OBJECTIVES: To identify changes in loneliness and carer stress between two time points for older people of different ethnicities who had repeated interRAI home care assessments.
METHODS: Participants consisted of community-dwelling older adults across New Zealand who received two interRAI-HC assessments between 5 July 2012 and 31 December 2019. Two multistate models were developed: the first model was not lonely versus lonely, and the second model was no carer stress versus carer stress. The one-year transition probabilities were calculated. Mean sojourn times were calculated for each state e

Surgery is a risk factor for chronic pain and long-term opioid use. As perioperative pain management continues to evolve, treatment modalities traditionally used for chronic pain therapy may provide additional benefit to patients undergoing surgery. Interventions such as radiofrequency ablation, cryoneurolysis, and neuromodulation may potentially be used in conjunction with acute pain procedures such as nerve blocks and multimodal analgesia. Pharmacological agents associated with chronic pain medicine, including gabapentinoids, ketamine, and selective serotonin reuptake inhibitors, may be usef

A 43-year-old woman and a 73-year-old man were referred separately from primary care to the urology service with short histories of frank haematuria. In both cases, histology from transurethral resection of their bladder tumours demonstrated the rare clear cell variant of urothelial/transitional cell carcinoma. Staging scans found the former patient had low-volume local disease, and the latter had locally advanced disease. The former patient went on to have partial cystectomy and pelvic lymph node dissection (with the endoscopic portion of the partial cystectomy undertaken by holmium:YAG laser

BACKGROUND: Disorders of the respiratory transition at birth are major reasons for neonates being admitted to intensive care units and separated from their mothers. This has potential long-lasting consequences for the mother-infant interaction as well as the long-term development of the child. There is an urgent need for effective preventive and therapeutic measures for this frequent disorder.
CASE REPORT: We report the cases of 2 newborn infants with respiratory transitional disorder treated off-label with the anthroposophic medicament Pulmo/Vivianit comp. based on pathophysiological conside

Critically ill patients have severe sleep disruption and typically encounter loss of circadian sleep pattern, steep fragmentation, increasing proportions of transitional stages of sleep, and loss of slow wave and rapid eye movement sleep. Mechanical ventilation is associated with these same sleep abnormalities, but what is attributable to the intensive care unit environment versus mechanical ventilation itself may be difficult to discern. Recent studies have shown that the ventilator mode and inappropriate settings can contribute to sleep fragmentation, and it is important to avoid overventila

Transitional cell carcinoma of the upper urinary tract most frequently occurs in the sixth and seventh decades of life. Standard care for these tumors involves a nephroureterectomy with removal of a cuff of bladder. Many investigators now recommend parenchymal-sparing operations in selected patients to avert the previous inevitability of dialysis. Recently, advancements in endoscopic technology have afforded two new, less invasive complementary techniques for the diagnosis and treatment of these lesions: ureteroscopy and nephroscopy. Ureteroscopy, due to its less invasive nature, is generally

IMPORTANCE: The United States has the world's highest rate of incarceration. Clinicians practicing outside of correctional facilities receive little dedicated training in the care of patients who are incarcerated, are unaware of guidelines for the treatment of patients in custody, and practice in health care systems with varying policies toward these patients. This review considers legal precedents for care of individuals who are incarcerated, frequently encountered terminology, characteristics of hospitalized incarcerated patients, considerations for clinical management, and challenges during

IMPORTANCE: Medication-related events (MREs) are a frequent cause of emergency department (ED) visits and patient harm.
OBJECTIVE: To asses … lar between groups.
CONCLUSIONS AND RELEVANCE: In this randomized clinical trial, a pharmacist-led transitional care intervention in the ED among patients with MRE significantly reduced ED visits and hospitalizations for the same MRE. If replicated in subsequent trials, this strategy offers a novel approach for reducing drug-related harms.

