A 14-year-old right-handed boy developed spells of lip twitching only while reading. With prolonged reading, he occasionally experienced loss of awareness and limb jerking (figure and video on the Neurology® Web site at www.neurology.org). Reading epilepsy is a rare form of reflex seizure in which reading (silently or aloud) may trigger orofacial myoclonus, stammering, aphasia, or generalized convulsions. Onset is typically in early adulthood, with 2:1 male predominance. It occurs in isolation or autosomal dominant fashion.(1) Previous reports suggest efficacy of clonazepam, valproate, or leve

BACKGROUND: Relative afferent pupillary defects are typically related to ipsilateral lesions within the anterior visual pathways.
OBJECTIVE: To describe a patient who had a workup for headache and was found to have an isolated left relative afferent pupillary defect without any other neurological findings.
DESIGN: We review the neuroanatomy of the pupillary light reflex pathway and emphasize the nasotemporal bias of decussating fiber projections, which accounts for the relative afferent pupillary defect contralateral to the described lesion. Result Magnetic resonance imaging of the brain rev

Traumatic hypopituitarism was diagnosed in an 11-month-old male neutered cat. The presenting complaints were polydipsia, polyuria and lethargy of three months' duration. Craniocerebral trauma, as a result of a road traffic accident, had preceded the onset of clinical signs by six weeks. Neurological examination revealed right-sided mydriasis, reduced visual and tactile left forelimb placing reflexes and decreased proprioception in both the left fore- and hindlimb. Initial laboratory findings included hypernatraemia, hyperchloraemia, mild azotaemia, eosinophilia and isosthenuria. Low basal cort

We reported the cases of thoracoscopic sympathectomy, that is, six cases of hyperhidrosis, three of post herpetic neuralgia, and four of reflex sympathetic dystrophy, including recurrent or incompletely resected or ineffective ones. Recently this procedure for hyperhidrosis had been performed frequently because of its effectiveness, less pain, early discharge and cosmetic aspect. For an ineffective case of hyperhidrosis abdominal respiration which emphasized the exhalation and using an upper abdomen decreased the sweating. The balance of autonomic nerve system, toward parasympathetic dominant,

This report describes a sudden decrease in blood pressure after conservative treatment of acute intracranial hypertension. A 63-year-old woman with acute hydrocephalus after undergoing clipping of an aneurysm of the right supracerebellar artery developed increased intracranial pressure, necessitating surgical management. On the operating table, the patient developed Cushing's reflex. Mannitol injection combined with hyperventilation was begun immediately to reduce her intracranial pressure. Fifteen minutes later, a sudden and prolonged suppression of circulation was observed (blood pressure 65

This 20 year old man suffered increasingly from multifocal bone- and back pain over the last 6 months. Painful weakness of the left leg with dysesthesia of the 4th and 5th toe, a weight loss of 15 kg and polydipsia and pollakiuria had developed. The clinical workup disclosed hard tumors in the right mandible and tibia, a waddling gait with bilateral sign of Trendelenburg, reduced muscular force in the left leg with missing achilles tendon reflex and a loss of sensibility in the distal S1 segment, epigastric tenderness on pressure and hypertension with a value of 160/100 mmHg. The result of transcutaneous intrafold injection and its influencing factors were studied for fifty five procedures in fifty two patients with unilateral recurrent nerve paralysis. Maximum phonation time, mean air flow rate and amplitude perturbation quotient of the voice were normalized or improved after the procedures more than 85%. In general assessment with functional and acoustic examinations, 76% of the procedures showed significant improvement. The paralysis after the thoracic surgery, right vocal fold paralysis and the paralysis complicated with sulcus vocalic tended to have insufficie

1. The changes in heart rate surrounding a period of apnoeic asphyxia, produced by either central or obstructive sleep apnoea, are characteristically a progressive bradycardia during the apnoeic period followed by an abrupt reversal to tachycardia with resumption of lung inflation. 2. In a subject who had undergone bilateral pulmonary vagotomy with sparing of the cardiac branches on the left side, the bradycardia is still present during the apnoeic period; however, there is no abrupt response to lung inflation. 3. A 3 year, 11 month-old Japanese male with congenital hypomyelination is described. Clinical features are delay of motor development, generalized muscle hypotonia and weakness, absent tendon reflexes due to peripheral neuropathy, and normal mental development. Electrophysiologically, nerve conduction velocities could not be measured. Histological examination of the right sural nerve revealed total of absence myelin of most of the myelinated fibers. Electronmicroscopically, there was a concentric network of lamellae formed by double-layered sheets of basement membranes with fragments of Schwann

