[Surgical treatment of carcinoid tumors of the intestine].

Carcinoid tumours arise from the neuroendocrine system and present a characteristic morphological picture. They occur in almost every organ, predilected sites are the appendix and the small intestine. Prognosis depends on the primary localization and tumour size. Carcinoids of the appendix and rectum are mostly small and thus have a good prognosis. Growth of bronchial, stomach and small intestinal carcinoids is aggressive and implicates a high percentage of metastatic disease. First choice therapy is the surgical removal of the tumour. Depending on tumour size surgical treatment includes fulgu

Somatostatin receptor 2 (SSTR2) expression is associated with better clinical outcome and prognosis in rectal neuroendocrine tumors.

Somatostatin analogues have recently been used as therapeutic targets for metastatic or surgically unresectable gastroenteropancreatic (GEP) neuroendocrine tumors (NETs), and associated somatostatin receptor (SSTR) expression has been well demonstrated in most GEP NETs, with the exception of rectal NETs. SSTR2 immunohistochemical expressions were evaluated in 350 surgically or endoscopically resected rectal NETs and compared to clinicopathologic factors. SSTR2 expression was observed in 234 (66.9%) rectal NET cases and associated tumors with smaller size (p = 0.001), low pT classification (p =

Successful management of perivascular epithelioid cell tumor of the rectum with recurrent liver metastases: A case report.

RATIONALE: The perivascular epithelioid cell tumor (PEComa) is rare in young man and rarely occurs in the large intestine. PATIENT CONCERNS: The clinical characteristics, diagnosis, and managements in a 28-year-old boy who presented with sudden onset of cramping and abdominal pain and intermittent melena with a blood pressure of 74/39 mm Hg was retrospectively reviewed. CT scan of the abdomen revealed a 8.9 × 7.2 cm mass in the pelvic floor. DIAGNOSES: Given the difficulty of obtaining a diagnostic specimen, surgical resection was performed. The pathology report of lower anterior resection w

[A case report of carcinoid of the rectum coexisting with adenocarcinoma in a single lesion].

A 69-year-old female, chiefly complaining of diarrhea, was admitted to the Toyama Medical & Pharmaceutical University Hospital. A barium enema and an endoscopic examination revealed a villous adenocarcinoma of the rectum. Computerized tomography disclosed multiple metastatic lesions in the liver. She underwent an amptatio recti and an intra-arterial cannulation of the common hepatic artery in order to administer anticancer agents. Gross findings of the resected specimen showed villous adenocarcinoma containing a small ulcerated lesion. Histologic findings of this lesion revealed a collision ne

Colorectal carcinoma detected initially with barium enema examination: site distribution and implications.

The authors analyzed the site distribution of 122 consecutive cases of colorectal carcinoma in which a barium enema examination was available for review. The carcinoma was detected initially with barium enema studies in 82 cases (67%), sigmoidoscopy in 18 (15%), colonoscopy in 12 (10%), digital rectal examination in seven (6%), and computed tomography in three (3%). Thirty (37%) of the 82 cases found initially at barium examination were located in the cecum and ascending colon, which is a considerably higher proportion than that reported in studies based on cancer registry data or in surgical

[Malignant degeneration of the intestine in Crohn's disease].

A woman suffering more than ten years with Crohn disease, makes one day a small intestinal obstruction. She undergoes a laparotomy in which a resection of 30 cm terminal ileum is done. Surprisingly the anatomo-pathologic diagnosis is adenocarcinoma between the Crohn's inflammatory lesions. Weedom and Greenstein have proven that Crohn disease and colon carcinoma are related in their follow-ups. But to find such a relationship between Crohn and carcinoma of the small intestine tumours is very small: +/- 0,3/100.000 in a year and so the cases in literature too. We have found 38 cases with enough

[Combined treatment of rectal cancer].

The comparative analysis of two methods of treatment (a combined method with the preoperative radiotherapy and a purely surgical one) of 363 patients with carcinoma of the rectum has shown that an intensive concentrated course of radiotherapy with irradiation of extended fields (the irradiation zone includes the small pelvis cavity and paraaortal lymph nodes up to the level of the second lumbar vertebra) is satisfactorily endured by the patients and does not complicate the operation. In the postoperative period the patients subjected to preoperative radiotherapy showed a longer healing of the

Gingival and mandibular metastases from rectal adenocarcinoma: case report and 20 year review of the english literature.

