The "50% rule" is used commonly to guide treatment of partial tears of tendons and ligaments. The purpose of this study was to examine the history and validity of the 50% rule in arthroscopic and orthopaedic surgery. A PubMed search yielded 1,039 articles that were reviewed to identify the origins of the 50% rule for hand flexor tendon lacerations, partial anterior cruciate ligament tears, partial-thickness rotator cuff tears, and partial injuries of the long head of the biceps tendon. The rule appears to have evolved from the hand literature toward somewhat arbitrary application for other ort

Rotator cuff injuries are a common cause of shoulder dysfunction, with diabetes mellitus and hyperlipidemia contributing to increased tendon vulnerability and impaired healing. In this article, a critical evaluation is presented on the comparative outcomes of open versus arthroscopic rotator cuff repair (RCR) in patients with these metabolic conditions. Findings suggest that arthroscopic RCR compared to open RCR offers advantages such as reduced tissue disruption, shorter recovery times, and lower infection rates, making it a preferred choice for metabolically compromised patients. However, op…

BACKGROUND: Rotator cuff tears lead to bone loss because of reduced mechanical loading at the tendon-bone interface, which results in poor healing after rotator cuff repair (RCR). Whether Yoda1, the specific Piezo1 agonist, can counteract bone loss and promote rotator cuff healing has not yet been explored.
HYPOTHESIS: Yoda1 promotes tendon-to-bone healing in a rat model of RCR and regulates M1/M2 macrophage polarization at the tendon-bone interface.
STUDY DESIGN: Controlled laboratory study.
METHODS: A total of 120 male rats aged 12 weeks, which were randomly divided into 4 groups, were us

Recently, superior capsule reconstruction (SCR) has achieved some results in the treatment of irreparable massive rotator cuff tears (IMRCT), but the incidence of various postoperative complications is high. The study aims to establish a model of IMRCT in rabbits, and compare the histomorphology and molecular biology differences between superior fulcrum reconstruction (SFR) and SCR, so as to provide a new and effective treatment method for the clinical treatment of IMRCT. Thirty-six mature New Zealand white rabbits were required for the experiment to build the model of IMRCT. The supraspinatus

BACKGROUND: Accurate assessment of fatty change in rotator cuff muscles is critical for clinical decision-making and predicting postoperative outcomes. While the Goutallier classification is widely used for grading fatty change, it is subjective and exhibits inter-rater variability. This study aims to evaluate the reliability and validity of a quantitative method using Otsu's thresholding for measuring fatty area in magnetic resonance imaging scans, comparing its performance to the Goutallier classification.
METHODS: Thirty-four patients with large to massive rotator cuff tears underwent preo

We describe a rare case of synovial chondromatosis in the subacromial bursa that led to rotator cuff damage and progressive glenohumeral osteoarthritis. A 66-year-old woman initially presented with shoulder pain and limited motion, and imaging revealed a loose body in the subacromial bursa and partial-thickness rotator cuff tear. Conservative treatment was initially effective, but symptoms recurred after migration of the loose body into the rotator cuff. Arthroscopic removal and partial synovectomy were performed, resulting in symptom relief. Histology confirmed benign osteochondroma. This cas

Superior capsular reconstruction was developed to restore patient biomechanics for patients with massive irreparable rotator cuff tears that preclude shoulder arthroplasty. Recent studies have shown excellent short-term clinical outcomes and improved pain and functional scores but high rates of complications including retear, loss of fixation, or incomplete healing. An alternative option, reverse total shoulder arthroplasty, is a reliable and safe method to ensure good muscle strength and return to play in this patient group.

The subscapularis (SSc) is the most powerful muscle of the rotator cuff. His role in shoulder stability and shoulder motion is well studied and due to functional and frequent association with concomitant other lesions, the repair of the SSc tendon is always desirable to restore force and stability to the shoulder.

BACKGROUND: Despite strong evidence supporting the efficacy of rotator cuff repair (RCR), previous literature has demonstrated that socioeconomic disparities exist among patients who undergo surgery. There is a paucity of literature examining whether payor type, including Medicare, Medicaid, and commercial insurance types, impacts early medical complications and rates of reoperation after RCR.
METHODS: Patients with Medicare, Medicaid, or commercial payor-type insurance who underwent primary open or arthroscopic RCR between 2010 and 2019 were identified using a large national database. Ninety

INTRODUCTION: One variable that could potentially affect failure of a rotator cuff repair (RCR) is the timing of beginning physical therapy (PT) after the procedure. Although many studies have demonstrated decreased stiffness with beginning PT early, studies have also demonstrated that early PT increases repair failure. The goal of this study was to identify revision surgery and capsulitis rates after RCRs from an available database and determine whether an association was present with the timing of PT post-RCR.
METHODS: Medicare patients within the PearlDiver database who underwent RCR were

BACKGROUND: Arthroscopic rotator cuff repair (RCR) is associated with substantial postoperative pain. Oral narcotic agents are the preferred analgesic postoperatively. However, these agents are associated with several side effects and a potential for abuse. This study evaluates the efficacy of ketorolac as an adjunctive agent for postoperative pain control after arthroscopic RCR.
METHODS: Adult patients undergoing arthroscopic RCR were prospectively enrolled and randomized to one of two groups. The control received our institution's standard-of-care pain protocol, including oxycodone-acetamin

RATIONALE: The shoulder joint is the most movable joint of the human body, and the incidence of aseptic inflammation of the muscles and tendons around the shoulder joint and acute and chronic muscle injuries is relatively high. Pulsed radiofrequency neuromodulation technology is gradually being used in shoulder joint diseases. Platelet-rich plasma (PRP) is a high-power platelet plasma solution obtained by centrifugation of autologous blood. Platelet cells contain many growth factors that promote tissue repair.

BACKGROUND: Current literature suggests a higher rate of rotator cuff disease development in patients with dyslipidemia (DL). Moderate to high levels of DL are associated with higher rates of retear and revision surgery after arthroscopic rotator cuff repair. Statins protect against development of rotator cuff disease and mitigate the need for rotator cuff repair.
PURPOSE: We aimed to investigate the influence of DL and statin use on postoperative functional outcomes.
STUDY DESIGN: Cohort study; Level of evidence, 3.
METHODS: Between 2010 and 2016, 266 patients underwent arthroscopic double

INTRODUCTION: Lifetime prevalence of shoulder pain is 70%, and approximately 50% of people with shoulder pain will experience pain for more than a year. Rotator cuff-related shoulder pain (RCRSP) is the most common shoulder condition and the main non-surgical intervention is exercise therapy. For approximately 30% of people with RCRSP, this approach does not lead to a significant reduction in symptoms. This may be due to an inappropriate dosage or choice of exercises. The aim of this investigation is to compare the short, mid and long-term effects, in terms of symptoms, functional limitations,
