Impact of point-of-care multiplex PCR for Mycoplasma pneumoniae community-acquired pneumonia in the emergency department.

OBJECTIVES: Community-acquired pneumonia (CAP) due to Mycoplasma pneumoniae is a frequent and challenging diagnosis in Emergency Departments (EDs), often leading to unnecessarily broad-spectrum or ineffective antibiotic therapy. We hypothesize that rapid diagnostic tools, such as point-of-care (POC) devices, could improve initial antibiotic therapy appropriateness and reduce unnecessary additional diagnostic tests. METHODS: We conducted a retrospective, single-centre, study involving 81 patients visiting our ED with Mycoplasma pneumoniae CAP diagnosed between January 2023 and June 2024. Patie

Angiotensin-converting enzyme inhibitor-induced bowel angioedema: clinical features, diagnostic challenges, and recovery predictors from survival analysis: a systematic review of current reported cases.

BACKGROUND: Angiotensin-converting enzyme inhibitor-induced bowel angioedema (ACEi-IAE) is a rare and frequently under-recognized condition. Its nonspecific gastrointestinal symptoms could lead to missed diagnoses, unnecessary procedures and inappropriate treatments. Given the scarcity of studies, we conducted a systematic review to summarize the clinical characteristics of ACEi-IAE, the diagnostic approach and factors predicting delayed recovery. METHODS: Electronic databases, including MEDLINE, OVID and EMBASE, were used to identified eligible studies from inception to November 2024. Eligib…

Shoulder Calcific Tendinitis Presenting as Septic Arthritis: A Case Report.

Calcific tendinitis of the shoulder is a common condition in which calcium hydroxyapatite crystals form within the rotator cuff tendons, mos … on clinical exams and imaging. Accurate diagnosis is therefore difficult. Misdiagnosis may lead to unnecessary procedures or inappropriate treatment. Careful assessment with ultrasound and MRI helps distinguish calcific tendinopathy from infection. Recognizing typical features and choosing the right imaging are essential for optimal care. Allergic Bronchopulmonary Aspergillosis Presenting With Pulmonary Solid Lesions and Elevated Serum Carcinoembryonic Antigen Mimicking Lung Cancer.

BACKGROUND Allergic bronchopulmonary aspergillosis (ABPA) is a pulmonary disorder that occurs in patients allergic to Aspergillus species. It is typically characterized by reversible airway obstruction, transient pulmonary infiltrates, eosinophilia, and fever. Chest radiographs and high-resolution computed tomography (CT) are essential for diagnosis and differential diagnosis. Carcinoembryonic antigen (CEA), a tumor-specific marker, is often elevated in various malignant tumors but can also be increased in nonmalignant conditions. This overlap can result in misdiagnosis and unnecessary anxiety

Application of the Postnatal Growth and Retinopathy of Prematurity (G-ROP) criteria at a tertiary referral hospital.

BACKGROUND: Application of current retinopathy of prematurity (ROP) screening criteria results in many unnecessary examinations, because only 5%-10% of infants screened require treatment. Application of screening criteria established by the Postnatal Growth and Retinopathy of Prematurity Study could significantly reduce unnecessary examinations without sacrificing sensitivity to detect treatment-requiring ROP. We evaluated the performance of the G-ROP criteria in a population of high-risk, outborn infants. METHODS: The medical records of consecutive infants screened and/or treated for ROP at

Chilaiditi Syndrome Presenting as Partial Colonic Obstruction.

Chilaiditi sign is a rare incidental radiographic finding where bowel is interposed between the diaphragm and the liver, often seen as air under the right hemidiaphragm. A majority of patients with Chilaiditi sign are asymptomatic and remain so throughout their lifetime. Chilaiditi sign is recategorized as Chilaiditi syndrome if it becomes symptomatic and is a very rare etiology of bowel obstruction. As bowel obstruction confers a huge financial burden to the health care system, studies of even the rarer etiologies are of significant value. Particularly in the case of Chilaiditi syndrome, the

Clinician Practice Patterns That Result in the Diagnosis of Coccidioidomycosis Before or During Hospitalization.

BACKGROUND: Coccidioidomycosis (CM) is common and important within endemic regions, requiring specific testing for diagnosis. Long delays in diagnosis have been ascribed to ambulatory clinicians. However, how their testing practices have impacted patient care has not been systematically unexplored. METHODS: We analyzed practice patterns for CM diagnoses over 3 years within a large Arizona healthcare system, including diagnosis location, patient characteristics, and care-seeking patterns associated with missed diagnosis. RESULTS: For 2043 CM diagnoses, 72.9% were made during hospital admissio

Ogilvie syndrome: peculiar manifestation of acquired immunodeficiency syndrome in non-institutionalized middle age female in Tanzania.

