Obstructive sleep apnoea in the general population: highly prevalent but minimal symptoms.

The aim was to assess the prevalence of obstructive sleep apnoea (OSA) as defined by an apnoea-hypopnea index (AHI) ≥15 in the middle-aged general population, and the interrelationship between OSA, sleep-related symptoms, sleepiness and vigilance.A general population sample of 40-65-year-old Icelanders was invited to participate in a study protocol that included a type 3 sleep study, questionnaire and a psychomotor vigilance test (PVT).Among the 415 subjects included in the study, 56.9% had no OSA (AHI <5), 24.1% had mild OSA (AHI 5-14.9), 12.5% had moderate OSA (AHI 15-29.9), 2.9% had severe

An open trial of cognitive therapy for chronic insomnia.

We describe the development of a cognitive therapy intervention for chronic insomnia. The therapy is based on a cognitive model which suggests that the processes that maintain insomnia include: (1) worry and rumination, (2) attentional bias and monitoring for sleep-related threat, (3) unhelpful beliefs about sleep, (4) misperception of sleep and daytime deficits and (5) the use of safety behaviors that maintain unhelpful beliefs. The aim of cognitive therapy for insomnia is to reverse all five maintaining processes during both the night and the day. In an open trial 19 patients meeting diagnos

Circadian regulation of arousal: role of the noradrenergic locus coeruleus system and light exposure.

STUDY OBJECTIVES: Noradrenergic locus coeruleus (LC) neurons regulate arousal. Previous studies have shown that noradrenergic LC neurons exhibit a circadian rhythm in impulse activity, which peaks during the active period. This is mediated by an indirect circuit projection from the suprachiasmatic nucleus (SCN) to the LC. Here we sought to evaluate the hypothesis that the LC regulates the circadian properties of the sleep-wake cycle. DESIGN: Sprague-Dawley rats maintained on a light-dark (LD) schedule or in constant darkness (DD) for 3 to 4 weeks were treated with DSP-4, a neurotoxic agent sp

Pupil miosis within 5 minutes in darkness is a valid and sensitive quantitative measure of alertness: application in daytime sleepiness associated with sleep apnea.

STUDY OBJECTIVES: The regulation of arousal and pupillary functions may be intimately linked via activity in the nucleus locus coeruleus. In this preliminary study, we tested the validity of the gradual pupillary miosis during 5 minutes in darkness, as a quantitative physiologic index of the arousal state of the brain. DESIGN: Cross-sectional assessment of 2 groups with between-group comparison and correlational analyses within the patient group. PARTICIPANTS: Eleven unmedicated male patients recently diagnosed with obstructive sleep apnea (OSA) with no comorbid conditions who had undergone

Does amygdalar perfusion correlate with antidepressant response to partial sleep deprivation in major depression?

This study used functional MRI (fMRI) to clarify the sites of brain activity associated with the antidepressant effects of sleep deprivation (SD). We hypothesized: (1) baseline perfusion in right and left amygdalae will be greater in responders than in nonresponders; (2) following partial sleep deprivation (PSD), perfusion in responders' right and left amygdalae would decrease. Seventeen unmedicated outpatients with current major depression and eight controls received perfusion-weighted fMRI and structural MRI at baseline and following 1 night of late-night PSD. Baseline bilateral amygdalar pe

Fluvoxamine versus desipramine: comparative polysomnographic effects.

Electroencephalogram sleep measures over a 4-week period were obtained on 35 inpatients with major depression (unipolar) who received either fluvoxamine or desipramine in a randomized double-blind trial. Fluvoxamine showed immediate rapid eye movement (REM) sleep suppression and an alerting effect on sleep continuity measures. In contrast, desipramine administration was associated with REM suppression and sleep continuity improvement. The "alerting" quality of fluvoxamine, similar to other serotonergic antidepressants, appears to be unrelated to a lack of clinical efficacy, but may be related

Cognitive performance and mood in patients with chronic insomnia during 14-day use of flurazepam and midazolam.

The 99 chronic insomniacs examined in the present multicenter study were given three cognitive tasks (reading comprehension, addition, and digit symbol substitution test [DSST]) as well as the Hopkins Symptom Checklist (HSCL) and the Profile of Mood States (POMS) in order to evaluate the effects of flurazepam (Dalmane) 15 and 30 mg, midazolam 15 mg, and placebo on cognitive performance and mood. Subjective evaluation of performance was also obtained. A significant person in the patient's life was also asked to evaluate the patient's mood before and during the 14-day treatment interval. Antidepressant withdrawal-induced activation (hypomania and mania): mechanism and theoretical significance.

Electrocortical and behavioral arousal are separate phenomena subserved by different neural substrata operating in parallel. A comprehensive theory of 'activation' must take into account the relationships between the electrical and behavioral activating systems. In pathological or experimentally induced states paradoxes, resolvable by a theory positing functional interaction between these systems, arise. EEG arousal is directly mediated, in both the waking and sleeping state, by cholinergic mechanisms. Antidepressant withdrawal precipitates cholinergic overdrive; this would account for the app

A pilot study of sleep, cognition, and respiration under 4 weeks of intermittent nocturnal hypoxia in adult humans.

