Interest of tumor necrosis factor-alpha -308 G/A and interleukin-10 -592 C/A polymorphisms in human African trypanosomiasis.

This study aimed to determine whether single nucleotide polymorphisms (SNPs) within tumour necrosis factor-alpha (TNF) and interleukin-10 (IL10) promoters and genes are associated with human African trypanosomiasis (HAT). The polymorphisms used in the analysis were TNF(-308G/A), TNF(-238G/A), TNF(-1031T/C), TNF(+488G/A), IL10(-1082G/A) and IL10(-592C/A). A familial case-control sample of 277 individuals (102 cases and 175 parents) and a matched case-control group of 225 subjects (88 cases and 137 unrelated controls) were gathered together in this study. A conditional logistic regression was us…

Disruptions of ultradian and circadian organization of core temperature in a rat model of African trypanosomiasis using periodogram techniques on detrended data.

Periodogram techniques on detrended data were used to determine the incidence of Trypanosoma brucei brucei infection on the distribution of the core temperature of rats and the expression of temperature rhythms. In such an animal model, sudden episodic hypothermic bouts were described. These episodes of hypothermia are used here as temporal marks for the purpose of performing punctual comparisons on temperature organization. The experiment was conducted on 10 infected and 3 control Sprague-Dawley rats reared under a 24 h light-dark cycle. Core temperature was recorded continuously throughout t

[Trypanosomiasis from Trypanosoma brucei gambiense in the center of north-west Uganda. Evaluation of 5 years of control (1987-1991)].

In Uganda, a case-finding and treatment programme has been implemented by Médecine Sans Frontières (MSF) and the Ministry of Health in the North of West-Nile province. Data collected in the hospital of Moyo from January 1987 to June 1991 were analyzed. Forty eight hundred and twenty two cases of trypanosomiasis due to T. B. gambiense has been recorded. Cumulative incidence rate for this period was 5.6%. Passive and active case-finding strategies were used, both based on Card Agglutination Test (CATT) as screening tool, followed by parasitological examinations. The mobile teams identified 1906 …

[Treatment of experimental trypanosomiasis of sheep caused by Trypanosoma brucei brucei: efficacy of Ro 15-0216 (a 2-nitroimidazole derivative)].

An experiment was carried out on treatment of Trypanosoma brucei brucei experimentally infected sheep with central nervous system involvement with a 2-nitroimidazole derivative: Ro 15-0216 (Hoffmann La Roche & Co Ltd, Basel, Switzerland). After a single intramuscularly or intravenously administration (25 or 50 mg/kg) of this compound in healthy sheep, levels of more than 1 microgram/ml corresponding to trypanocidal concentrations are detected in both blood and cerebrospinal fluid. However, because of short half life, two or three injections a day appeared to be necessary. Six infected sheep we

[Epidemiological importance of the animal reservoir of Trypanosoma brucei gambiense in the Congo. 1. Prevalence of animal trypanosomiasis in the foci of sleeping sickness].

Parasitological and serological tests were performed in 1287 domestic animals originating from the Brazzaville area and three sleeping sickness foci in Congo. Nannomonas and Trypanozoon were the only sub-genera present in the blood of animals, with prevalences of 16.9% and 0.5%, respectively. The use of Testryp CATT allowed a better estimation of the real frequency of animal trypanosomiasis which in some foci, was found to be three times higher than the parasitological prevalence. Seven stocks of Trypanozoon were isolated from pigs and sheep. A biochemical analysis indicated that they belonged…

Immune complexes in serum and in cerebrospinal fluid in African trypanosomiasis. Correlation with polyclonal B cell activation and with intracerebral immunoglobulin synthesis.

The possible occurrence of immune complexes (IC) in serum and in cerebrospinal fluid (CSF) has been studied in 36 patients with African trypanosomiasis (Trypanosoma brucei gambiense). In serum, very high levels of IC were detectable by the (125)I-C1q-binding and by the conglutinin-binding assays with positive results in 94 and 87%, respectively, of untreated patients. Circulating IC were found in both early and late stages of the disease, without significant quantitative differences; their size was 15-25S. There was a significant negative correlation between C3 values and C1qBA. Our studies su…

[The epidemiology of human trypanosomiasis due to Trypanosoma gambiense in a focus of the Ivory Coast. I. The distribution of the disease in the population (author's transl)].

The authors have studied the distribution of trypanosomiasis in the population of the focus around Vavoua in the Ivory Coast. Because of numerous immigrants from Upper Volta who have settled in the area, the composition of the population differs from the general population of the Ivory Coast. The cases diagnosed from 1977 to 1979 were studied by village, ethnic group, sex and age. The study shows that the highest incidence of sleeping sickness occurs among men, in the age-groups from 10 to 30 years and among immigrants from Upper Volta.

Modelling to Quantify the Likelihood that Local Elimination of Transmission has Occurred Using Routine Gambiense Human African Trypanosomiasis Surveillance Data.

BACKGROUND: The gambiense human African trypanosomiasis (gHAT) elimination programme in the Democratic Republic of Congo (DRC) routinely collects case data through passive surveillance and active screening, with several regions reporting no cases for several years, despite being endemic in the early 2000s. METHODS: We use mathematical models fitted to longitudinal data to estimate the probability that selected administrative regions have already achieved elimination of transmission (EOT) of gHAT. We examine the impact of active screening coverage on the certainty of model estimates for transm…

Nifurtimox-eflornithine combination therapy for second-stage African Trypanosoma brucei gambiense trypanosomiasis: a multicentre, randomised, phase III, non-inferiority trial.

