Spring Ligament Reconstruction for Progressive Collapsing Foot Deformity: Contemporary Review.

The spring ligament is one of the main stabilizers of the medial arch of the foot and the primary static supporter of the talonavicular join … f nonanatomic and anatomic reconstruction techniques that have been described, including autologous tendon transfers, allografts, and synthetic augmentation. Although many have only been characterized in biomechanical cadaver studies, this article reviews preliminary clinical studies that have shown promising results. There is a need for more high-quality studies …

Flexor Hallucis Longus decompression under ultrasound guidance: A cadaver study.

INTRODUCTION: The Flexor Hallucis Longus (FHL) is a muscle that can be subject to multiple impingements caused by exaggerated plantar flexion in athletes. The most common impingement is due to inflammation of the tendon at the retrotalar pulley. The constraints exerted on the FHL are responsible for a pathology called functional Hallux Limitus. Surgical treatment consists of tenolysis of the FHL arthroscopically or via open surgery, by opening the pulley. The objective of this study was to evaluate the risk of lesions of the neurovascular pedicle and the posterior tibial tendon after tenolysis

Management of a Thumb-In-Palm Deformity in Adult Patients With Upper Motor Neuron Syndrome.

The thumb-in-palm (TIP) deformity in adults with upper motor neuron syndrome results from an imbalance of extrinsic and intrinsic muscular f … nonfunctional, absent motor control. Techniques include tenotomies, tenodeses, tendon lengthenings, tendon transfers, tendon reroutings, neurectomies, and joint releases. A focused physical examination is key in developing a patient-specific treatment algorithm.

Supernumerary flexor carpi ulnaris--case report and review.

Anomalies of Flexor carpi ulnaris (FCU) are uncommon and predominantly consist of abnormal insertion or extra tendinous bands. The presence of two separate bellies of FCU muscle with separate tendons has been termed as Digastric FCU. It is a very rare anomaly with only six previously reported cases and no report of clinical use of such anomalous muscle. We describe a case of anomalous FCU found incidentally in a patient operated for complex Post Volkmann's ischemic contracture deficits. On table each head of FCU was found to form separate belly and tendon. One tendon was used for the thumb opp

Stabilization of finger ulnar deviation in rheumatoid arthritis: extensor indicis proprius tenodesis.

Ulnar deviation of fingers in rheumatoid arthritis is a common deformity that can markedly compromise hand function. These deformities also result in an aesthetic impairment that is often troubling to patients. A variety of surgical procedures have been recommended to correct the ulnar deviation, either at the wrist level or at the metacarpophalangeal (MCP) joints. We present a surgical technique that uses the extensor indicis proprius as a tenodesis across the dorsum of the MCP joints of the fingers. The technique can also be used in conjunction with other surgical procedures, including prost

Neuromuscular issues in cavovarus foot.

Differential muscle weakness can cause a cavus foot deformity. Presenting complaints in the hindfoot may include ankle instability, secondary arthritis, or peroneal tendonitis. Presenting complaints in the forefoot may include stress fractures, callus formation over the lateral border of the foot, claw toes, first ray overload, and metatarsalgia. More general presenting complaints can include a drop-foot gait, decreased walking tolerance, and difficulty with shoe or orthotic fitting. To surgically correct the foot shape, soft tissue contractures need to be released, bone deformity corrected, a

Reconstruction of the anterior cruciate ligament with quadriceps tendon.

The author describes the technique he uses to reconstruct the anterior cruciate ligament (ACL) arthroscopically with autologous bone-quadriceps tendon (BQT) graft. The patellar bone is fixed in a femoral tunnel about 2.5 cm long, in a position that allows the tendinous extremity of the graft to appear on the extra-articular exit of the tibial tunnel. The tibial tunnel is filled, making the integration of the tendon in the bone easier. The tendinous extremity of the graft is pulled by nonabsorbable wires, remaining strictly fixed by a staple or a screw in the anterointernal cortex of the tibia.