Voluntary lid closing inability has been observed in two patients with right-sided frontal ischemic damage. The patients developed a transient inability to close their eyelids voluntarily at the same time as a transient forced grasping in the left hand and left hemiparesis not affecting the face. Automatic and reflex lid closures were retained as well as the ability to keep the eyes closed and to reopen them readily on command. In previous reports, inability to close eyelids voluntarily has been attributed to apraxia, paralysis, or motor impersistence. The localization of the lesion observed i

The authors report the case a 11-month-old baby girl admitted by the "Mother and Child Welfare" doctor. On admission, she was very hypotrophic, febrile and pale. Furthermore, she had anaemia complicated with painful invalidating oedema of both thighs. X-ray film show moderate lesions of the metaphysis, but very rapidly, there was a hugh bilateral sub-periosteal hematoma. They insist on the value of periosteal hematoma as a characteristic sign of scurvy. This was later confirmed by laboratory investigations and by the finding of urinary infection due to bilateral reflex on cystography and to ri

BACKGROUND: Anoxic brain injury is a potentially lethal condition characterized by cerebral hypoperfusion and irreversible neuronal injury. Arterial spin-labeling (ASL) perfusion and diffusion-weighted imaging (DWI) magnetic resonance imaging (MRI) have been proposed as tools to detect cerebral ischemic changes and may aid in the assessment of anoxic injury.
AIM: To explore the relationship between regional ASL perfusion patterns and clinical outcomes following cardiac arrest.
METHODS: We performed a retrospective review to identify patients with clinical suspicion of anoxic brain injury who

1. Mastication was produced by stimulation of the right motor-sensory cortex of urethan-anesthetized rabbits with 15-s trains of shocks (1-ms duration) at 50 Hz. Movements of the lower jaw and jaw muscle electromyograms (EMGs) were recorded on magnetic tape for later computer analysis. 2. The stimulus site was chosen, and stimulus intensity adjusted, so that stereotyped movements were produced that included a wide swing of the mandible to the left side during jaw closure. 3. Control trials were alternated with trials in which a steel ball (2 mm diam) was thrust between the anterior molar teeth

INTRODUCTION: The Vision Screener® (Plusoptix) was developed for the recognition of amblyogenic refractive errors without cycloplegia. The aim of this study was to evaluate the sensitivity and specitifity for the detection of amblygenic refractive errors in preschool children. Furthermore it was investigated whether small angle strabismus as another cause for amblyopia could be detected by manual evaluation of corneal reflex images on printouts.
PATIENTS AND METHODS: 83 children aged between 0.5 and 6 years without strabismus had undergone 3 measurements without cycloplegia with the Vision Sc

We report a 47-year-old woman with progressive multifocal leukoencephalopathy (PML). She was a carrier of HTLV-I virus, and developed subacute right hemiparesis and marked motor aphasia. She had a malignant lymphoma in the left neck and basal cell carcinoma in the right inguinal region. Three months after the onset, she became unable to walk because of the right leg weakness or to speak because of motor aphasia. Magnetic resonance imaging (MRI) revealed multifocal T2-high lesions in the white matter of the left frontal lobe, and a brain biopsy revealed demyelinating pathology. A biopsy of the

INTRODUCTION: The visually enhanced vestibulo-ocular reflex (VVOR) and vestibulo-ocular reflex suppression (VOR-S) are critical in evaluating vestibular and cerebellar contributions to oculomotor control. This study aimed to investigate the feasibility and reliability of metronome-synchronised, manual head rotation video-oculography for evaluating VVOR and VOR-S in healthy participants. A secondary aim was to determine the effects of head rotation frequency and direction, and participant's age, on VVOR and VOR-S gain and saccades.
METHODS: Twenty-four healthy individuals aged 20-79 years unde

The Hand Blink Reflex (HBR) is a subcortical reflex response influenced by limb position in relation to the Defensive Peripersonal Space (DPPS) of the face. We investigated its cortical correlates with functional Near-Infrared Spectroscopy (fNIRS) by measuring cortical hemodynamic activity before and after evoking the HBR in NEAR (hand close to the face) and FAR (hand far from the face) positions, with the aim to explore whether cortical activity associated with arm positioning predicted HBR modulation. We hypothesized that HBR would elicit activation of the frontoparietal network involved in
