[Diagnosis of unusual fractures of the wrist bones].

Fractures of the navicular bone are the most frequent fractures in the region of the wrist bone. Injuries of the other carpal bones are often primarily overlooked, since the examiner commands over less routine in diagnosing these rare fractures and the carpal bones are often superimposed over one another. We examined 9.723 x-ray films of the hand performed because of suspected fracture of the os naviculare, in order to find out how often lesions of the carpal bones - with the exception of those of the scaphoid bone - were discovered via the x-ray series of the os naviculare manus. For each ind…

Clinical result of operative treatment for scaphoid non-union in the skeletally immature: percutaneous versus open procedure.

This article is a retrospective study of 13 cases of scaphoid non-union in skeletally immature patients. For the fracture fixation, three cases of stable fibrous union with minimal sclerosis, without deformity or cystic changes were considered for the percutaneous Herbert screw fixation. Ten cases were managed with the open reduction and internal fixation with or without bone grafting. The average union time was 10.5 weeks post-operatively. The average union time was lesser in percutaneous Herbert screw fixation group (nine weeks) than open procedure group (11.5 weeks). All cases achieved unio

Scaphoid fractures treated by Herbert screw fixation.

Forty-one scaphoid fractures had Herbert screw fixation. There were three acutely displaced fractures (Type B), nine delayed unions (Type C), and 29 non-unions (Type D). All procedures were carried out by the senior author between December 1983 and May 1987. The average time from injury to surgery was 19 months. All Type D fractures were bone-grafted. Six of the 29 Type D fractures failed to unite radiologically following surgery. Incorrect screw placement was a factor in two cases. The average time from injury to surgery in three of the others was seven years. Radiological evidence of screw l

Avascular necrosis of the carpal scaphoid: Preiser's disease: report of 6 cases and review of the literature.

Six cases of nontraumatic avascular necrosis of the scaphoid are reported. Two cases were due to chronic steroid intake with several other sites of bone necrosis; for the other 4 cases no etiology could be established. Four were treated with a proximal row carpectomy, 3 with a very satisfying result, whereas one had to be converted into a wrist arthrodesis.

[Perilunar, transscaphoid, transcapital, transstyloid fracture-dislocation of the wrist. Operative reconstruction].

Report of a new type of fracture dislocation of the wrist and the intercarpal joints, which has not been published until now. A perilunar dislocation was associated with fractures of the scaphoid and capitate bones as well as a fracture of the radialstyloid. Open reduction and internal fixation was followed by fracture healing and slight posttraumatic arthritis. Loss of function was minimal and working capacity not reduced.

Lateral Approach for Scaphoid Excision and Capitolunate Arthrodesis in the Treatment of Scapholunate Advanced Collapse and Scaphoid Nonunion Advanced Collapse Wrists: A Case Series.

OBJECTIVE: The dorsal approach is commonly used in open wrist arthrodesis. However, the extensor compartments and the dorsal wrist capsule need to be opened. We propose and evaluate a lateral approach using a small incision over the scaphoid anatomical snuffbox, which could be more straightforward for performing scaphoid excision and capitolunate arthrodesis in the treatment of scapholunate advanced collapse (SLAC) and scaphoid nonunion advanced collapse (SNAC). METHODS: Between 2016 and 2021, 10 patients were enrolled retrospectively and underwent the lateral approach for scaphoid excision a

[Acutrak headless compression screw fixation for the treatment of scaphoid non-union].

OBJECTIVE: To evaluate the early clinical and radiographic outcome of scaphoid non-unions treated with Acutrak headless compression screw. METHODS: From January 2008 to July 2011,21 patients with scaphoid non-union were treated in our department. There were 18 males and 3 females with a mean age of (23.6 +/- 4.6) years; 12 cases were on right hand and 9 were on left. According to Herbert-Fisher classification, there were 10 cases with type D1, 7 cases with type D2, 3 cases with type D3, and 1 case with type D4. The mean time from injury to operation was (12.4 +/- 2.7) months. All patients wer

Scaphoid nonunion in the presence of a Scapho-Lunate coalition: a rare case report.

INTRODUCTION: Carpal coalitions are rare congenital anomalies, with an estimated prevalence of 0.1% in Europeans. Scapholunate (SL) coalitions are particularly uncommon (~ 2% of coalitions). Altered carpal kinematics in this context are thought to predispose to fracture and nonunion. To date, no case of scaphoid pseudarthrosis in association with an SL coalition has been reported. MATERIALS AND METHODS: We describe a 25-year-old man presenting with chronic wrist pain and limited extension following multiple skeletal injuries. Examination revealed tenderness in the anatomical snuffbox. Imaging

Intraosseous ganglion cyst of the scaphoid with an extraosseous component -an uncommon cause of volar wrist pain: case report and literature review.

