Chondromyxoid Fibroma of the Skull Base: Our Experience with an Elusive Disease.

Chondromyxoid fibroma (CMF) is an extremely rare lesion of the skull base. This histologic type typically predilects metaphysis of the long bones. It is locally invasive/infiltrative, and this tendency is more concerning in the skull base, where a radical resection is often technically impossible because of the presence of vital neurovascular structures. We present a case of a 19-year-old woman who presented with a sudden onset of right facial weakness, progressively worsening to a severe disfiguring motor weakness. Gadolinium-enhanced brain magnetic resonance imaging showed an osteolytic lesi

UM-Chor1: establishment and characterization of the first validated clival chordoma cell line.

OBJECTIVE Chordomas are rare malignant tumors thought to arise from remnants of the notochord. They can be located anywhere along the axial skeleton but are most commonly found in the clival and sacrococcygeal regions, where the notochord regresses during fetal development. Chordomas are resistant to many current therapies, leaving surgery as the primary method of treatment. Cancer cell lines have been useful for developing new cancer treatments in a laboratory setting that can then be transferred to the clinic, but there are only 4 validated chordoma cell lines available. The objective of thi

Shattering the Rock: Technique of Bilateral Optic Nerve Mobilization and Drilling Heavily Calcified Craniopharyngiomas for Its Excision.

BACKGROUND: Resection of heavily calcified craniopharyngioma is quite challenging. The stretched optic nerves, perforators, and stalk are likely to be jeopardized further during attempts to break the calcified chunks, especially through narrow corridors. We describe a surgical technique to mobilize bilateral optic nerves and drill the calcified chunk to crumple it. METHODS: This technique was used in 6 patients with heavily calcified craniopharyngiomas (2 recurrent) who had presented with progressive visual loss. Frontotemporal craniotomy was used in 5 patients, and fronto-temporo-orbito-zygo

Treatment of a skull-base giant cell tumor with endoscopic endonasal resection and denosumab: case report.

A 34-year-old man with a 1-week history of diplopia was referred to the authors' hospital. Neurological examination revealed left abducens nerve palsy. Computed tomography showed a lesion in the left sphenoid sinus involving the medial wall of the left internal carotid artery (ICA) and osteolytic change at the clivus bordering the lesion. Magnetic resonance imaging demonstrated an extensive soft-tissue mass occupying the left sphenoid sinus. Surgical intervention by the endoscopic transnasal method allowed most of the lesion to be removed. Only the portion attached to the medial wall of the IC

Stereotactic radiosurgery in the management of vestibular schwannoma and glomus jugulare: indications, techniques, and results.

Gamma Knife stereotactic radiosurgery (GKS) has become an important management strategy for an increasing number of patients with skull base tumors. For select patients with lateral skull base disorders, given the proximity to sensitive critical structures such as the brainstem, cranial nerves, and cochlea, this technology has emerged as a first-line treatment to achieve the paramount goals of long-term tumor control and maintenance of existing neurologic function. This article reviews the indications, technique, and results of GKS for the treatment of vestibular schwannoma and glomus jugulare

Meningioma arising from the anterior skull base and filling the nasal cavity.

Meningiomas are usually benign tumors that are frequently encountered in the intracranial region. They account for 15% of the overall intracranial tumors. Of the intracranial meningiomas, less than 3% extend up to the sinonasal region. The frequency of meningioma in females is 2 times higher than that in males; it is most commonly observed in the fifth decade of life. It is usually asymptomatic and detected incidentally. A small number of meningiomas may cause some symptoms including seizure attacks and cranial nerve paralysis according to their locations. On the physical examination of a 56-y

The fully endoscopic supraorbital trans-eyebrow keyhole approach to the anterior and middle skull base.

BACKGROUND: The purely endoscopic eyebrow supraorbital approach is an alternative to both the endoscopic endonasal and eyebrow microsurgical routes to the anterior and middle cranial base. It combines an enhanced visualization provided by the endoscope and the absence of cerebrospinal fluid (CSF) leaks and nasal complications. METHODS: A trans-eyebrow 2-cm craniotomy is designed to allow the placement of a straight endoscope and visualize the skull base from the cribriform plate to the mesiotemporal lobe. RESULT: Visualization is considerably improved, while the keyhole mini-invasive concept

[Solitary intrasellar plasmocytoma revealed by a diplopia: a case report].

