[Endoscopic treatment in acute biliary pancreatitis].

Overall mortality in acute pancreatitis is over 10%, but exceeds 40% in acute forms. In France, it has been estimated that 40 to 60% of the acute forms are of biliary origin, usually by obstruction of the pancreatic duct. Endoscopic sphincterotomy can liberate the main bile duct and separate the bile ducts from the pancreatic ducts in a minimally invasive procedure. Nevertheless, the decision to use an endoscopic treatment rather than conservative medical treatment raises several questions. Can the endoscopic technique alleviate the obstacle causing pancreatitis? Can it improve the prognosis o…

A rare case: Transcatheter coil embolization in a patient with cystic duct stump injury following cholecystectomy.

Acute cholecystitis is a common inflammatory condition of the gallbladder, primarily associated with gallstones. Complications such as bile duct injury and bile leakage can occur following cholecystectomy, significantly impacting morbidity and mortality. Early diagnosis and intervention are essential to improve patient outcomes, with treatment options including endoscopic sphincterotomy and stenting. However, percutaneous coil embolization has emerged as a critical intervention due to the anatomical complexity of the region.

[Differential indications for peroral pancreaticoscopy].

Pancreatoscopy by the new mother-baby-endoscopy-system is an effective and safe diagnostic technique. Before the pancreatic duct can be entered by the pancreatoscope (diameter 3.2 mm), a sphincterotomy of the pancreatic sphincter must be performed. In 11 of 13 cases, the macroscopic and cytologic or histologic diagnosis respectively could be made. Main indications for this technique are intraductally localized lesions, where endoscopically guided cytology or histology can clarify the dignity of the lesion.

Nitrate therapy in a patient with papillary dysfunction.

A 64-yr-old woman with ERCP manometry proven papillary dysfunction is described. This patient responded to nitrate therapy with disappearance of the pain and decrease in both basal and phasic sphincter activity. It is suggested that nitrate administration should be used to distinguish between patients with functional and organic pathology of the sphincter of Oddi. Nitrate therapy should be tried in patients with papillary dysfunction and only if this fails, should endoscopic sphincterotomy be considered.

Wirsungocele as a Rare Cause of Recurrent Pancreatitis: Etiology and Therapeutic Insights.

Wirsungocele, a cystic dilation at the end of the main pancreatic duct, is associated with recurrent acute pancreatitis. A 52-year-old man presented to our hospital with recurrent epigastric pain over an 8-month period with a history of multiple medical visits for the same complaint. Endoscopic ultrasound (EUS) and magnetic resonance cholangiopancreatography (MRCP) revealed focal cystic dilatation at the end of the main pancreatic duct; thus, he was diagnosed with Wirsungocele. He underwent endoscopic pancreatic sphincterotomy and 5Fr 4 cm pancreatic duct stent placement; the pancreatic duct s…

Evaluation of the effectiveness and safety of single-operator cholangiopancreatoscopy with the SpyGlass™ system.

BACKGROUND AND STUDY AIMS: The single-operator cholangiopancreatoscopy (SOCP) with the SpyGlass™ system is a endoscopy technique whose use h … 1.2%, respectively; while histology results were 36.4% and 100% respectively. A previous endoscopic sphincterotomy was associated with a lower rate of AEs (2.4% vs 41.7%; p<0.001). CONCLUSIONS: SOCP with SpyGlass™ is a safe and effective technique to diagnose and treat pancreatobiliary pathology. The presence of sphincterotomy performed prior to the procedure c …

He had one repeat hospitalization for acute blood loss anemia attributed to sphincterotomy, which did not require transfusion or further intervention. Liver biopsy showed cholestatic hepatitis. "Post-cut": An endoscopic technique for managing impacted biliary stone within an entrapped extraction basket.

BACKGROUND AND STUDY AIMS: Ampullary impaction of an entrapped stone-basket complex is not an infrequent yet challenging event during endoscopic retrograde cholangiopancreatography (ERCP). The aim of this study is to evaluate the feasibility, safety, and efficacy of "post-cut" for the management of such scenarios. PATIENTS AND METHODS: Patients with impacted biliary stone with an entrapped basket during ERCP at West China Hospital, Chengdu, China, from October 2004 to August 2014 were included in this retrospective study. Adequate biliary sphincterotomy was performed in all cases before attem…

ERCP and wire-guided sphincterotomy were performed in 6 (43%) of 14 patients with gallstone-related pancreatitis and elevated liver enzymes with no complications. Effects of large-balloon dilation on the major duodenal papilla and the lower bile duct: histological evaluation by using an ex vivo adult porcine model.

BACKGROUND: Several articles have reported the usefulness of large-balloon dilation after endoscopic sphincterotomy in removing bile duct stones. Its histological effect on the duodenal papilla and the lower bile duct, however, is not well established. OBJECTIVE: To elucidate the histological consequence of large-balloon dilation. DESIGN: Animal experiment. SETTING: A referral center. MATERIALS AND INTERVENTIONS: After the evaluation of normal anatomy of the major duodenal papilla and the lower bile duct in resected specimens from pigs, large-balloon dilation (12-20 mm) after small sphinct…

For procedures with a higher risk of bleeding (e.g. polypectomy and biliary sphincterotomy), an individual approach is required. This approach might include stopping oral anticoagulant therapy with or without the administration of unfractionated heparin or low-molecular-weight heparin for the pre-procedure and post-procedure periods, duri …

Botulinum toxin A injection in ISDN ointment-resistant chronic anal fissures.

BACKGROUND: In the treatment of chronic anal fissures (CAFs), surgical sphincterotomy is more commonly being replaced by chemical sphincterotomy. After the good results of our pilot study including 32 patients, we now describe the effect of botulinum toxin A (BT-A) in a consecutive series of 100 patients with isosorbide dinitrate (ISDN) ointment-resistant CAFs. METHODS: From October 2002 until August 2005, 100 patients (52 males, 48 females) with a median age of 45 (20-79) years were treated with an injection of 40-100 IU BT-A (Dysport, Ipsen, The Netherlands) in the internal anal sphincter. …

Endoscopic management of tumors of the major duodenal papilla: Refined techniques to improve outcome and avoid complications.

BACKGROUND: Adenomas of the major duodenal papilla have malignant potential and are traditionally treated by pancreaticoduodenectomy. This is a report of our experience with endoscopic management and a description of techniques for decreasing complications and enhancing efficacy. METHODS: Forty-one patients were referred for endoscopic management of papillary tumors. If there was no duct invasion and the appearance suggested a benign lesion, biductal sphincterotomy with pancreatic duct stent placement was performed. If the lesion could be elevated by injection of an epinephrine solution, piec…

Outcome of mechanical lithotripsy of bile duct stones in an unselected series of 704 patients.

BACKGROUND/AIMS: This paper evaluates the potential benefit of non mechanical bile duct stone lithotripsy techniques. The efficacy, limitations and risks of mechanical lithotripsy as first choice procedure were studied. MATERIAL AND METHODS: Endoscopic sphincterotomy was performed by Erlangen-type papillotomes, stone extraction by Olympus baskets and mechanical lithotripsy by the Wurbs-system. In an unselected series of 704 patients, everyone with common bile and hepatic duct stones (independent of size, number, location and stone consistency) was included in the study. RESULTS: Complete sto…

Anti-CMV treatment and sphincterotomy were unsuccessful in treating anomalies of the intrahepatic biliary tract.
