Incidence, Causes, and Timing of Unplanned Reoperations Following Three-Column Osteotomy for Pediatric and Adult Spinal Deformities: A Long-Term Single Center Study.

This study provides one of the most extensive long-term follow-up analyses of unplanned revisions following 3CO for spinal deformities, with a mean follow-up duration of 9.8 years, which has been underexplored in prior research. The findings provide critical data for preoperative discussions with patients and their families regarding the risks of unplanned reoperation. Additionally, the study highlights the need for long-term surveillance and proactive strategies to mitigate revision risks, particularly in patients undergoing multilevel 3CO.

The impact of preoperative supine radiographs on surgical strategy in adult spinal deformity.

OBJECTIVE: Preoperative planning for adult spinal deformity (ASD) surgery is essential to prepare the surgical team and consistently obtain postoperative alignment goals. Positional imaging may allow the surgeon to evaluate spinal flexibility and anticipate the need for more invasive techniques. The purpose of this study was to determine whether spine flexibility, defined by the change in alignment between supine and standing imaging, is associated with the need for an osteotomy in ASD surgery. METHODS: A single-center, dual-surgeon retrospective analysis was performed of adult patients with

The Risk of Curve Progression and Surgery in African Americans With Adolescent Idiopathic Scoliosis.

STUDY DESIGN: Retrospective clinical cohort study. OBJECTIVE: To determine if certain risk factors (age, curve magnitude, skeletal maturity, gender, and curve pattern) traditionally associated with curve progression and surgical intervention in the general population apply equally to African Americans. SUMMARY OF BACKGROUND DATA: Currently, information is limited on the role that a patient's race plays in the risk of curve progression of adolescent idiopathic scoliosis (AIS), and existing studies have conflicting results. METHODS: Retrospective search of records identified patients who were

The relationship between sagittal curvature and extensor muscle volume in the lumbar spine.

A previous modelling study predicted that the forces applied by the extensor muscles to stabilise the lumbar spine would be greater in spines that have a larger sagittal curvature (lordosis). Because the force-generating capacity of a muscle is related to its size, it was hypothesised that the size of the extensor muscles in a subject would be related to the size of their lumbar lordosis. Magnetic resonance imaging (MRI) data were obtained, together with age, height, body mass and back pain status, from 42 female subjects. The volume of the extensor muscles (multifidus and erector spinae) caud

Natural history of spinopelvic alignment differs from symptomatic deformity of the spine.

STUDY DESIGN: Cross-sectional study and systematic review of the literature. OBJECTIVE: Describe the natural history of spinopelvic alignment parameters and their behavior in patients with degenerative spinal deformity. SUMMARY OF BACKGROUND DATA: Normal stance and gait requires congruence between the spine-sacrum and pelvis-lower extremities. This is determined by the pelvic incidence (PI), and 2 positional parameters, the pelvic tilt, and sacral slope (SS). The PI also affects lumbar lordosis (LL), a positional parameter. The final goal is to position the body's axis of gravity to minimize

Posterior vertebral column resection for severe pediatric deformity: minimum two-year follow-up of thirty-five consecutive patients.

STUDY DESIGN: Retrospective review of a prospectively accrued patient cohort. OBJECTIVE: The ability to treat severe pediatric spinal deformity through an all-posterior vertebral column resection (VCR) has obviated the need for a circumferential approach in both primary and revision settings. We examined indications, correction rates, and complications of this challenging procedure in the pediatric population. SUMMARY OF BACKGROUND DATA: Traditionally, severe pediatric spinal deformities were treated through a combined anterior/posterior spinal fusion. METHODS: Between 2000 and 2005, 35 con

The reliability and diagnostic value of radiographic criteria in sagittal spine deformities: comparison of the vertebral wedge ratio to the segmental cobb angle.

STUDY DESIGN: A prospective, radiographic cohort study. OBJECTIVES: This study assessed the radiographic reliability and diagnostic value of the vertebral wedge ratio (WR) to the more segmental Cobb angle (CA) regarding sagittal spine deformities. SUMMARY OF BACKGROUND DATA: The use of the CA has been used to assist in the radiographic diagnosis of various sagittal spine deformities. However, the reliability and diagnostic aptitude of the CA remains speculative and may not be as receptive to individual variations of vertebral integrity in sagittal spine deformities. METHODS: Sixty patients

[Early-onset scoliosis: pathophysiology, diagnosis and treatment].

