It is shown that in patients with ulcer associated with Helicobacter pylori (HP) there is a close correlation between the severity of the lesion of gastroduodenal protective mucous barrier and that of endothelial dysfunction manifesting in elevated level of endothelin-1, serum levels of TBK-active products, inhibition of blood flow and narrowing of the celiac trunk. The correlation becomes stronger with expanding contamination of gastroduodenal mucosa with HP. Thus, HP may participate in breaking the protective mucous barrier in endothelial dysfunction.

BACKGROUND: Peptic ulcer surgery may carry an increased risk for pancreatic cancer development. Molecular analysis of K-ras codon 12, frequently mutated in conventional pancreatic cancers, might provide insight into the aetiological mechanisms.
METHODS: The relative risk of pancreatic cancer was computed by multivariate and person-year analysis in a cohort of 2633 patients who had undergone gastrectomy. Lung cancer risk was analysed as an indirect means of assessing smoking behaviour. K-ras codon 12 mutational analysis was performed on 15 postgastrectomy pancreatic cancers.

Ulcerative gastroduodenal bleedings have remained one of the most complicated problems of emergency abdominal surgery up to the present. According to the literature, they make up approximately one half of all gastroduodenal bleedings and are accompanied by high lethality--from 10 to 30%. At that the highest percentage of lethality falls at elderly and old patients.

Results of organo-saving operations in 807 patients (72% men, 28% women) with duodenal ulcers were analyzed. Bilateral truncal vagotomy (with pyloro-, duodenoplasty, gastroenterostomy, gastroduodenostomy, economical stomach resection) were performed in 27.1% patients, selective--in 1.4%, selective proximal--in 65.5%, anterior selective and posterior truncal vagotomy--in 6.0%. Lethality was 1.6%, long-term results were excellent and good in 75.6% cases. Satisfactory results were seen in 8.6% patients, bad results--in 15.8%.

The treatment of solitary rectal ulcer syndrome (SRUS) remains problematic and is less than ideal. Prospective, well-designed studies assessing the efficacy of treatment for SRUS are few; most of the knowledge imparted for treating SRUS is experiential. As such, firm treatment recommendations can not be made. Rather, a conservative, stepwise, individualized approach must be employed. Sonography was performed in two patients with acute epigastric pain. Isolated thickening of the duodenal wall with an echogenic line within were considered to be signs of duodenal ulcer in keeping with a suggestive clinical background. The extension of this line beyond the duodenal wall and the periduodenal fluid were indicators of perforation. The images in our two patients indicate the usefulness of ultrasonography when performed carefully in selected cases.
