The approaches and outcomes of skull base surgery for pediatric sarcoma after initial therapy.

OBJECTIVE: Surgical removal of the residual pediatric sarcoma after initial therapy is common; however, that in the skull base area still presents a formidable challenge. We reviewed the approaches and outcomes of skull base surgery for pediatric sarcoma. METHODS: Thirteen patients with pediatric sarcoma who received skull base surgery were reviewed retrospectively. Tumor sites, surgical approach, complications, regional recurrence after surgery, and survival rate were analyzed. RESULTS: The residual tumor sites were the infratemporal fossa (8), nasal cavity and paranasal sinus (4) and orbit…

Modification of three-dimensional prototype temporal bone model for training in skull-base surgery.

Experience with dissection of the temporal bone is essential for training in skull-base surgery, but only a limited number of neurosurgical residents have the opportunity of cadaver dissection. A modification of a commercially available prototype three-dimensional (3D) temporal bone model is proposed to include artificial dura mater, venous sinuses, and cranial nerves for such surgical training. The base 3D temporal bone model incorporates the surface details and the inner ear structures and air cells. Model dural sinuses and dura mater made from silicone, cranial nerves made from rubber fiber

Skull base invasion of adenoid cystic carcinoma of the lacrimal gland : a case report.

Although adenoid cystic carcinoma (ACC) of the lacrimal gland is a rarely encountered orbital tumor, it invades intracranially more frequent … He was free of progression and recurrence at 6 months after treatment. The authors report a case of skull base invasion by an ACC of the lacrimal gland to remind neurosurgeons planning intervention that this disease shows a tendency to invade intracranially.

Construction of a three-dimensional interactive model of the skull base and cranial nerves.

OBJECTIVE: The goal was to develop an interactive three-dimensional (3-D) computerized anatomic model of the skull base for teaching microneurosurgical anatomy and for operative planning. METHODS: The 3-D model was constructed using commercially available software (Maya 6.0 Unlimited; Alias Systems Corp., Delaware, MD), a personal computer, four cranial specimens, and six dry bones. Photographs from at least two angles of the superior and lateral views were imported to the 3-D software. Many photographs were needed to produce the model in anatomically complex areas. Careful dissection was nee…

[Experiences with the subfrontal approach to manage extensive fractures of the frontal skull base].

BACKGROUND: Surgical management of multiple traumatized patients with head and neck trauma is highly individualized and depends on a number of factors including etiology, intracranial pressure, concomitant injuries, patient age and the possibility of an interdisciplinary procedure. Severe head and neck trauma are often connected with fractures of the frontal skull base or nasoethmoido-orbital complex and CSF leakage. If there is suspicion of a CSF leakage surgical management to cover the dura-defect is essential. An intradural approach is necessary in case of concomitant intradural injuries wh…

Spinal anesthesia for a parturient with an iatrogenic skull base defect and CSF leak.

A patient with an iatrogenic skull base defect and cerebrospinal fluid leak presented with rupture of amniotic membranes at 37 weeks' gestation. She had daily rhinorrhea that began in her first trimester of pregnancy and 11 months after endoscopic sinus surgery performed for chronic sinusitis. Consultation among the otolaryngologist, obstetrician, and obstetrical anesthesiologist were completed during the patient's second trimester to determine the most desirable type of delivery and anesthesia. A cesarean section was performed so as to avoid pushing in the second stage of labor that could wor…

Intraosseous dural arteriovenous fistula of the skull base associated with hearing loss. Case report.

The most common clinical presentations of dural arteriovenous fistulas (DAVFs) are bruit, headache, increased intracranial pressure, and intracranial hemorrhage. In particular locations, such as the cavernous sinus or middle cranial fossa, cranial nerve involvement due to dural arterial steal or venous occlusion may develop. A case in which a DAVF is associated with hearing loss, however, has not previously been reported. The authors report a case in which an intraosseous DAVF and associated hearing loss probably resulted from cochlear nerve or vascular compression caused by the draining vein

[A complex of skull base and cervical spine anomalies and Chiari type I malformation].

To study a relationship between the skull base and cervical spine anomalies and Chiari type I malformation (CMI), 364 patients with CMI were examined using craniography and magnetic resonance imaging of the brain and spinal cord. To reveal bone anomalies and CMI, morphometric data analysis was performed. The control group consisted of 120 patients without central nervous system pathology, cranial and cervical spine anomalies. The data obtained allowed dividing the skull base and cervical spine anomalies identified in the CMI patients into 3 groups: 1) primary anomalies (basilar invagination, p…

[Tumors of the anterior skull base. Effects of perioperative imaging on adjuvant radiotherapy planning].

OBJECTIVES: Treatment of anterior skull base tumours is still considered being a major challenge for all involved disciplines. Surgical management, complications, as well as the role of imaging for multimodality treatment options should be evaluated critically. PATIENTS AND METHODS: 57 patients underwent interdisciplinary one step trans-basal approaches for resection of benign and malignant anterior fossa tumours. Beside assessment of surgical results and perioperative morbidity, in 13 patients with malignant tumours perioperative imaging was evaluated with regard to further possible radio-on…

[Surgical occlusion of cerebrospinal fistulas of the anterior skull base using intrathecal sodium fluorescein].

