A focus of combat casualty care research is to develop treatments for when full resuscitation after hemorrhage is delayed. However, few animal models exist to investigate such treatments. Given the kidney's susceptibility to ischemia, we determined how delayed resuscitation affects renal function in a model of traumatic shock. OBJECTIVE: Pragmatic surveys for shock management by Thai physicians are unavailable. The objective of this study is to identify the shock management patterns on both; the incidence of septic shock and hemorrhagic shock here in Thailand.
MATERIAL AND METHOD: Two thousand questionnaires were sent to physicians who are called on to care for patients in shock across Thailand. The questionnaire is composed of 58 items regarding all aspects of the management of septic and hemorrhagic shock. A frequency scale has been defined by 5 levels of patient proportion estimates from routine practices.

Traumatic forequarter amputation is rare and extremely mutilating. A 21-year-old male factory worker who sustained a traumatic shoulder girdle amputation is described. The injury mechanism was traction from the machine's force combined with the compressive force on the sternoclavicular joint. The factors leading to survival from this injury include rapid transportation, prompt and effective resuscitation, including treatment of shock, and adequate surgical management. Recognition of the complications associated with retrosternoclavicular dislocation is emphasized.

We report on a case of toxic shock syndrome (TSS) in a 17-year-old healthy patient after surgery of the right knee joint following a traumatic patella luxation. Two days after the intervention the patient showed fever, diarrhea, cough, and a diffuse erythroderma without any signs of postsurgical wound infection. Septic shock and acute renal failure followed. With further improvements of the prehospital rescue systems, an increasing number of patients with extreme injuries such as traumatic hemipelvectomy are admitted to trauma centers alive. We investigated morphological findings of human biopsy material taken from lung and muscle, as well as lung tissue taken from animals in hypovolatemic-traumatic shock experiments. We present 2 rare cases of patients with uncontrolled hemorrhagic shock induced by traumatic ascending aortic pseudoaneurysm rupture into the esophagus. Two men were presented to the hospital after traffic accidents.
