BACKGROUND: Although it brings satisfactory rib hump correction, concomitant thoracoplasty with surgical correction of scoliosis decreases pulmonary function values. To achieve satisfactory rib hump correction and avoid impairment to pulmonary function, we design a new kind of thoracoplasty-convex short length rib resection (CSLRR). This study is to evaluate the effect and outcome of CSLRR in conjunction with pedicle screw instrumentation.
METHODS: Seventy patients with thoracic adolescent idiopathic scoliosis treated by pedicle screw instrumentation were retrospectively analyzed after a mini

We report a case of delayed massive hemothorax, a 72-year-old women, due to diaphragmatic injury by multiple lower rib fracture on 4th day aftrer traffic injury. We tried to stop bleeding by transcatheter arterial embolization, but the control of bleeding was difficult, necessitating the emergency surgery. The diaphragmatic injury about 3 cm diameter was found, and was sutured with absorption thread. The post operative course was uncomplicated. Patient was discharged on 51th day after injury. Careful observation is important for delayed hemothorax after lower rib fracture.

STUDY DESIGN: Cadaver dissection and measurement.
OBJECTIVE: To describe a previously undocumented intracostal ligament that limits the potential space through which the T1 ventral ramus passes before joining the C8 ventral ramus.
SUMMARY OF BACKGROUND DATA: Preclavicular entrapment of the T1 ventral ramus can lead to radiculopathy, neurogenic thoracic outlet syndrome, or both, the so called "double crush" phenomenon. The usual sites of entrapment include the neural foramen, the interscalene interval, an aberrant cervical rib, the first rib itself, or an apical thoracic mass.
METHODS: A tot

Aneurysmal bone cyst is a rare benign tumor of the bone that can be difficult to distinguish from malignant tumors, especially when it presents in an unusual location. Herein, we report a six-year-old girl with a primary aneurysmal bone cyst in an uncommon location. It originated from the 4th rib and she presented with a huge chest wall mass. Despite the large size of the cyst, the only symptom was persistent cough. She was successfully treated with total excision of tumor without any complication. The patient has been followed up for 56 months without any recurrence.

OBJECTIVE: Pneumothorax (PTX) after trauma is a preventable cause of death. Drainage procedures such as chest tube insertion have been traditionally advocated to prevent fatal tension PTX. We evaluated the safety of close observation in patients with delayed PTX complicating rib fracture after minor chest trauma.
MATERIALS AND METHODS: Adult patients (>18 years) with a diagnosis of chest trauma and 3 or fewer fractured ribs were reviewed. Case patients were divided according to age, location and number of fractured ribs, mechanism of trauma, and initial pulmonary complication after thoracic t…

Thoracic outlet syndrome in children is an extremely rare disorder. Only 5 previous reports were presented in the literature with a description of thoracic outlet syndrome in 8 children. In these cases, the diagnosis of a cervical rib was common. We describe a case of neurogenic thoracic outlet syndrome in a 9-year-old patient where the initial symptoms occurred 2 months after a fall. Clinically, the patient developed progressive pain, numbness and tingling along the inner surface of the left forearm and in the palmar surface of the fourth finger and lateral aspect of the fifth finger. BACKGROUND: There is a diagnostic dilemma when a child presents with rib fractures after cardiopulmonary resuscitation (CPR) where child abuse is suspected as the cause of collapse. We have performed a systematic review to establish the evidence base for the following questions: (i) Does cardiopulmonary resuscitation cause rib fractures in children? (ii) If so, what are the frequency and characteristics of these fractures that may help to distinguish them from rib fractures caused by physical abuse?
METHODS: We performed a literature search of original articles, references, textbooks, and con

The overall prevalence of total rib anomalies in cases and controls was equal (14.9% and 14.2%, respectively). Cervical rib anomalies …

STUDY DESIGN: The association of capping and fusion of the iliac apophysis, and closure of the proximal humerus and rib epiphyseal growth plates to the end of spinal growth, was evaluated in a cohort of patients with juvenile and adolescent idiopathic scoliosis.
OBJECTIVES: To determine the association of closure of the proximal rib epiphysis growth plate, the proximal humeral epiphyseal growth plate, and capping (Risser 4), and fusion (Risser 5) of the iliac apophysis to growth cessation in patients with idiopathic scoliosis.
SUMMARY OF BACKGROUND DATA: Accurate evaluation of remaining spin

Intrahepatic cholangiocarcinomas are second most common primary tumors of the liver. They are usually seen in 6th to 7th decades of life and at an advanced stage leading to poor prognosis. Their occurrence in the young age group is rare. Histopathological features of this tumor are well documented but literature regarding cytomorphological features on FNA is limited. We describe the cytological features of this tumor in a young woman presenting primarily with a rib metastasis. FNA smears from hard lump in the right chest wall and liver mass showed small round tumor cells arranged in the form o

Castleman's disease is a rare disorder characterized by lymphoid hyperplasia. It may present as asymptomatic involvement of one lymph node group or as a multicentric disease with systemic features. We report a patient with Castleman's disease who presented with axillary lymphadenopathy associated with a solitary plasmacytoma originating from a rib. The affected rib was surgically resected and radical radiotherapy was subsequently administered to the axillary lymph nodes. In this particular case, a joint surgical and oncologic approach resulted in a successful outcome.

Chest wall deformity is one of the potential complications of rib cartilage harvesting for auricular reconstruction. An eight year old boy underwent two stage microtia reconstruction using the 6th-9th costal cartilage for the 3-D cartilage framework. One year later an anterior chest wall deformity was noticed with bulging of the 5th rib, asymmetry of the lower rib cage line and deviation of the lower part of the sternum. The anterior chest wall is made of ribs, sternum and respiratory muscles forming a dynamic structure. By using these pieces of rib cartilage an imbalance is created in which t…

The case is reported of a 32 year old woman with acute on chronic upper limb ischaemia due to thrombus from a cervical rib that had compressed the right subclavian artery of her dominant hand after use of a rowing machine. If not detected early, this condition can be debilitating especially in the young. A multidisciplinary approach is advisable to ensure a satisfactory outcome.

The acute chest syndrome is a generic term for pulmonary complications of sickle cell diseases with heterogeneous etiologies that include pneumonia, vaso-occlusion of pulmonary arterioles, rib infarction, and fat embolism syndrome. My review summarizes these etiologies, the evidence, and pathophysiology supporting the hypothesis that infarction of segments of ribs by the same vaso-occlusive process responsible for the acute episodes of pain (characteristic of the sickle cell diseases) is often involved in the acute chest structure. Inflammation associated with the infarct then causes splinting…

No studies have demonstrated a direct relationship between pregnancy and rib fracture. A case of spontaneous rib fracture in the third trimester presents the opportunity to examine factors unique to pregnancy that may predispose the patient to stress fractures of the lower ribs. A 28-year old woman in week 31 of her pregnancy presented with the chief complaint of acute onset of right upper quadrant pain. A chest radiograph demonstrated a minimally displaced fracture of the right 10th rib. During pregnancy, the enlarging uterus causes certain opposing muscular forces to act on the ribs, making …
