The effects of education on foot care behaviors and self-efficacy in type 2 diabetes patients.

BACKGROUND: Diabetic foot significantly affects the quality of life of patients with diabetes. It leads to loss of labor force, psychosocial trauma, and high treatment costs due to serious morbidity and mortality. Nurses have an important responsibility to improve the metabolic status of individuals with diabetes, to protect them from foot complications, and to teach patients foot care skills. AIM: This study investigated the effects of education on type 2 diabetes patients regarding diabetic foot care and self-efficacy. MATERIALS AND METHODS: This quasi-experimental study was conducted from

Demographic & clinical correlates of admission into a specialized psychiatric inpatient service for children and adolescents in Egypt: An observational retrospective study.

WHAT IS KNOWN ON THE SUBJECT: Admitting children and adolescents in psychiatric inpatient units is a relatively new and still debatable practice in Egyptian society and in the entire Arab world. WHAT THE PAPER ADDS TO EXISTING KNOWLEDGE: Egyptian young people diagnosed with complex behavioural problems, such as in other parts of the world, desperately need and do get benefit from inpatient psychiatric service. The demographic and clinical characteristics of children in need for such service are not clearly different from those reported in other parts of the world. WHAT ARE THE IMPLICATIONS F

Nephrolithiasis and Risk of Incident Bone Fracture.

PURPOSE: Higher urine calcium is a common feature of calcium nephrolithiasis and may be associated with lower bone mineral density in individuals with kidney stones. However previous population based studies of kidney stones and the risk of bone fracture demonstrate conflicting results. We examined independent associations between a history of kidney stones and incident fracture. MATERIALS AND METHODS: We performed prospective studies using data from the Nurses' Health Study of 107,001 women with 32 years of followup and the Health Professionals Follow-up Study of 50,982 men with 26 years of

Rh isoimmunization in Sub-Saharan Africa indicates need for universal access to anti-RhD immunoglobulin and effective management of D-negative pregnancies.

Transplacental or fetomaternal hemorrhage (FMH) may occur during pregnancy or at delivery and lead to immunization to the D antigen if the mother is Rh-negative and the baby is Rh-positive. This can result in hemolytic disease of the fetus and newborn (HDFN) in subsequent D-positive pregnancies. The aim of this study is to highlight the challenges associated with the effective management and prevention of Rh alloimmunization among Rh-negative women in Sub-Saharan Africa. In most Sub-Saharan African countries, there is poor and sometimes no alloimmunization prevention following potentially sens

Cost-effectiveness of microendoscopic discectomy versus conventional open discectomy in the treatment of lumbar disc herniation: a prospective randomised controlled trial [ISRCTN51857546].

BACKGROUND: Open discectomy is the standard surgical procedure in the treatment of patients with long-lasting sciatica caused by lumbar disc herniation. Minimally invasive approaches such as microendoscopic discectomy have gained attention in recent years. Reduced tissue trauma allows early ambulation, short hospital stay and quick resumption of daily activities. A comparative cost-effectiveness study has not been performed yet. We present the design of a randomised controlled trial on cost-effectiveness of microendoscopic discectomy versus conventional open discectomy in patients with lumbar

Cross-Sectional Survey of Critical Care Provision in Ministry of Health Hospitals in Zambia.

BACKGROUND: Critical care is an essential component of universal health care; however, its provision in low- and middle-income countries remains poorly understood. Although Zambia has expanded critical care services over the last 15 years, national evidence regarding workforce capacity, infrastructure and service provision remains limited. AIMS: To evaluate critical care workforce, service provision and patient case mix across Ministry of Health hospitals in Zambia. STUDY DESIGN: A cross-sectional survey was administered across 14 public hospitals with critical care units across Zambia, repr

Incidence of Brainstem Dysfunction and Brain Death and End-of-Life Care Practices: A Two-Year Retrospective Review.

OBJECTIVES: Brainstem dysfunction and brain death are important determinants of End-of-life care (EoLC) decisions in intensive care units (ICUs). In many low- and middle-income settings, withdrawal or withholding of life-sustaining treatment remain ethically and culturally challenging despite high costs and limited critical-care resources. This study reviewed the incidence, causes and outcomes of brainstem dysfunction/brain death and examined relatives' decisions regarding EoL care (EoLC) in a tertiary hospital ICU. MATERIALS AND METHODS: A retrospective chart review was conducted in the ICU

Posttraumatic Stress Disorder Is Associated with Increased Risk of Ovarian Cancer: A Prospective and Retrospective Longitudinal Cohort Study.

Ovarian cancer is the deadliest gynecologic cancer. Chronic stress accelerates tumor growth in animal models of ovarian cancer. We therefore postulated that posttraumatic stress disorder (PTSD) may be associated with increased risk of ovarian cancer. We used data from the Nurses' Health Study II, a longitudinal cohort study with 26 years of follow-up, conducted from 1989 to 2015 with 54,710 subjects. Lifetime PTSD symptoms were measured in 2008. Self-reported ovarian cancer was validated with medical records. Risk of ovarian cancer was estimated with Cox proportional hazards models and further

A cross-sectional pilot study of compassion fatigue, burnout, and compassion satisfaction in pediatric palliative care providers in the United States.

