PPARβ/δ contributes to the antidiabetic effect and the increase in GDF15 caused by metformin.

Metformin, the most prescribed drug for the treatment of type 2 diabetes mellitus, increases the circulating levels of the metabolic regulator growth differentiation factor 15 (GDF15) via transcriptional regulation, with the kidneys being responsible for this increase. Since peroxisome proliferator-activated receptor (PPAR)β/δ agonists mimic many of the effects of metformin, including the rise in circulating GDF15 levels, we herein investigated whether the metformin-mediated antidiabetic effects and GDF15 upregulation were dependent on this nuclear receptor. Male Ppard-/- and wild-type (WT) mi

Blood PCSK9 Impacts Alzheimer's Disease Risk in an APOE Genotype-Dependent Manner: A Prospective Cohort Study.

BACKGROUND AND AIMS: Apolipoprotein E (APOE) and proprotein convertase subtilisin/kexin type 9 (PCSK9) are both lipid proteins and related to immunity/inflammation. We hypothesized that PCSK9 impacts on Alzheimer's disease (AD) risk in an APOE genotype dependent manner. METHODS: We used the Framingham Heart Study (FHS) Offspring cohort (Gen 2), with data on plasma PCSK9 protein concentration, as the baseline exposure for 1,704 study subjects. Using Cox regression models, the outcomes were incidents of AD or all-cause dementia. Using another FHS dataset with 3,048 individuals with genetic data

Beyond lipid lowering: Effects of PCSK9 inhibition on inflammation and HDL function.

BACKGROUND: Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibition effectively lowers low-density lipoprotein cholesterol (LDL-C) and cardiovascular risk, but its pleiotropic effects remain insufficiently defined. This study examined whether PCSK9 inhibition influences vascular and systemic inflammation and high-density lipoprotein (HDL) antioxidant function. OBJECTIVE: To investigate the effects of PCSK9 inhibition on vascular inflammation, systemic inflammatory markers, and HDL antioxidant function in a real-world patient cohort. MATERIAL AND METHODS: In this monocentric, prospec

PCSK9 and Breast Cancer Survival: A Mendelian Randomization Study.

BACKGROUND: Proprotein convertase subtilisin/kexin type 9 (PCSK9) affects lipid metabolism. A recent study identified an association between rs562556 within the PCSK9 gene and breast cancer survival (BCS), suggesting PCSK9 inhibition as an early intervention strategy to prevent metastasizing breast cancer. We attempt to replicate these findings using genome-wide association study (GWAS) data and Mendelian Randomization (MR). METHODS: We used GWAS data from the Breast Cancer Association Consortium (BCAC, N = 91,686) and performed analyses in the FinnGen study (N = 4,648; additive and recessive

Efficacy and Safety of an Early Aggressive Lipid-Lowering Strategy Using Triple Therapy After Mechanical Thrombectomy.

BACKGROUND: Current guidelines recommend initial high-intensity statin monotherapy and "the wait and watch paradigm," even in patients with acute ischemic stroke (AIS). However, newer lipid-lowering therapies allow aggressive low-density lipoprotein cholesterol (LDL-C) reduction without drug class restriction. The effectiveness of triple therapy (early aggressive strategy) comprising a proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor, ezetimibe, and a high-intensity statin after endovascular thrombectomy (EVT) for emergent large vessel occlusion (ELVO) remains uncertain. Novel drugs approved by the EMA, the FDA and the MHRA in 2025: A year in review.

In the 2025 novel drug mini-review, one can take a full measure of the ingenuity that underlies current drug design and development, despite the year's smaller harvest (46 novel drugs) compared to 2024 (53) and 2023 (70). 54% of the novel drugs are first-in-class (FIC). The emphasis on proteins/antibodies is maintained (~25% novel drugs in 2025), an industry trend that does not seem to abate. Fewer than half of the novel medicines address major or common disorders. Among the FIC drugs, it is worth mentioning the Nav1.8 channel inhibitor suzetrigine, the first non-opioid approved to palliate ac

LDL subspecies and lipidome change by evolocumab add-on therapy to empagliflozin in patients with type 2 diabetes: A prespecified secondary analysis of a randomized clinical trial.