Since it was first documented in 1948 by Sir William Heneage Ogilvie, numerous cases of Ogilvie syndrome have been described in literature due to various medical and surgical causes. Nonetheless, only a handful of cases only have been documented due to underlying Acquired Immunodeficiency Syndrome (AIDS). A 41-year-old female was admitted with an acute abdomen secondary to partial mechanical intestinal obstruction or paralytic ileus based on signs and symptoms and Abdominal X-Ray (AXR). She was known to be HIV/AIDS WHO clinical stage II on treatment. On diagnostic imaging studies she had diste

Ventriculoperitoneal Shunt Infections Cause Acute Abdomen and Peritonitis: A Case Series.

BACKGROUND: Patients with ventriculoperitoneal shunt (VPS) often present to emergency departments with acute abdomen. It is challenging to distinguish between the abdominal problems caused by a VPS and acute surgical abdomen having another cause because VPS infections occasionally cause peritonitis. The frequencies and clinical features of acute abdomen caused by VPS infection are unknown. METHODS: This was a retrospective analysis of all patients with a VPS who presented with acute abdomen to emergency department for a 10-year period. Clinical data, diagnostic workflow, and subsequent treatm

Invasive cervical resorption: etiology, diagnosis, classification and treatment.

BACKGROUND: Invasive cervical resorption (ICR) is not well understood by the professional, being misdiagnosed, leading to inappropriate treatment and unnecessary loss of tooth. INTRODUCTION: ICR is defined as a localized process of resorption, which begins in the cervical area of the tooth, just below the epithelial junction and above the ridge crest in the area of the connective tissue insertion. Possible predisposing factors include external trauma, orthodontic movement, surgical procedures, periodontal disease and its treatments, endogenous bleaching, pressure generated by wind instruments

Do religious restrictions influence ectopic pregnancy management? A national qualitative study.

INTRODUCTION: In the United States, ectopic pregnancies are relatively common and associated with significant maternal morbidity and mortality. The Ethical and Religious Directives for Catholic Health Care Services (the Directives) govern the provision of care in Catholic-affiliated hospitals and prohibit the provision of abortion in almost all circumstances. Although ectopic pregnancies are not viable, some Catholic ethicists have argued that the Directives preclude physicians at Catholic hospitals from managing tubal pregnancies with methods and procedures that involve "direct" action agains

Lasers and losers in the eyes of the law: liability for head and neck procedures.

IMPORTANCE: Although some have noted that malpractice litigation may be "plateauing," defensive medical practices are pervasive and make up a considerable proportion of the "indirect" costs medicolegal issues contribute toward our health care system. Accordingly, these trends have spurred considerable interest in characterizing factors that play a role in alleged medical negligence, along with outcomes and awards. OBJECTIVES: To conduct a focused examination of malpractice litigation regarding laser procedures in the head and neck and to determine the reasons for initiating litigation as well

EFSUMB Guidelines on Multiparametric Ultrasound Thyroid Nodule Evaluation: PART I.

Thyroid nodules are common incidental findings, but only a small proportion of cases are malignant (4-6.5%) or symptomatic. Numerous follow-up examinations and invasive diagnostic procedures, such as fine-needle aspiration, fine-needle biopsies, and thyroidectomies, are performed, leading to potentially costly and time-consuming diagnostic procedures and overtreatment. Most experts and scientific societies (EFSUMB, WFUMB, etc.) encourage the use of multiparametric ultrasound evaluation (MPUS) to improve thyroid nodule characterization due to the continuous technological developments including

Combined Endoscopic Ultrasonography and Endoscopic Retrograde Cholangiopancreatography in Patients With Malignant Distal Biliary Obstruction Is Associated With Reduced Time to Oncological Therapy Compared With ERCP and Sampling Alone.

OBJECTIVES: Standard endoscopic retrograde cholangiopancreatography (ERCP) sampling techniques for pancreaticobiliary malignancy have modest yields that could lead to delays in treatment. We evaluated whether combining EUS-guided tissue acquisition (EUS-TA) with ERCP improved time to first outpatient evaluation and treatment. MATERIALS AND METHODS: All patients without a prior pathological diagnosis who underwent index ERCP at Leeds Teaching Hospitals NHS Trust, United Kingdom, for malignant distal biliary obstruction from 2015 to 2020 were considered. RESULTS: A total of 292 patients were i

Conservative Composite Resin Restoration for Proximal Caries - Two Case Reports.

The paradigms of contemporary caries management have shifted to minimal intervention dentistry. Conservative restorative procedures are recommended to replace the complete removal of all carious tissues in the management of dental caries. This article reports two clinical cases of conservative restorations of proximal caries. Different conservative cavity designs and restorative techniques were performed in the two cases. Proximal caries in posterior teeth were prepared using the box-only preparation technique or the proximal tunnel preparation technique. The cavities were restored directly wi