Early-onset scoliosis corresponds to a group of heterogeneous spinal conditions that present before 10 years of age with a curvature in the coronal plane of 10° or more. Regardless of the etiology, adequate control of the deformity is required to avoid cardiopulmonary deterioration and preserve quality and life expectancy. Diagnosis can be difficult and is usually incidental at a young age. To choose the best treatment, it is important to consider the patient's age, etiology, and rate of progression. Among the non-operative treatments, the most effective are the use of serial casts or brace, w

Comparison of ultrasound scanning for scoliosis assessment: Robotic versus manual.

BACKGROUND: Ultrasound (US) imaging for scoliosis assessment is challenging for a non-experienced operator. The robotic scanning was developed to follow a spinal curvature with deep learning and apply consistent forces to the patient's back. METHODS: Twenty three scoliosis patients were scanned with US device both, robotically and manually. Two human raters measured each subject's spinous process angles on robotic and manual coronal images. RESULTS: The robotic method showed high intra- (ICC > 0.85) and inter-rater (ICC > 0.77) reliabilities. Compared with the manual method, the robotic appr

Swim bladder tumor in the young adult scoliotic medaka (Oryzias latipes).

A swim bladder tumor was detected in one scoliotic medaka aged 22 weeks. The tumor was located in the dorsal abdominal cavity, with maximum dimension of 1,850 × 1,500 µm. No swim bladder lumen was identified, and the region where the swim bladder lumen would have been located, was replaced with adipose tissues. The tumor was a non-invasive, expansile, and encapsulated solid mass with a few cysts, and comprised a homogenous population of well-differentiated, densely packed, gas glandular epithelium-like cells. The tumor mass was connected to a rete mirabile that showed a hyperplastic capillary

Molecular genetic prenatal diagnosis for a case of autosomal recessive spondylocostal dysostosis.

Autosomal recessive spondylocostal dysostosis type 1 (ARSCD1) is a member of the heterogeneous group of disorders termed the spondylocostal dysostoses that are characterized by multiple vertebral segmentation defects and rib anomalies. In these patients, the entire vertebral column is malformed and is replaced by multiple hemivertebrae giving rise to truncal shortening, abdominal protrusion and non-progressive spinal curvature. Genetic studies have shown that some cases of ARSCD are due to mutations in the somitogenesis gene, Delta-like 3 (DLL3), that encodes a ligand for the Notch signalling

Muscle fiber types in thoracic erector spinae muscles. Fiber types in idiopathic and other forms of scoliosis.

Histochemical studies of the thoracic part of the erector spinae muscles in scoliosis have shown a consistently higher proportion of Type 1 fibers on the convex side. The fiber distribution of the multifidus and semispinalis muscles was studied in adolescent idiopathic scoliosis (AIS). Compared with sex- and age-matched controls, a "normal" percentage of Type 1 fibers was found on the convex right side and significantly smaller percentage of Type 1 fibers on the concave side. Boys with AIS showed the same pattern as girls. In two patients with double major curves, the same pattern could be see

Modified Posterior Vertebral Column Resection for Patients with Thoracolumbar Kyphotic Deformity.

Old compression vertebrae fracture or congenital kyphoscoliosis with abnormal vertebral body development and other diseases that invade the spine may cause severe thoracolumbar kyphotic deformity, often accompanied by intractable low back pain or compression of the spinal cord, leading to severe neurological symptoms or even paralysis. If conservative treatment cannot relieve the symptoms or correct the deformities, surgical treatment is usually needed. For severe kyphotic deformity, reconstruction of the physiological curvature and rigid fixation determine the prognosis of the patients. Osteo

An experimental study in chickens for the pathogenesis of idiopathic scoliosis.

Experimentally induced scoliosis was investigated in pinealectomized chickens using pathologic and neurophysiologic means. A total of 90 chickens were tested; 30 served as a normal control, 30 received an autografted pineal body in the intramuscular tissue of the trunk, and 30 underwent pinealectomy without autograft. Scoliosis developed in all pinealectomized chickens within 2 weeks, showing gradual progression during the next 5 or 6 weeks. At 3 months, the three-dimensional spinal deformity consisted of lateral curvature and vertebral body rotation, resulting in a prominent lordoscoliosis at