Intraosseous ganglion is a relatively rare cause of chronic wrist pain. The ganglion is most commonly found in the os lunatum and os scaphoideum and in many cases includes an extraosseous component, both of which can be a source of clinical difficulty. In our case report, we present a patient with atypical chronic volar wrist pain who was diagnosed to have an intraosseous ganglion of the os scaphoideum with a volar extraosseous component as the main source of her problems. The patient had both components of the ganglion removed and the defect in the os scaphoideum was filled with an autogenous

Distinct Cell-Intrinsic Functional Differences Between Iliac Crest and Distal Radius Autografts.

PURPOSE: Autologous bone grafts demonstrate osteoconductive, osteoinductive, and osteogenic properties. Hand surgeons commonly augment surgical fixation with autografts to promote fracture healing. This study compared the intrinsic stem cell-like properties of 2 commonly used autograft sources in hand surgery: the iliac crest and distal radius. METHODS: A total of 9 subjects who received an iliac crest bone graft and distal radius bone graft harvest as a part of the standard care of distal radius malunion or nonunion correction or scaphoid nonunion open reduction and internal fixation were en

Evaluation of an artificial intelligence system for diagnosing scaphoid fracture on direct radiography.

PURPOSE: The aim of this study is to determine the diagnostic performance of artificial intelligence with the use of convolutional neural networks (CNN) for detecting scaphoid fractures on anteroposterior wrist radiographs. The performance of the deep learning algorithm was also compared with that of the emergency department (ED) physician and two orthopaedic specialists (less experienced and experienced in the hand surgery). METHODS: A total 390 patients with AP wrist radiographs were included in the study. The presence/absence of the fracture on radiographs was confirmed via CT. The diagnos

Costo-osteochondral graft (rib graft) reconstruction of the irreparable proximal scaphoid.

We present our series of 21 cases in which proximal scaphoid nonunions with fragmentation were treated with costo-osteochondral graft reconstruction (rib graft). The median follow-up was 29 months. Union was achieved in all 21 patients. There were significant improvements in subjective and objective outcome measurements and carpal alignment was well maintained in all patients, as shown by normal postoperative capitolunate angle measurements. No donor site complications were encountered. Rib graft reconstruction offers a reliable and straightforward option for the difficult problem of the irrep

Effect of Trapeziectomy on Carpal Stability.

Background: The scaphoid-trapezoid-trapezium (STT) articulation stabilizes the scaphoid and links the proximal and distal carpal rows. The purpose of the study was to determine whether trapezium excision in the treatment of trapeziometacarpal (TM) arthritis affects carpal stability. Methods: A retrospective chart and radiographic review was performed on all wrists that underwent trapeziectomy with suspensionplasty or ligament reconstruction, and tendon interposition for TM arthritis between 2004 and 2016. Radiographic outcome measures included the modified carpal height ratio (MCHR) and radios

Approach to the Perpendicular Fixation of a Scaphoid Waist Fracture-A Computer Analyzed Cadaver Model.

PURPOSE: In scaphoid fracture screw fixation, the screw is commonly placed along the long axis of the bone, without consideration of the fracture plane. This position is not perpendicular to transverse waist fractures or to the more common horizontal oblique fractures. Our aim was to examine the feasibility and describe possible approaches to, placing a screw perpendicular and in the center of the scaphoid waist fracture. METHODS: Computed tomography of 12 cadaver wrists was performed in 3 positions to examine possible approaches in flexion, neutral, and extension of the wrist. The scans were

Pilot study evaluating a clinical decision tool on suspected scaphoid fractures.

In an earlier study we have proposed a scaphoid decision-protocol in order to improve diagnostic accuracy in case of suspected scaphoid fractures. This pilot study evaluated this protocol. In this pilot study (n = 31) most cases with clinical suspicion of scaphoid fractures reached a positive test result on the combined 7 clinical tests (93.5%). Using this test combination, no scaphoid fractures were missed (no false-negatives; sensitivity 100%), but it also included many patients with no scaphoid fracture. Many of these, however, were found to have another fracture. In total, 48% had a scapho

Radioscapholunate arthrodesis.

Although RSL fusion is a viable option for isolated radiocarpal arthritis, the enthusiasm for this procedure should be tempered with the reality that kinematics of the wrist is not entirely suited for independent midcarpal flexion and extension [10]. Limited wrist flexion and extension is expected following a successful RSL arthrodesis. The effects of imposed abnormal kinematics are further shown by the high incidence of RSL nonunions, occurrence of scaphoid fractures, and postoperative deterioration of the midcarpal joint [15,22]. In a young patient with posttraumatic arthritis or rheumatoid
