Cone beam computed tomography for the assessment of linear scleroderma of the face.

BACKGROUND: To date, standardized methods for assessing the disease progression of linear scleroderma of the face (LSF) are lacking. OBJECTIVES: We investigated whether Cone Beam Computed Tomography (CBCT) may represent a reliable tool for assessing linear scleroderma of the face (LSF). METHODS: Ten patients with LSF and five age-matched controls underwent CBCT assessment. The transverse sections at three anatomic levels of the maxillofacial bones were analyzed. Measurements of soft tissue and total thickness of both affected and unaffected side of the face were made by a standardized method

Delay in Diagnosis of Congenital Linear Scleroderma until Adulthood.

BACKGROUND: Scleroderma encompasses a spectrum of disorders characterized by thickening of the skin and subcutaneous tissue with increased collagen deposition. Linear scleroderma is subdivided into progressive hemifacial atrophy and en coup de sabre subtype. OBJECTIVE: We report a case of congenital linear scleroderma identified in an adult, misdiagnosed since birth as birth trauma. METHODS: We completed a review of the literature for similar cases using PubMed and Medline. RESULTS: This is the first report of congenital linear scleroderma en coup de sabre diagnosed in an adult following an

Sclerosing postirradiation panniculitis.

Sclerosing postirradiation panniculitis is an unusual variant of panniculitis that appears as a complication of megavoltage radiotherapy. Most patient are women with a history of breast cancer who develop nodular lesions on the anterior chest skin several months after receiving megavoltage therapy for breast carcinoma. Sclerosing postirradiation panniculitis is an unusual cutaneous complication of megavoltage radiotherapy that should be distinguished from subcutaneous metastatic disease, cellulitis, and connective tissue diseases involving the subcutaneous fat. The differential diagnosis can b

[Achalasia in circumscribed scleroderma].

HISTORY: A 38-year-old man had been suffering from circumscribed scleroderma for 12 years. Dysphagia had been diagnosed 5 years ago and for the last 2 years he had retrosternal dysphagia for solid and liquid food. His symptoms had increased markedly 6 months before presenting at our hospital and the patient had lost 15 kg of weight. 2 months ago, a percutaneous endoscopic gastrostomy (PEG) had been inserted at another hospital. INVESTIGATIONS: The patient presented in a reduced general and nutritional state. The routine laboratory tests and tumor markers were within normal range. Endoscopy sh

[Scleroderma-like lesions in the course of autoimmune hepatitis. Case report].

UNLABELLED: We present a case of a 31-year old man with a 17 year history of Raynaud's symptom and contractures of the both hands' interdigitalis joints, progressive skin sclerosis of the arms, forearms and hands, telangiectases on the face skin. Because of the painful leg ulcers on the outer surface of both shins and on the dorsal part of the right foot (they appeared almost 10 years ago) the patient finally was admitted to our Department. Physical examination revealed hepatosplenomegaly, in laboratory tests leukopenia, mild thrombocytopenia, elevated ESR, elevated serum levels of IgG, IgM, I

Immunohistochemical investigations and introduction of new therapeutic strategies in scleromyxoedema: case report.

BACKGROUND: Scleromyxoedema is a rare chronic skin disease of obscure origin, which may often be associated with severe internal co-morbidity. Even though different casuistic treatment modalities have been described, to date, curing still seems to be impossible. CASE PRESENTATION: We report a 44-year-old Caucasian female presenting with remarkable circumscribed, erythematous to skin-coloured, indurated skin eruptions at the forehead, arms, shoulders, legs and the gluteal region. Routine histology and Alcian blue labelling confirmed a massive deposition of acid mucopolysaccharides. Immunohisto

Necrotizing vasculitis in a patient with generalized morphea.

Generalized morphea is rarely associated with systemic overlap. We report an unusual case with generalized morphea involving cutaneous large vessel vasculitis, mononeuritis multiplex, and lupus anticoagulant without any evidence of the coexistent systemic lupus erythematosus.

Disaccharide analysis of skin glycosaminoglycans in atrophoderma of Pasini and Pierini.

There are divergent opinions as to whether atrophoderma of Pasini and Pierini (APP) is a nosologic entity or a primary atrophic morphoea. In this study, we used high performance liquid chromatography to analyse the skin disaccharide contents of glycosaminoglycan (GAG) in two patients with APP and compared the results with those from a typical atrophic morphoea patient. Perilesional uninvolved skin was used as a control in each patient. In the atrophic phase morphoea, both the total amount of disaccharide per skin punch-biopsy and the amount of DeltaDi-4S(DS) - the main disaccharide unit of der

Frontoparietal scleroderma (en coup de sabre) following Blaschko's lines.

We describe three patients with frontoparietal scleroderma showing multiple lesions. The lesions were in two different lines that seemed to belong to Blaschko's lines. It remains controversial whether linear scleroderma follows Blaschko's lines, but our observations presented here suggest that frontoparietal scleroderma occurs along the lines of Blaschko.

Papular mucinosis associated with scleroderma.

A 49-year-old Japanese woman had been suffering from limited cutaneous scleroderma with papular mucinosis. Papular mucinosis was characterized by multiple, asymptomatic, elevated, skin-colored papules on the dorsal regions of the hands. Histopathological findings of the hard papules showed a marked deposition of hyaluronic acid along sclerosis in the middle and the lower parts of dermis. Serological studies revealed a positive antinuclear antibody (speckled type). Intravenous administration of prostaglandin E1 derivatives reduced the size of the papules, the degree of sclerodactyly and the sev

Plasma cell panniculitis: a unique clinical and pathologic presentation of linear scleroderma.

A 64-year-old woman had an 11-month history of a highly inflamed linear eruption that affected the left arm, including the dorsal aspect of the hand. It was characterized histologically by an intense plasma cell infiltrate extending throughout the dermis and subcutaneous fat. During the subsequent 2 years, the area evolved into a linear sclerotic plaque in keeping with the diagnosis of linear scleroderma. This case represents a clinical and pathologic presentation of linear scleroderma not previously emphasized in the literature.

The titre of anti-centromere antibodies: its relationship to Raynaud's phenomenon and vascular occlusion.

A retrospective survey of all patients with positive anti-centromere antibody (ACA) determinations over a 5-year period was undertaken. Of the 52 patients identified, seven denied symptoms of Raynaud's phenomenon, all of whom had antibody titres of 2560 or less. One of these showed a 16-fold increase in titre with the onset of Raynaud's, 12 years after original presentation. The remaining 45 patients experienced Raynaud's on cold exposure. Within this group, seven developed thrombotic vascular disease, five of whom were associated with an ACA titre of 20,480 or above. Another two with similar

Finger systolic pressures and skin temperatures in severe Raynaud's syndrome: the relationship to healing of skin lesions and the use of oral phenoxybenzamine.

Systolic pressures and skin temperatures were studied in 23 patients with severe Raynaud's syndrome who were given oral phenoxybenzamine. There was a relationship between the measurements and the severity of the disease. Statistically significant correlation was found between the measurements and the time for healing of skin lesions. The chances of early healing are high when digital systolic pressure exceeds 55 mm Hg, the branchial-finger pressure difference is less than 50mm Hg, and the maximal difference among fingers less than 30 mm Hg. Early healing is also likely when finger temperature
