Outbreak of pulmonary tuberculosis in a Chinese high school, 2009-2010.

BACKGROUND: In February 2009, a high school student was diagnosed with sputum-smear positive pulmonary tuberculosis (TB). One year later, 2 other students in the same grade developed sputum-smear positive TB. METHODS: We used tuberculin skin testing (TST), chest radiography, sputum smear, and symptomatology for case identification. We defined latent TB infection (LTBI) as a TST induration of 15 mm or larger, probable TB as a chest radiograph indicative of TB plus productive cough/hemoptysis for at least 2 weeks or TST induration of 15 mm or larger, and confirmed TB as 2 or more positive sputu

[Outbreak of extensively drug-resistant pulmonary tuberculosis in a hemodialysis facility].

We experienced an outbreak of extensively drug-resistant pulmonary tuberculosis (XDR-TB) in a hemodialysis facility. The primary case involved a 51-year-old male hemodialysis patient, with a history of treatment for Mycobacterium tuberculosis infection seven years previously. There was no drug resistance, and the patient completely recovered after undergoing treatment with isoniazid (INH), rifampicin (RFP) and ethambutol (EB). He was admitted to another hospital due to a recurrence of pulmonary tuberculosis in June 2006. At first, he was treated with HRS [INH, RFP and streptomycin (SM)]; howev

Chest radiographic findings in patients with HIV/AIDS and pulmonary tuberculosis.

The study describes the main chest radiographic changes in people living with HIV/AIDS and pulmonary tuberculosis, confirmed by sputum culture. This was a descriptive study involving a total of 42 sputum tests from 42 people living with HIV/AIDS and a clinical suspicion of pulmonary tuberculosis. All patients attended two referral hospitals in Recife-PE, Brazil, between August 2009 and January 2012. The most common isolated radiological change was parenchymal consolidation, encountered in six (14.3%) patients, followed by patterns of interstitial infiltrate, diffuse micronodular (miliary), and

Mycobacterium abscessus lung disease in a patient with previous pulmonary tuberculosis.

Patients with pre-existing lung damage, such as due to pulmonary tuberculosis (PTB), are susceptible to nontuberculous mycobacteria (NTM) infections. For patients with previous PTB, it is difficult to differentiate NTM lung disease from PTB, especially in tuberculosis (TB) high-burdened countries. Here, we report a case of Mycobacterium abscessus lung disease with a previous history of PTB. The patient underwent a right upper lobe lobectomy due to disappointing treatment response with anti-tuberculosis therapy. However, the disease worsened after the surgery. Five years later, she was diagnose

[Predictive factors for recurrence of pulmonary tuberculosis in Tunisia: a retrospective study].

INTRODUCTION: Tuberculosis is still endemic in Tunisia. Even though recurrent TB (RT) is rare, there is an increased risk of resistance. The purpose of this study is to investigate predictors of RT. PATIENTS AND METHODS: This was a case controlled retrospective study, comparing two groups of patients with confirmed pulmonary tuberculosis: a relapse group (GR) of 64 patients with RT and a control group (GT) of 105 patients. RESULTS: Between September 1995 and December 2007, the incidence of RT was 9.48%. All the patients were male. There was no difference in the average age of the two groups.

New and improved diagnostics for detection of drug-resistant pulmonary tuberculosis.

PURPOSE OF REVIEW: Tuberculosis (TB) remains a global emergency and continues to kill 1.7 million people globally each year. Drug-resistant TB is now well established throughout the world and most TB patients are not being screened for drug resistance due to lack of laboratory resources and rapid accurate point-of-care tests. Accurate and rapid diagnosis of TB and drug-resistant TB is of paramount importance in establishing appropriate clinical management and infection control measures. During the past decade, there have been significant advances in diagnostic technologies for TB and drug-resi

The evaluation of the Quantiferon-TB Gold test in pulmonary and extrapulmonary tuberculosis.

In our study we evaluated the Quantiferon-TB Gold (QFT-G) test and compared it with tuberculin skin test (TST) positivity in 44 pulmonary and 21 extrapulmonary tuberculosis cases in whom active tuberculosis infection was suspected on clinical, microbiologic and radiologic grounds. QFT-G positivity was 75% in pulmonary tuberculosis and 76.2% in extrapulmonary tuberculosis. TST positivity was 68.2 and 62%, respectively. Although QFT-G positivity was higher than TST positivity, the difference was statistically insignificant (P=0.77). Similar to TST, QFT-G is one of the supplementary diagnostic te

Prevalence and its predictors of extrapulmonary involvement in patients with pulmonary tuberculosis.