The necessity for a well-prepared and sustainable transition to adult care is evident, especially for adolescents with complex diseases. For the field of endocrinology this particularly involves the transfer in cases of hypothalamic-pituitary dysfunction, diseases of the adrenal and thyroid glands, disorders of glucose metabolism as well as rare genetically linked syndromes with associated endocrinopathies, hereditary disorders of bone metabolism and tumor predisposition syndromes. These often belong to the so-called group of rare diseases and necessitate a high level of specialized multidisci

BACKGROUND: The COVID-19 pandemic mandated rapid transition from face-to-face encounters to teleneurology visits. While teleneurology is regularly used in acute stroke care, its application in other branches of neurology was limited. Here we review how the recent pandemic has created a paradigm shift in caring for patients with chronic neurological disorders and how academic institutions have responded to the present need.
METHOD: Literature review was performed to examine the recent changes in health policies. Number of outpatient visits and televisits in the Department of Neurology was revi

Pneumonia is a major cause of mortality worldwide and a serious problem in critical care medicine, but the immunophysiological processes that confer either protection or morbidity are not completely understood. We show that in response to lung infection, B1a B cells migrate from the pleural space to the lung parenchyma to secrete polyreactive emergency immunoglobulin M (IgM). The process requires innate response activator (IRA) B cells, a transitional B1a-derived inflammatory subset which controls IgM production via autocrine granulocyte/macrophage colony-stimulating factor (GM-CSF) signaling.

AIM: This exploratory study evaluates rating scale usage by experts from the European Reference Network for Rare Neurological Diseases (ERN-RND) for paediatric MD, considering factors like diagnosis, intellectual disability, age, and transition to adult care. The aim is to propose a preliminary framework for consistent application.
METHODS: A multicentre survey among 25 ERN-RND experts from 10 European countries examined rating scale usage in paediatric MD, categorizing MD into acute, non-progressive, and neurodegenerative types. Factors influencing scale choice and the transition to adult ca

INTRODUCTION: Patients suffering from metastatic upper gastrointestinal cancer are burdened by physical, social, existential and psychological problems, though documentation of these problems may be insufficient. In Denmark, basic palliative care is fragmented and characterised by quality differences. This challenges cohesive palliative care interventions as patients experience transitions in the course of illness. The aim of this study was to identify and present the illness trajectory and to investigate the documentation of palliative needs for patients with metastatic upper gastrointestinal

BACKGROUND: The prison population in England and Wales exceeds 88,000, with a high turnover - 47% of sentenced admissions in 2023 served less than 12 months. Transitions from prison to the community are recognised as high-risk periods for medication-related harm, driven by complex health needs, short custodial stays, and fragmented healthcare systems. While national and international guidance exists to support safe medication management, implementation during prison-community transitions remains inconsistent, and evidence on both the drivers of unsafe medication practices and potential solutio

Transitions of care, such as intrahospital handoffs and hospital discharge, are high-risk periods for adverse events like medication errors and diagnostic delays, often stemming from communication lapses and care fragmentation. Evidence-based strategies, including multidisciplinary team collaboration, standardized handoff protocols, and electronic health record tools, can mitigate these risks and improve patient safety. Emerging technologies, such as artificial intelligence and remote monitoring systems, offer opportunities to personalize care and provide real-time support. Engaging patients a

Successful transition of patients between health care entities is difficult. Historically, the process of transitioning individuals through the corrections system with regard to health care was made even more challenging by the limitation of paper records. Recently, the advent of electronic medical records has improved health care nationwide. The use of health information exchange (HIE) systems in areas such as emergency medicine has also impacted patient outcomes. To date, this technology has not been used in the corrections system. Semi-structured interviews were conducted with 12 correction

Quitlines provide evidence-based tobacco treatment and multiple calls yield higher quit rates. This study aimed to identify subgroups of smokers with greater quitline engagement following referral during hospitalization. Data were from a randomized clinical trial assessing the effectiveness of fax referral (referral faxed to proactive quitline) versus warm handoff (patient connected to quitline at bedside) (n = 1054). Classification and regression trees analyses evaluated individual and treatment/health system-related variables and their interactions. Among all participants, warm handoff, high

Nurse practitioners are essential members of the multidisciplinary team managing heart transplant candidates and recipients, a patient population with highly specialized, complex care needs. Nurse practitioners contribute to successful patient outcomes by delivering expert care, facilitating smooth transitions, and maintaining continuity of care throughout the patient's journey. This continuum of care progresses from pretransplant evaluation and optimization, to the vulnerable perioperative and recovery period, and throughout the patient's transition back into the community, which entails life

INTRODUCTION: Post-intensive care syndrome affects more than half of intensive care unit (ICU) survivors, resulting in long-term physical, psychological, and cognitive impairments. Structured post-ICU recovery programs, including nurse-led follow-up, have improved patient outcomes in high-income countries. However, little is known about such models in low- and middle-income countries (LMICs), where post-ICU care is often lacking.
OBJECTIVE: The aim of this scoping review was to identify and map the existing nurse-led follow-up care interventions for ICU survivors in LMICs to inform future dev
