Tamsulosin for Prevention of Urinary Retention After Midurethral Sling.

IMPORTANCE: Midurethral sling (MUS) is a common procedure to correct stress urinary incontinence but carries a risk of postoperative urinary retention (POUR). OBJECTIVE: The objective of this study was to evaluate the efficacy of perioperative tamsulosin for reducing the incidence of POUR in patients undergoing MUS placement, with or without concomitant pelvic organ prolapse (POP) surgery. STUDY DESIGN: This was a retrospective cohort study of female patients who underwent MUS placement, with or without concomitant gynecologic surgery. Patients were excluded if they underwent transobturator

Timing of Midurethral Sling Placement During Minimally Invasive Sacrocolpopexy.

IMPORTANCE: Surgeons traditionally place the synthetic midurethral sling (MUS) after completing the apical suspension procedure during surgical procedures for pelvic organ prolapse (POP) and stress urinary incontinence (SUI). We hypothesize it may be more efficient to perform the MUS procedure at the beginning of the case without compromising MUS treatment success. OBJECTIVE: The objective of this study was to determine whether MUS timing affects SUI treatment outcomes 3 months after surgery. STUDY DESIGN: A retrospective cohort study was performed of patients who had minimally invasive sacr

Route matters: comparing vaginal (TOT) and laparoscopic (MERT-Burch) approaches in stress urinary incontinence and postoperative sexual function recovery.

BACKGROUND: Stress urinary incontinence (SUI) negatively affects women's sexual well-being and quality of life through anxiety about urine leakage during intercourse. AIM: To compare the effects of vaginal (transobturator tape, TOT) and laparoscopic (Multiple Endoclip Retraction Technique-Burch, MERT-Burch) routes on postoperative sexual function, quality of life, and time to comfortable coitus. METHODS: This retrospective study included 114 sexually active women who underwent TOT (n = 55) or laparoscopic MERT-Burch colposuspension (n = 59) between January 1, 2021, and August 5, 2025. Long-Term Outcomes of Transvaginal Mesh in Younger Women: A Retrospective Study.

INTRODUCTION AND HYPOTHESIS: Despite FDA bans due to safety concerns, transvaginal mesh (TVM) offers lower recurrence rates for pelvic organ prolapse (POP) compared to native tissue repair. Current Chinese guidelines recommend TVM for recurrent or severe POP (POP-Q stage III-IV) in patients over 60 years. However, evidence regarding its long-term outcomes in younger patients (median age ≤ 60 years) remains limited. METHODS: This retrospective study included women who underwent TVM from 2013 to 2023 at a tertiary medical center. Baseline characteristics and perioperative details were extracted

Preoperative Analgesia in Midurethral Sling: A Cohort Study.

INTRODUCTION AND HYPOTHESIS: Stress urinary incontinence (SUI) is a common condition among adult women in the USA, with its prevalence expected to rise owing to an aging population. Midurethral sling (MUS) procedures are a well-established surgical treatment for SUI. Enhanced Recovery After Surgery (ERAS) protocols advocate for multimodal pain management strategies, including preoperative analgesia. However, limited data exist on the effectiveness of preoperative pain medications in MUS procedures. This study examines the impact of preoperative analgesia on postoperative pain scores and opioid

Impact of United States Food and Drug Administration Position Statements on Complication Rates Within 30 Days After Stress Urinary Incontinence Sling Surgery.

INTRODUCTION: The US Food and Drug Administration (FDA) has issued 3 notices regarding the use of synthetic material in stress urinary incontinence (SUI) sling surgeries. In 2011, the FDA published a 15-page report on the use of mesh in SUI procedures, reviewing nearly 4000 cases of adverse events including erosion, infection, pain, and difficulty urinating. This study investigated the impact of these notices on sling placement patterns and their complication rates. METHODS: This was a retrospective analysis of the American College of Surgeons National Surgical Quality Improvement Program dat

Long-term effectiveness and safety of open Burch colposuspension vs retropubic midurethral sling for stress urinary incontinence-results from a large comparative study.

BACKGROUND: There are few adequately powered long-term trials comparing midurethral sling and Burch colposuspension. Recent concerns about synthetic mesh with new stringent clinical and research governance support the need for evidence to facilitate shared decision making. OBJECTIVE: This study aimed to compare long-term outcomes of open Burch colposuspension with the retropubic midurethral sling. STUDY DESIGN: A matched cohort study of 1344 women with urodynamic stress incontinence (without intrinsic sphincter deficiency) who underwent surgery for stress urinary incontinence. Women had eith

Current Role of Urethrolysis and Partial Excision in Patients Seeking Revision of Anti-Incontinence Sling.

OBJECTIVES: Stress urinary incontinence is highly prevalent and sling surgery has increased since 2000. Urethrolysis traditionally had been standard management of complications after anti-incontinence surgery; however, partial excision is a less aggressive option. This study describes the different populations in a contemporary cohort that undergo sling excision and urethrolysis and their surgical outcomes. METHODS: Chart analysis was performed on patients assigned Current Procedural Terminology codes for removal or revision of sling for stress incontinence, urethrolysis, or revision of graft

Care Seeking Patterns for Women Requiring a Repeat Pelvic Organ Prolapse Surgery Due to Native Tissue Repair Failure Compared to a Mesh Complication.

OBJECTIVE: To explore patient migration patterns in patients requiring repeat surgery after Pelvic Organ Prolapse (POP) repair as there is a limited understanding of care seeking patterns for repeat surgery after POP repair. We hypothesized that undergoing repeat surgery for a prolapse mesh complication would be associated with an increased incidence of migration to a new facility for care compared to those undergoing repeat surgery for recurrent POP. METHODS: In this retrospective population based study, all females who underwent an index POP repair procedure (with or without mesh) at nonfed