BACKGROUND: For more than 25 years, intrathecal 5% sodium fluorescein has been routinely used at the University ENT Hospital at Graz during surgical closures of CSF leaks of the anterior skull base. Especially with endoscopic approaches, this technique has been of significant help in identifying and localizing dural defects. No fluorescein-related complications occurred in the series reported. MATERIAL AND METHODS: In a retrospective study, indications, techniques, and results of surgical closures of CSF leaks of the anterior skull base are reviewed. During 5 1/2 years from 1990-1995, 72 pati…

Surgical exposure of the petrous internal carotid artery: practical application for skull base surgery.

When exposing the horizontal petrous carotid artery in preparation for intrapetrous carotid bypass, the surgeon has no definite landmarks to localize the perimeter of the cochlea. The results of this study provide a practical, consistent, and safe method to maximize carotid artery exposure while minimizing cochlear injury. We measured the carotid-cochlea distance (mean, 4.3 mm) and the carotid-cochlear angle (mean, 10.8 degrees) in 33 temporal bones in which the extended middle fossa approach had been performed. We correlated this distance to the width of a Sheehy weapon knife, which can be ea

Adenosquamous carcinoma of the facial bones, skull base, and calvaria: CT and MR manifestations.

A 62-year-old woman had proptosis of the right eye, decreased visual acuity of the left eye, and no other focal neurologic deficits. She had a grand mal seizure 1 month before admission. The CT and MR studies showed extensive bone destruction of the margins of the right orbit, the floor of the middle cranial fossa, the right cavernous sinus, and much of the calvaria. There was considerable dural disease and tumor in the right orbit, paranasal sinuses, and scalp, as well as mucoceles of the left ethmoidal sinus with desiccated secretions. The diagnosis was adenosquamous carcinoma, an aggressive

Burkitt's lymphoma of the skull base presenting as cavernous sinus syndrome in early childhood.

Primary non-Hodgkin's lymphoma of the skull base presenting with neuro-ophthalmologic abnormalities or cavernous sinus involvement is very rare in children. We have found only 13 reported cases of cavernous sinus involvement by lymphoma [1]. We report the case of the youngest child diagnosed with Burkitt's lymphoma of the cavernous sinus and sphenoid sinus, whose first presentation was cavernous sinus syndrome with neuro-ophthalmologic findings.

Magnetic resonance angiography: analysis of vascular lesions of the temporal bone and skull base.

Contrast-enhanced, high resolution, computed tomography (CT), along with gadolinium-enhanced magnetic resonance imaging (MRI), provide the skull-base surgeon with essential information for diagnosis and surgical planning. Evaluation of skull-base vasculature has traditionally been performed with angiography. Four-vessel angiography is an invasive procedure that subjects the patient to the potential risks of vascular injury, stroke, renal damage, anaphylaxis, and radiation. The development of gradient-recall echo pulse sequence in MRI provides a means to evaluate the vasculature with a noninvas

The zygomatico-temporal approach to the skull base: a critical review of 11 patients.

The zygomatico-temporal approach to the base of the skull is a relatively new but established surgical technique. The approach involves the removal of the zygomatic bone to provide access to the skull base, middle cranial fossa, parasellar region and interpeduncular cistern with minimal brain retraction. An excellent view of the bifurcation of the basilar artery and suprasellar region is provided. The outcome of 11 patients undergoing this procedure is reported with particular reference to the post-operative morbidity and the cosmetic result.

Defect reconstruction and cerebrospinal fluid management in neurotologic skull base tumors with intracranial extension.

Intracranial extension (ICE) is the spread of tumor into the subarachnoid space through dura or along cranial nerve roots. The single-stage removal of the skull base tumor with its ICE has been confounded by cerebrospinal fluid management and defect reconstruction. The purpose of this report is to review a current protocol for managing the cranial base tumor and its ICE as a unit. The ventricular shunting paper of 1987 is retracted. All tumors were managed at a single stage. Defect reconstruction was size dependent. Ninety-eight neurotologic skull base tumors with ICE were managed from 1971 to…

The surgical exposure of penetrating injuries to the carotid artery at the skull base.

Injuries of the internal carotid artery at the skull base present complex technical problems for the surgeon. Exposure is difficult, distal control of the vessel for shunting or repair is frequently impossible, and thus ligation of the ICA is often necessary as definitive treatment. Three cases involving penetrating injury to the carotid artery at the skull base are presented. The issue of primary repair vs. ligation for carotid injuries is briefly addressed, and the various methods of surgical exposure of the distal extracranial ICA are reviewed.

Craniosynostosis: computed tomographic evaluation of skull base and calvarial deformities and associated intracranial changes.

Computed tomography has proven useful in children with craniosynostosis for the evaluation of deformity of the skull base, calvarium, and parenchymal brain structures. A retrospective analysis of 24 children seen during a 4-year period who had adequate preoperative, postoperative, and follow-up scans was carried out. Bone windows were used, and both bone thinning adjacent to fused sutures and thickening of affected sutures were demonstrated. Changes in calvarial contour were easily followed. Current trends in craniofacial reconstructive surgery have placed emphasis on skull base abnormalities;…