STUDY OBJECTIVES: A pilot study to examine the effects of intermittent nocturnal hypoxia on sleep, respiration and cognition in healthy adult humans. METHODS: Participants were eight healthy, non-smoking subjects (four male, four female), mean age of 26.4+/-5.2 years, and BMI 22.3+/-2.6 kg/m(2), exposed to 9h of intermittent hypoxia between the hours of 10 P.M. and 7 A.M. for 28 consecutive nights. At a simulated altitude of 13,000 feet (FIO(2) 0.13), intermittent hypoxia was achieved by administering nasal nitrogen, alternating with brief (approximately 5s) boluses of nasal oxygen. Pre- and

Amantadine use during acute hospitalization for neurological recovery after stroke: a systematic review.

BACKGROUND: Disorders of consciousness and impaired neurological recovery are common during acute hospitalization for stroke, yet evidence-based pharmacologic strategies to enhance neurorecovery remain limited. Amantadine is frequently used off-label in this setting based on data from traumatic brain injury; however, evidence supporting its use in stroke is unclear. OBJECTIVE: To systematically review the available literature evaluating the use of amantadine during acute hospitalization in adult patients with stroke. METHODS: This systematic review was conducted in accordance with PRISMA gui

Shift work sleep disorder: prevalence and consequences beyond that of symptomatic day workers.

STUDY OBJECTIVES: Although there are considerable data demonstrating the impact of shift work on sleep and alertness, little research has examined the prevalence and consequences of shift work sleep disorder in comparison to the difficulties with insomnia and excessive sleepiness experienced by day workers. The present study was designed to determine the relative prevalence and negative consequences associated with shift work sleep disorder in a representative sample drawn from the working population of metropolitan Detroit. DESIGN: Random-digit dialing techniques were used to assess individu

Obstructive sleep apnoea syndrome: comparison between polysomnography and portable sleep monitoring based on jaw recordings.

INTRODUCTION: Obstructive sleep apnoea syndrome (OSAS) constitutes a new major public health problem because of its several pathophysiologic consequences such as cognitive disorders, excessive daytime sleepiness with risks of traffic accidents, cardiovascular implications, and decrease of quality of life. The necessity of a gold-standard polysomnography to ensure an accurate diagnosis implies an expensive, technical and time-consuming examination. Thus, it seems logical to develop new systems so as to diagnose SAS and to make it possible to detect apnoeas/hypopnoeas easily during sleep even at

Pain coping strategies for tension-type headache: possible implications for insomnia?

STUDY OBJECTIVES: Insomnia has been identified as a risk factor for tension-type headache, although the pathogenesis of sleep disturbance in this population is unclear. The present study examined pain-related self-management strategies in a nonclinical, young-adult sample for preliminary evidence to support a novel hypothesis for the development of insomnia in this population. METHODS: Self-report data on triggers of headache, pain interference with sleep, and pain-related self-management strategies were analyzed for 32 women with tension-type headache and 33 women with minimal pain who serve

Differences between those who panic by day and those who also panic by night.

This study examined the characteristics, correlates, background demographic, and personality variables associated with panic among those who panic exclusively from a waking state and those who also panic out of sleep. Participants were recruited through advertising in print media and through anxiety/panic support groups. Some group differences in the characteristics of panic and the associations between panic and other variables were found. Specifically, levels of anxiety sensitivity were higher, panic duration was longer, and panic was less strongly related to catastrophic cognitions for the

Ketogenic metabolic therapy as a candidate adjunct for CBTp delivery in schizophrenia spectrum disorders.

Schizophrenia spectrum disorders (SSDs) are managed with pharmacotherapy alongside psychosocial interventions, including cognitive-behavioral therapy for psychosis (CBTp). In routine services, uptake and completion of CBTp remain limited, and participation barriers disrupt initiation, retention, and between-session follow-through. Ketogenic metabolic therapy (KMT) is a dietary intervention intended to induce and maintain nutritional ketosis through carbohydrate restriction and a higher fat dietary pattern, with education, monitoring, and safety management integrated into care. Early reports ut

Recent perspectives on orexin/hypocretin promotion of addiction-related behaviors.

The neuropeptide hypocretin/orexin plays a broad and important role in physiological functions ranging from addiction, stress, and anxiety to sleep, energy metabolism, and homeostatic regulation. A number of recent reviews addressing the importance of orexin for different addictive behaviors, especially the contribution of orexin-1-receptors (Ox1Rs) in responding for intoxicants in higher-motivation individuals and situations, and orexin-2-receptor (Ox2Rs) in stress-related aspects of addictive responding. This may parallel the importance of more lateral orexin neurons in the hypothalamus for

Stress and obesity/metabolic syndrome in childhood and adolescence.

Chronic distress contributes to the development of obesity and comorbid states. Stress is the disturbance of the complex dynamic equilibrium that all organisms must maintain, and is associated with activation of the Stress system comprising of the hypothalamic-pituitary-adrenal axis and the arousal/sympathetic nervous systems. The stress system functions in a baseline circadian fashion and interacts with other systems of the organism to regulate a variety of behavioral, endocrine, metabolic, immune and cardiovascular functions. The experience of perceived or real uncontrollable intense and/or

[Orexin: clinical and therapeutic implications].

INTRODUCTION. Recent research has reported the existence of a new class of neuropeptides, called orexins or hypocretins, which are produced by a small group of neurons in the hypothalamus and whose actions are mediated by two types of receptors: OX1R and OX2R. More specifically, the orexinergic neurons have been located exclusively in cells in the lateral, dorsomedial and perifornical areas of the hypothalamus. Despite this highly specific anatomical origin, the orexinergic neurons are projected widely into a number of brainstem, cortical and limbic regions. DEVELOPMENT. This fuzzy pattern of