BACKGROUND: Human African trypanosomiasis (HAT; sleeping sickness) caused by Trypanosoma brucei gambiense is a fatal disease. Current treatment options for patients with second-stage disease are toxic, ineffective, or impractical. We assessed the efficacy and safety of nifurtimox-eflornithine combination therapy (NECT) for second-stage disease compared with the standard eflornithine regimen. METHODS: A multicentre, randomised, open-label, active control, phase III, non-inferiority trial was done at four HAT treatment centres in the Republic of the Congo and the Democratic Republic of the Cong…

Semen sialic acid surge and modulation of alpha-L-fucosidase activity: possible link to loss in reproductive capacity during trypanosomiasis.

The profiles of semen sialic acid and the enzyme alpha-L-fucosidase were studied in rams undergoing chronic infection by Trypanosoma congolense. Our data showed a significant surge in the level of sialic acid with parasitaemia. The pattern followed a polynomial function we had reported for erythrocyte sialic acid in mice undergoing acute infection by T. congolense. The activity of the enzyme alpha-fucosidase decreased progressively with approximately 60% decrease at the end of the 14 weeks of infection. Representative semen samples from the control and infected rams were subjected to kinetic c

Wildlife surveys detect Trypanosoma brucei gambiense in duikers and mangabeys in Gabonese historical and active foci of human African trypanosomiasis.

Human African trypanosomiasis (HAT) continues to represent a public health concern in Central Africa, particularly in Gabon, where sporadic cases persist in historical foci despite ongoing elimination efforts. The existence of an autonomous sylvatic cycle involving non-human animal reservoirs has been hypothesized but remains insufficiently documented. The objective of this study was to characterise the diversity of trypanosomes present in wild animals from a number of historically and currently endemic provinces of Gabon. Between 2019 and 2022, a total of 240 blood samples were collected from…

Bold strides towards the elimination of gambiense human African trypanosomiasis (gHAT) as a public health problem-A case study of Angola.

BACKGROUND: The chronic form of human African trypanosomiasis (HAT) caused by Trypanosoma brucei gambiense and commonly referred to as gambiense-HAT (gHAT) is endemic in 7 of Angola's 18 provinces. Major epidemics of the disease occurred in the country in the 1920s to 1940s and 1990s -mid 2000s, and current estimates are that up to a third of the country's population is at risk of infection. Whereas gHAT was first reported in Angola in 1871, control efforts did not begin until 30 years later in 1901. This case study describes the history of gHAT in Angola, outlines the policies and strategies …

SNPs in IL4 and IFNG show no protective associations with human African trypanosomiasis in the Democratic Republic of the Congo: a case-control study.

Background: Human African trypanosomiasis (HAT) is a protozoal disease transmitted by tsetse flies. Infection with trypanosomes can lead directly to active HAT or latent infection with no detectable parasites, which may progress to active HAT or to spontaneous self-cure. Genetic variation could explain these differences in the outcome of infection. To test this hypothesis, polymorphisms in 17 candidate genes were tested ( APOL1 [ G1 and G2], CFH, HLA-A, HPR, HP, IL1B, IL12B, IL12RB1, IL10, IL4R, MIF, TNFA , IL6, IL4, IL8, IFNG, and HLA-G). Experiences of the one-health approach by the Uganda Trypanosomiasis Control Council and its secretariat in the control of zoonotic sleeping sickness in Uganda.

Elimination of sleeping sickness from endemic countries like Uganda is key if the affected communities are to exploit the potential of the available human and livestock resources (production and productivity). Trypanosoma brucei rhodesiense, the parasite that causes acute sleeping sickness in humans, is transmitted by tsetse flies and co-exists in non-human animal reservoirs. Uganda by Act of Parliament in 1992 decided to handle the complex approach to control of sleeping sickness and animal trypanosomiasis by establishing the Uganda Trypanosomiasis Control Council (UTCC) and its secretariat t…

Knowledge, attitudes and practices about human African trypanosomiasis and their implications in designing intervention strategies for Yei county, South Sudan.

BACKGROUND: A clear understanding of the knowledge, attitudes and practices (KAP) of a particular community is necessary in order to improve control of human African trypanosomiasis (HAT).New screening and diagnostic tools and strategies were introduced into South Sudan, as part of integrated delivery of primary healthcare. Knowledge and awareness on HAT, its new/improved screening and diagnostic tools, the places and processes of getting a confirmatory diagnosis and treatment are crucial to the success of this strategy. METHODOLOGY: A KAP survey was carried out in Yei County, South Sudan, to…

Impact of mass chemotherapy in domestic livestock for control of zoonotic T. b. rhodesiense human African trypanosomiasis in Eastern Uganda.

INTRODUCTION: Human African trypanosomiasis (HAT) comprises two fatal parasitic diseases. Uganda is home to both chronic T. b. gambiense (gHAT) and the acute zoonotic form T. b. rhodesiense (rHAT) which occur in two large but discrete geographical foci. The area affected by rHAT has been rapidly expanding due to importation of T. b. rhodesiense infected cattle into tsetse infested but previously HAT free districts. Migration of rHAT has resulted in a considerable human health burden in these newly affected districts. Here, we examined the impact of a single, district-wide, mass chemotherapeuti…

The design and synthesis of potent and selective inhibitors of Trypanosoma brucei glycogen synthase kinase 3 for the treatment of human african trypanosomiasis.

Glycogen synthase kinase 3 (GSK3) is a genetically validated drug target for human African trypanosomiasis (HAT), also called African sleeping sickness. We report the synthesis and biological evaluation of aminopyrazole derivatives as Trypanosoma brucei GSK3 short inhibitors. Low nanomolar inhibitors, which had high selectivity over the off-target human CDK2 and good selectivity over human GSK3β enzyme, have been prepared. These potent kinase inhibitors demonstrated low micromolar levels of inhibition of the Trypanosoma brucei brucei parasite grown in culture.