The interposition of extensor digitorum brevis in the resection of calcaneonavicular bars.

In this series of 19 resected calcaneonavicular bars for peroneal spastic flatfoot (mean follow-up 3.4 years), we demonstrate that interposition of extensor digitorum brevis led to asymptomatic mobile feet without evidence of recurrence of the bar in 90% of the patients; this compares well with 43% recurrence rates of bars with associated pain and stiffness when extensor digitorum brevis is not interposed. We therefore recommend this method for prevention of the bar and their symptoms.

[Current aspects of treatment for clubfoot in infants and young children].

The congenital club-foot is one of the frequent malformations (incidence around 2%). The treatment during early childhood implies conservative measures with corrective manipulations and casting, surgical procedures with extensive soft tissue releases. The technically demanding medial release is a well established procedure; however it is often not performed because the peritalar malpositions are often underestimated in the young child. After surgical correction stabilization of the foot for prolonged time is important and follow-up is mandatory until adulthood is reached.

The interfascicular matrix enables fascicle sliding and recovery in tendon, and behaves more elastically in energy storing tendons.

While the predominant function of all tendons is to transfer force from muscle to bone and position the limbs, some tendons additionally function as energy stores, reducing the cost of locomotion. Energy storing tendons experience extremely high strains and need to be able to recoil efficiently for maximum energy storage and return. In the equine forelimb, the energy storing superficial digital flexor tendon (SDFT) has much higher failure strains than the positional common digital extensor tendon (CDET). However, we have previously shown that this is not due to differences in the properties of

A comprehensive study of the extensor tendons to the medial four digits of the hand.

BACKGROUND: Awareness of the anatomy and variations of the extensor tendons on the dorsum of the hand is necessary when assessing the traumatized or diseased hand and when considering tendons for repair or transfer. A complete quantitative documentation of the extensor tendons is lacking. METHOD: The arrangements of extensor tendons to the medical four fingers namely, the extensor digitorum communis (EDC), extensor indicis proprius (EIP) and extensor digiti minimi (EDM) on the dorsum of the hand and the intertendinous connections between them were studied in 100 upper limb specimens. Motion analysis and surgical results of anterior transfer of flexor hallucis longus for equinovarus gait in children with hemiplegia.

BACKGROUND: Rigid equinovarus foot deformities are seen in patients with cerebral palsy (CP). This retrospective study was undertaken to evaluate flexor hallucis longus tendon (FHL) transfer with gastrocsoleus recession (GSR) using motion analyses and quantitative measurement, and to investigate postoperative complications. METHODS: This study included 10 hemiplegic CP patients who underwent FHL transfer with GSR, and were evaluated by motion analyses consisting of weight distribution in static standing position and three-dimensional gait analysis, both pre and post-operatively. They were ass

[Modified collateral ligament reconstruction with the tendon of the abductor pollicis tendon in treatment of chronic radial instability of the basal thumb joint].

Injuries to the radial collateral ligament of the MP joint of the thumb occur much less frequently than injuries to the ulnar collateral ligament. Ligament injury is usually a result of direct trauma. The resultant instability to the thumb usually impairs hand function. In acute lesions to the radial collateral ligament, direct repair is often possible. In cases of chronic radial joint instability, secondary ligament reconstruction with a free tendon graft, most commonly the palmaris longus tendon, is usually recommended. We describe another technique using the tendon insertion of the abductor

Long thoracic nerve injury.

Injury to the long thoracic nerve causing paralysis or weakness of the serratus anterior muscle can be disabling. Patients with serratus palsy may present with pain, weakness, limitation of shoulder elevation, and scapular winging with medial translation of the scapula, rotation of the inferior angle toward the midline, and prominence of the vertebral border. Long thoracic nerve dysfunction may result from trauma or may occur without injury. Fortunately, most patients experience a return of serratus anterior function with conservative treatment, but recovery may take as many as 2 years. Bracin