OBJECTIVE: Compassion fatigue (CF) is secondary traumatic distress experienced by providers from contact with patients' suffering. Burnout (BO) is job-related distress resulting from uncontrollable workplace factors that manifest in career dissatisfaction. Compassion satisfaction (CS) is emotional fulfillment derived from caring for others. The literature on BO in healthcare providers is extensive, whereas CF and CS have not been comprehensively studied. Because of ongoing exposure to patient and family distress, pediatric palliative care (PPC) providers may be at particular risk for CF. We co

Impact of a specialized multidisciplinary tracheostomy team on tracheostomy care in critically ill patients.

BACKGROUND: A multidisciplinary tracheostomy team was created in 2005 to follow critically ill patients who had undergone a tracheostomy until their discharge from hospital. Composed of a surgeon, surgical resident, respiratory therapist, speech-language pathologist and clinical nurse specialist, this team has been meeting twice a week for rounds involving patients who transitioned from the intensive care unit (ICU) to the medical and surgical wards. Our objective was to assess the impact of this multidisciplinary team on downsizing and decannulation times, on the incidence of speaking valve p

Multimedia article. Successful endoscopic wire-guided balloon dilatation of angulated and tight ileal pouch strictures without fluoroscopy.

BACKGROUND: Ileal pouch strictures that are visually inaccessible by an endoscope may be balloon-dilated by exchange guide wire across the stricture with the aid of fluoroscopy. We present a technique of wire-guided balloon dilation without fluoroscopy to navigate strictures in the ileal pouch. METHODS: A 50-year-old Caucasian female presented with a 24-year history of ulcerative colitis (UC) with restorative proctocolectomy and ileal pouch anal anastomosis (IPAA) for 7 years. She developed Crohn's disease (CD) of the pouch with multiple strictures at the afferent limb of the pouch and a pouc

Disposable laryngeal tube suction--a randomized comparison of two insertion techniques performed by novice users in anaesthetised patients.

OBJECTIVE: Laryngeal tubes are supraglottic airway devices that can be used in alternative to a tracheal tube to provide ventilation during cardiopulmonary resuscitation. The product line has recently been expanded by the disposable laryngeal tube suction (LTS-D). We tested the hypothesis that, with a modified insertion technique (MIT), the rate of correct placement attempts within 45 s could be significantly increased compared to the standard insertion technique (SIT) recommended by the manufacturer. METHODS: Fifty-four adult patients undergoing trauma surgery under general anaesthesia had a

Evaluating Emergency Department Transfers for Falls in Skilled Nursing Facilities: A Single-Site Observational Study of Witnessed and Unwitnessed Falls.

OBJECTIVES: Falls are a leading cause of morbidity and mortality among older adults, with residents of skilled nursing facilities (SNFs) at a particularly high risk. Although emergency department (ED) transfers after falls are common, they carry risks of delirium, infection, and care fragmentation. The objective of this study is to compare ED transfer rates between witnessed and unwitnessed falls in SNFs. DESIGN: Single-site, observational study. SETTING AND PARTICIPANTS: A 120-bed SNF in the greater Roanoke area, serving both long-term and short-term rehabilitation residents. Data were coll

Social deprivation results in longer perioperative admission and decreased rates of home discharge after distal femur fracture.

INTRODUCTION: Distal femur fractures (DFF) are orthopaedic injuries associated with poor outcomes, despite recent improvements in technology. Perhaps socioeconomic distress is a factor influencing DFF outcomes. The purpose of this study was to investigate the impact of social deprivation via the Area Deprivation Index (ADI) in the perioperative period after DFF. MATERIALS AND METHODS: This was a retrospective review carried out at a Level I Academic Trauma Center of patients ≥ 55 years old with operatively treated distal femur fractures from 2000-2023. Demographic information, Charlson Comorb

"Anything can happen": experiences of people using opioids in a xylazine market.

BACKGROUND: Xylazine is associated with a number of negative health outcomes, such as severe wounds and withdrawal. As xylazine increasingly emerges in drug markets across the United States, there is an urgent need for a national response to reduce related harm. As such, healthcare systems, harm reduction organizations, and community members continue to work to identify and develop the best approaches for harm reduction interventions to address xylazine challenges. This study engaged people who use opioids (PWUO) in Philadelphia to explore current experiences with and responses to xylazine adu

Palliative care patient emergency department visits at tertiary university-based emergency department in Ireland.

INTRODUCTION: Palliative care patients often present to the emergency department (ED) for various reasons e.g., acute illness, pain, altered mental status, and complications of therapy. Many visits involve less severe etiologies e.g., dyspnea, constipation, fear as patients approach the end of life, which may be more effectively and efficiently managed outside of the ED. The objective of this study is to identify and assess the frequency of presenting complaints, primary diagnosis, triage acuity, need for admission, in an Irish setting. METHODS: A single-center retrospective, observational st