Extrapulmonary organ involvement in human immunodefiaency virus (HIV)-infected patients with pulmonary tuberculosis (TB) is reported to be 26%, however, the clinical predictors of extrapulmonary involvement in pulmonary TB patients has not been reported yet. We tried to determine the clinical predictors of presence of extrapulmonary involvement in patients with pulmonary TB. Cross-sectional study was performed including all adult patients with culture-proven pulmonary TB diagnosed between January 1, 2004 and July 30, 2006, at a tertiary referral hospital in South Korea. The presence of extra-p

Sputum microscopy for the diagnosis of HIV-associated pulmonary tuberculosis in Tanzania.

BACKGROUND: In many resource poor settings only sputum microscopy is employed for the diagnosis of HIV-associated pulmonary tuberculosis; sputum culture may not be available. METHODS: We determined the diagnostic accuracy of sputum microscopy for active case finding of HIV-associated pulmonary tuberculosis using TB culture as the reference standard. RESULTS: 2216 potential subjects screened for a TB vaccine trial submitted 9454 expectorated sputum specimens: 212 (2.2%) were sputum culture positive for Mycobacterium tuberculosis (MTB), 31 (0.3%) for non-tuberculous mycobacteria, and 79 (0.8%)

Autonomic nervous system dysfunction in the course of active pulmonary tuberculosis.

BACKGROUND: Functional alterations of the autonomic nervous system have been described in relation to chronic hypoxemia in chronic obstructive pulmonary diseases. Aim of the present study was to investigate the occurrence of neuro-vegetative dysfunction during active tuberculosis in the absence of hypoxemia. MATERIALS AND METHODS: Fifteen patients affected by pulmonary tuberculosis under standard therapy and 17 matched controls were enrolled. Activation of the sympathetic system was induced by the tilt-up test. Systolic and diastolic arterial pressures and the R-R interval were monitored for

[Characteristics of A-interferon-generated dendritic cells in patients with pulmonary tuberculosis].

The phenotype of the dendritic cells (DC) generated from the adhesion fraction of mononuclear cells in the presence of GM-CSF and alpha-interferon was studied in patients with pulmonary tuberculosis. Despite the absence of significant differences in the count of mature CD83+DCs in the groups of patients (n = 38) and healthy donors (n = 30), elevated CD14(+)-monocyte levels and few activated CD25(+)-DCs were indicative of the impaired process of DC maturation/generation in patients with pulmonary tuberculosis, particularly in a subgroup of patients with a low T-cell proliferative response again

Dosing schedules of 6-month regimens and relapse for pulmonary tuberculosis.

RATIONALE: The optimal approach for reducing tuberculosis relapse is open. OBJECTIVES: We examined the possibility of reducing relapse by increasing dosing schedules. METHODS: We conducted a systematic review of published clinical trials involving adult cohorts with pulmonary tuberculosis treated using 6-mo rifamycin-containing regimens, which were grouped under seven categories ordered by dosing schedules. Assuming cavitation and positive 2-mo culture were the driving forces for relapse, a static deterministic model apportioned observed numbers with and without relapse in each cohort into e

Resequencing and association analysis of the SP110 gene in adult pulmonary tuberculosis.

Recently, the Intracellular pathogen resistance 1 (Ipr1) gene was shown to control susceptibility to Mycobacterium tuberculosis in mice. We examined whether common sequence variants of its human orthologue, the SP110 gene, are associated with susceptibility to tuberculosis in a human population. We resequenced SP110 exons in 96 individuals and identified new polymorphisms. Then, we combined our sequence and HapMap data for 83 distinct polymorphisms and selected tags that capture information for all common variants in the 100 kb region around SP110. We genotyped 29 single nucleotide polymorphis

Pulmonary tuberculosis--a review of clinical features and diagnosis in 232 cases.

The diagnosis of pulmonary tuberculosis is often delayed due to atypical clinical features and difficulty in obtaining positive bacteriology. We reviewed 232 cases of pulmonary tuberculosis diagnosed in Kedah Medical Centre, Alor Setar from January 1998 to December 2002. All age groups were affected with a male predominance (Male:Female ratio = 60:40). Risk factors include underlying diabetes mellitus (17.7%), positive family history (16.8%) and previous tuberculosis (5.2%). Nearly half (45.3%) of patients had symptoms for more than one year. Only 22% of patients had typical symptoms of tuberc