Gingival metastases from non-oral malignancies are very uncommon. The case is presented of a man with rectal adenocarcinoma, whose first clinical evidence of metastatic disease was a gingival metastasis. Although the prognosis of patients with gingival metastases is felt to be poor, prompt initiation of appropriate therapy may afford significant palliation. A 20-year literature review of uncommon metastatic sites for rectal adenocarcinoma and non-oral malignancies which can metastasize to gingiva is briefly presented. Gingival metastasis should be included in the differential diagnosis of a gi

Recent advancements of neoadjuvant strategies in locally advanced rectal cancer.

The field of neoadjuvant therapy for locally advanced rectal cancer (LARC) has undergone profound changes, shifting from a primary focus on local control to a comprehensive strategy emphasizing long-term oncological outcomes and patient-centered care. While traditional neoadjuvant chemoradiotherapy (nCRT) followed by total mesorectal excision remains a cornerstone, its limitations in preventing distant recurrence and improving overall survival are increasingly recognized. Total neoadjuvant therapy (TNT), delivering systemic chemotherapy before or after nCRT, has thus emerged as a new paradigm.

Watch and Wait Strategy for Locally Advanced Rectal Cancer: Update From Results of Recent Clinical Trials.

Locally advanced rectal cancer (LARC) is a common malignant tumor in the digestive system. Currently, despite growing recognition of personalized medicine, systematic risk stratification and full incorporation of patient preferences for anal preservation and treatment tolerance remain challenging in some clinical settings. This review summarizes the latest clinical research progress on the watch-and-wait (W&W) strategy for LARC, including the applicable population of W&W, the factors predicting tumor regression, and the methods to increase the W&W rate. By synthesizing the current evidence, th

Challenges and strategies to improve recruitment in international, multi-centre research studies in locally recurrent rectal cancer: experience from the Locally Recurrent Rectal Cancer-Quality of Life (LRRC-QoL) study.

BACKGROUND: Locally recurrent rectal cancer (LRRC) is an emerging area for research; however, it represents significant challenges as a relatively rare form of advanced pelvic malignancy, from both a recruitment and study setup and delivery perspective. To date, there have been relatively few published trials in this setting. High-quality, multi-centre, prospective studies could offer helpful insights regarding the challenges associated with delivering studies in rare disease settings such as LRRC, and how to effectively address them. METHODS: The Locally Recurrent Rectal Cancer-Quality of Li

Electrochemotherapy for Anorectal Tumors: A Narrative Literature Review.

This narrative review explores the potential role of electrochemotherapy (ECT) in treating anorectal tumors, focusing on its effectiveness, feasibility, and associated toxicities. ECT, which combines chemotherapy with the application of an electric field to enhance drug uptake by tumor cells, has shown promise as a local treatment, particularly in cases where conventional therapies such as radiotherapy have been exhausted or are unsuitable. The review, conducted according to SANRA guidelines, included 18 studies, on ECT in anorectal tumors, ranging from preclinical trials in dogs to case repor

[Two Cases of Port-Site Recurrence after Laparoscopic Surgery for Colorectal Cancer].

Port-site recurrence is a rare manner of recurrence specific to laparoscopic surgery. Herein, we report 2 cases of port-site recurrence after laparoscopic and robot-assisted surgeries for colorectal cancer at our institution. Case 1: A 57-year-old woman underwent laparoscopic ileocecal resection for cancer of the ascending colon. Thirty-seven months after the surgery, the patient presented to our hospital with a mass in the right lower abdomen. It was observed in the abdominal wall, corresponding to a port-insertion scar on the right lower abdomen. As no metastases to other organs were detecte

[Watch and Wait for Rectal Cancer].

About one third of all colorectal carcinomas (CRC) are localised in the rectum. As part of a multimodal therapy concept, neoadjuvant therapy achieves downstaging of the tumour in 50-60% of cases and a so-called complete clinical response (cCR), defined as clinically (and radiologically) undetectable residual tumour after completion of neoadjuvant therapy, in 10-30% of cases. In view of the perioperative morbidity and mortality associated with radical rectal resection, including the occurrence of a symptom complex known as low anterior resection syndrome (LARS) and the need for deviation, at lea