INTRODUCTION: Plasmocytomas rarely invade the skull base. It can be solitary or multiple. The clinical presentation mainly consists in a progressive neuropathy involving many cranial nerves, or may simply present as an optochiasmatic syndrome. OBSERVATION: The authors report a case of a female patient aged 68 years presenting with an optochiasmatic syndrome with an anosmia and paralysis of the right sixth (VI) cranial nerve progressing over two years prior to her hospitalisation. Cerebral MRI showed a voluminous intrasellar lesion, isointense on T1 and hyperintense on T2, enhancing intensely

Surgical anatomy of the jugular foramen.

The jugular foramen (JF) is a canal that makes communication between the posterior cranial fossa and the upper neck for one third of the cranial nerves and for the main venous channel of the brain. From a lateral view, the JF is protected by multiple layers of muscles and by the outer surface of the petrous bone. Surgical exposure of the JF is usually justified by the removal of benign tumors that grow in this region. In the first part of the present study we describe the surgical anatomy of the JF Then, we detail the relevant points of a stepwise surgical progression of three lateral skull ba

A de novo desmoid tumor of the surgical site following foramen magnum meningioma resection in a patient with Gardner's Syndrome: a case report and review of the literature.

BACKGROUND: Gardner's Syndrome is a familial adenomatous polyposis (FAP) syndrome that presents with colorectal polyps, craniofacial osteomas, lipomas, epidermoid cysts, and ampullary cancers. These patients are at significant risk for developing desmoid tumors, which are locally infiltrating fibromatous lesions. These are most commonly found in the abdominal cavity, but may be extra-abdominal. Although FAP-associated desmoid tumors often occur at sites of surgical scarring, this is the first report following resection of an intracranial meningioma. CASE DESCRIPTION: We present a patient with

Acute and long-term complications of therapeutic radiation for skull base tumors.

Acute toxicity from radiation of skull base tumors typically resolves spontaneously within days to weeks after completion of therapy. Long-term complications have a latency of months to years after radiation and are usually irreversible. The need to keep the probability of chronic toxicity at an acceptable level has determined the upper limits of radiation doses for the treatment of tumors. The threat of chronic toxicity thus affects the ability to control skull base tumors with radiation. Use of conformal radiation techniques with image guidance, allowing minimization of normal tissue volume

Transnasal endoscopic excision of midline nasal dermoid from the anterior cranial base.

We recommend that every patient with a midline nasal dermoid be evaluated preoperatively by fine-cut CT scan, in the axial and coronal planes, using both bone and soft-tissue algorithms. Because of CT limitations for assessing soft tissue at the cranial base, a complementary thin-section, high-resolution, multiplanar MRI scan should be considered. Transnasal endoscopic excision of midline nasal dermoid should be used if the dermoid is located within the nasal cavity, and there is minimal or no cutaneous involvement. This approach can be combined with a short vertical midline lenticular excisio

Combined interhemispheric translamina terminalis and pterional approach for a dorsum sellae meningioma.

This video demonstrates a surgical technique of resecting dorsum sellae meningioma using a combined interhemispheric translamina terminalis approach and pterional approach with clinoidectomy. The tumor, 5 cm in maximum diameter, originated from the dorsum sellae, compressed the third ventricle and the midbrain, and displaced the pituitary stalk ventrally. Feeding arteries of the tumor were bilateral meningohypophyseal trunks, mainly from the right side. The authors performed devascularization of the tumor via a right pterional approach following frontotemporal craniotomy, and debulking of the

Extended endoscopic transsphenoidal approach infrachiasmatic corridor.

An extended endoscopic transsphenoidal approach is required for skull base lesions extending to the suprasellar area. Inferior approach using the infrachiasmatic corridor allows access to the lesions through the tumor growth that is favorable for the extended transsphenoidal approaches. Infrachiasmatic corridor is a safer route for the inferior approaches that is made up by basal arachnoid membrane and Liliequist's membrane with its leaves (diencephalic and mesencephalic leaf). This area extends from the optic canal and tuberculum sella to the corpus mamillare. We performed extended endoscopic