BACKGROUND: Sodium-glucose cotransporter-2 inhibitors (SGLT2i) and proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) reduce cardiovascular risk in type 2 diabetes. Whether these benefits are partly mediated by modifications in low-density lipoprotein (LDL) phenotype, beyond changes in LDL-cholesterol levels, remains unknown. METHODS: In this prespecified analysis of the EXCEED-BHS3 randomized trial, 110 individuals with type 2 diabetes were assigned to 16 weeks of empagliflozin (E) or empagliflozin plus evolocumab (EE). Changes in LDL subspecies profiles were assessed in the f

PCSK9 inhibitor treatment and outcomes in patients with atherosclerotic cardiovascular disease but without prior ischaemic events: an observational study.

BACKGROUND AND AIMS: Low-density lipoprotein cholesterol (LDL-C)-lowering therapies are proven effective in atherosclerotic cardiovascular disease (ASCVD), but real-world evidence for proprotein convertase subtilisin/kexin type 9 inhibitor (PCSK9i) monoclonal antibodies (mAb) remains limited. This study evaluated their impact in patients with ASCVD without prior ischaemic events. METHODS: Patients initiating PCSK9i mAb from January 2016 to December 2022 were identified in the Optum Research Database. A 1:2 propensity score-matched comparator cohort of PCSK9i non-initiators was developed. The

Multi-biomarker approach for predicting cardiac magnetic resonance parameters at 30 days after ST-segment elevation myocardial infarction.

BACKGROUND: Established biomarkers are used for risk stratification after STEMI, but their relationship with cardiac magnetic resonance (CMR)-derived infarct characteristics at 30 days remains incompletely understood. Because 30-day CMR reflects an intermediate post-infarction stage between acute myocardial injury and the later chronic infarct state, we evaluated whether baseline high-sensitivity troponin T (hsTnT), N-terminal pro-B-type natriuretic peptide (NT-proBNP), cellular communication network factor 1 (CCN1), and proprotein convertase subtilisin/kexin type 9 (PCSK9) are associated with

Lipoprotein(a) reduction with inclisiran, alirocumab, evolocumab, enlicitide, and lerodalcibep: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND AND AIMS: Lipoprotein(a) [Lp(a)] is a causal contributor to atherosclerotic cardiovascular disease (ASCVD). While no therapies are currently approved solely for lowering Lp(a), subgroup analyses suggest that individuals with elevated Lp(a) may gain added benefit from intensive lipid-lowering strategies. No prior meta-analysis has compared evolocumab, alirocumab, inclisiran, lerodalcibep, and enlicitide in their Lp(a)-lowering efficacy. We aimed to determine whether proprotein convertase subtilisin/kexin type 9 (PCSK9) targeted therapies differ significantly in Lp(a) reduction, or wh

Safety and Efficacy of Oral PCSK9 Inhibitors in Hypercholesterolemia: A Systematic Review and Network Meta-Analysis.

OBJECTIVE: Oral proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) show promise for the management of hypercholesterolemia, but a systematic review and network meta-analysis are needed to inform clinical practice; we aimed to fill this gap. METHODS: Multiple databases were searched through November 14, 2025, to identify randomized controlled trials (RCTs) comparing oral PCSK9i with placebo in adults with hypercholesterolemia. Primary outcomes were adverse events and percent change in low-density lipoprotein cholesterol (LDL-C); secondary outcomes included LDL-C goal attainment

Antibody E06 and the Role of Oxidized Phospholipids in Atherogenesis and Inflammation.

Oxidized phospholipids (OxPL) are generated at sites of oxidative stress and tissue injury, where they function as prototypic danger-associated molecular patterns that trigger inflammation, cell death, and tissue remodeling. The natural IgM antibody E06, and derivative formats such as E06-scFv, recognize the phosphocholine epitope on OxPL but not on native phospholipids, providing powerful tools to probe the biology of OxPL across cardiovascular and noncardiovascular disease. In this review, we summarize experimental and translational evidence implicating OxPL as mediators of atherogenesis, my

Biological Mechanisms Underlying the Cardiovascular Effects of Branched-Chain Amino Acids: A Proteome-Wide Mendelian Randomization Study.

BACKGROUND: Ischemic heart disease (IHD) is the leading cause of morbidity and mortality. Branched-chain amino acids (BCAAs) are associated with higher IHD risk, but the underlying biological pathways remain unclear. OBJECTIVES: This study aims to explore these pathways using 2-step proteome-wide Mendelian randomization. METHODS: We examined the associations between genetic proxies for BCAAs and 2922 proteins in the United Kingdom Biobank Pharma Proteomics Project, supplemented by a meta-analysis with data from deCODE to identify proteins associated with BCAAs. Next, we tested their effects
