The clinical and laboratory findings in a patient with severe gastro-intestinal bleeding and the Ehlers-Danlos syndrome are described. Coagulation studies and a lack of history of previous haemorrhage were consistent with a diagnosis of acquired von Willebrand's syndrome. His response to treatment with blood transfusion, Factor VIII, cimetidine, tranexamic acid and cyclophosphamide is described. Family studies revealed other members with Ehlers-Danlos syndrome but normal coagulation.

Ten patients with acute promyelocytic leukemia (APL) were treated with all-trans retinoic acid (ATRA). Eight of 10 patients achieved complete remission (CR), and among the 8 newly diagnosed cases, 7 achieved CR. Five of 8 CR cases remained in CR after 8 to 30 months. We report a case of a 67-year-old Chinese man admitted to the emergency department for a persistent bleed on the tongue. Initial digital pressure and application of topical tranexamic acid did not stop the bleeding. An improvised clamp applied to the tongue was successful in achieving haemostasis. Using this method, there was no need for a clinical staff to be present for manual application of pressure. This frees up the staff for other clinical duties and matters.

Tissue fibrinolysis in the rat stomach and transverse colon was studied after X-ray irradiation of the abdomen. Tissue biopsies were examined on unheated fibrin plates and with a histochemical technique modified from Todd. A significantly increased fibrinolytic activity was found in the mucosa of the stomach and colon 3 days after irradiation, while fibrinolysis localized to submucosal vessels was unchanged. Mucosal fibrinolysis in the stomach was significantly inhibited by intragastric administration of tranexamic acid.

A variety of haemostatic agents can be helpful for patients with von Willebrand disease in case of bleeding. There are orally administered substances, such as tranexamic acid, oestrogens and prednisone; agents for topical or local use; and medicines for injection or infusion, including desmopressin, blood plasma products and recombinant activated factor VII. The choice depends on the type of haemorrhage, the severity of the disease, presence of inhibitors, availability and cost. Some of these aspects are reviewed here.
