Relatively few studies have identified specific risk factors for failure. This study aimed to assess implant survivorship and complication rates, identify risk factors for constraint-related complications, and assess the effect of multiple factors present in a single case. METHODS: We conducted a retrospective analysis of 101 rTHAs for various aseptic indications and as second-stage procedures for periprosthetic joint infection (PJI) utilising 2 models of conventional single-art

Causes and Frequencies of Reoperations After Endoprosthetic Reconstructions for Extremity Tumor Surgery: A Systematic Review.

BACKGROUND: Excision of bone tumors and endoprosthetic reconstruction allow patients early weightbearing and a potential functional advantage compared with amputation. These reconstructions do not restore the limb to normal status, however, and patients are subject to complications that may result in revision or loss of the limb. Because better understanding of these complications based on current information might help the patient and surgeon in decision-making, we undertook a systematic review of studies published on this topic. QUESTIONS/PURPOSES: (1) What are the primary modes and proport

Distal femoral arthroplasty using custom-made prostheses. The first 218 cases.

The first 218 cases of limb salvage using cemented intramedullary Stanmore massive distal femoral arthroplasties with a single-axis hinged knee, followed for an average of 58 months with a maximum of 251 months, were reviewed. Loosening (5%), local recurrence (4%), intramedullary stem fracture (3%), and infection (2%) were the major complications requiring surgical intervention. Loosening and implant fracture were treated by the insertion of a second massive replacement in all but one case. All recurrences and all but one infected case required amputation. Survivorship analysis showed that the

Pain and infection are the primary reasons for elective retrieval of bone-anchored implants.

PURPOSE: Elective removal of bone-anchored hearing implants and epistheses is uncommon, limiting the understanding of the biological factors contributing to adverse reactions, implant success or failure. Therefore, the tissue-level response at the implant interface remains poorly understood. METHODS: This study examined 21 explanted bone-anchored implants and interfacial tissues from 19 patients using a controlled retrieval protocol. The clinical data were correlated with histological, immunohistochemical, microbiological, and structural analyses. RESULTS: Pain (12/19) and infection (7/19) wer

A rocking biologic prosthetic valve: Cutibacterium acnes prosthetic valve endocarditis.

Cutibacterium acnes is a low-virulence pathogen that can cause prosthetic valve endocarditis (PVE), presenting significant diagnostic challenges due to its slow growth and indolent clinical course. We report a case of a 57-year-old male with a history of tissue aortic valve replacement, who presented with 6 months of intermittent fever and a nonproductive cough. Initial tests showed elevated inflammatory markers and moderate aortic regurgitation, but no vegetations were identified on transthoracic or transoesophageal echocardiography. After 13 months of symptoms, a PET scan showed significant

Successful prosthesis salvage using continuous local antibiotic perfusion for prosthetic joint infection following revision total elbow arthroplasty in a patient with rheumatoid arthritis.

INTRODUCTION: Prosthetic joint infection (PJI) represents a serious and challenging complication in a patient with rheumatoid arthritis. We report a case of PJI following revision total elbow arthroplasty with rheumatoid arthritis that was successfully managed with debridement, antibiotics, and implant retention augmented by continuous local antibiotic perfusion (CLAP). CASE PRESENTATION: A 69-year-old woman with a 39-year history of rheumatoid arthritis previously underwent revision total elbow arthroplasty for aseptic loosening. Two years following the revision surgery, she developed left e

Antimicrobial suppressive therapy in prosthetic valve endocarditis rejected from surgery despite indication.

BACKGROUND: Prosthetic valve endocarditis (PVE) incidence is rising in older patients, often rejected for surgery, leading them to a poor prognosis. Optimal antibiotic management is unknown for these patients. We compared the efficacy and safety of suppressive (SAT) versus conventional antimicrobial therapy (CAT) in this setting. METHODS: We conducted a prospective, multicentric, cohort study in southwest France including patients with PVE rejected from surgery despite indication, and surviving the initial 6 weeks of intravenous therapy. Beyond this period, patients could or not receive SAT,

Zoonotic aortic graft infection by Streptococcusequi.

A 69-year-old woman with hypertension had undergone total arch replacement with an open stent graft 7 years prior. She was referred to our hospital for evaluation after experiencing fever (>38 °C) and cough. Chest radiography revealed a prominent aortic arch, and contrast-enhanced computed tomography demonstrated aortic arch enlargement and peri-graft fluid collection containing air. These findings indicated graft infection and prompted immediate intervention. Blood cultures grew Streptococcus equi subspecies zooepidemicus, a zoonotic pathogen associated with horses. Notably, the patient worke

Fatal Lactobacillus endocarditis in a patient with transcatheter aortic valve replacement.

Lactobacillus species may translocate from the gastrointestinal tract into systemic circulation from ingested probiotics or commensal flora. Their pathogenic potential is still debated. Lactobacillus endocarditis is a rare entity with only around 120 cases reported in the literature. Here, we report the first case of fatal Lactobacillus endocarditis with involvement of a transcatheter aortic valve replacement with the following goals: to reaffirm the pathological significance of Lactobacillus spp, to demonstrate the potential limitations of the modified Duke criteria in diagnosing infective en

Brucella abortus exposure during an orthopedic surgical procedure in New Mexico, 2010.

We describe a periprosthetic Brucella abortus infection in a case-patient undergoing hip replacement revision surgery, and the subsequent investigation of laboratory and surgical staff exposures. Although exposures are rare, it is important to have infection prevention recommendations for surgical procedures among patients with suspected or unidentified Brucella spp. infection.
