Pathogenicity of ureaplasmas for animals and man.

Since their original isolation from the genital tract of man, ureaplasmas, previously termed T-strain mycoplasmas, have been isolated from a variety of animal species. Under experimental conditions they have been shown to cause mastitis in cattle, goats and mice, and observations made on naturally-occurring bovine pneumonia, as well as the results of experimental inoculation, suggest that ureaplasmas are responsible for a portion of bovine cuffing pneumonia. Ureaplasmas have been isolated from the genital tract of a wide variety of animals and have the potential for causing disease in this ana

Fate of indeterminate lesions detected on noncontrast computed tomography scan for suspected urolithiasis: a retrospective cohort study with a minimum follow-up of 15 months.

OBJECTIVE: To investigate the fate of indeterminate lesions incidentally found on noncontrast computed tomography (NCCT) for suspected urolithiasis. METHODS: A retrospective review of 404 consecutive cases of suspected urolithiasis was undertaken between May 2010 and April 2011. Data were collected for patient demographics, presence of calculus disease, and additional urologic or nonurologic pathologies and their clinical relevance. The indeterminate or suspicious lesions were followed up and the data were reviewed in September 2012. RESULTS: In total, 404 patients underwent NCCT for renal c

How to manage total avulsion of the ureter from both ends: our experience and literature review.

OBJECTIVES: To evaluate the treatment alternatives of total avulsion of the ureter from both ends including ureteropelvic junction (UPJ) and ureterovesical junction (UVJ). METHODS: Total ureteral avulsion on both ends of the ureter was examined in 4 cases performing ureteroscopy. In two male patients of the four cases, avulsion was noticed intraoperatively and ureteral re-anastomosis at UPJ and re-implantation at UVJ were performed immediately. Boari flap was performed for one female patient immediately and for the other female patient who was referred from another hospital after the ureteros

Bladder stones in Afghan children.

OBJECTIVES: Military surgeons have been providing humanitarian care in Afghanistan since 2002. There are scant published reports on the details of that care. We report here the experience of deployed U.S. Army general surgeons in the management of an endemic problem, bladder stones in Afghan children. METHODS: A retrospective review was performed of pediatric patients presenting to an International Security Assistance Force humanitarian clinic over a 12-month period from October 2010 to November 2011. Symptoms at presentation, diagnostic modalities, and treatment provided were analyzed. The g

Robot-assisted simple prostatectomy (RASP): does it make sense?

UNLABELLED: Study Type - Therapy (case series) Level of Evidence 4. What's known on the subject? and What does the study add? The open simple prostatectomy (OSP) is the 'gold standard' for high-volume prostate adenomas. It shows very good functional results despite its invasiveness. Minimally invasive approaches, e.g. laparoscopy or holmium laser enucleation of the prostate, have been 'tested' but none have proved a substitute for the OSP. The robot-assisted approach provides optimal functional results and is easy to perform for experienced robotic surgeons. Extending the indication of robotic

Female pseudohermaphroditism associated with cloacal anomalies: faulty differentiation in the caudal developmental field.

A 1(1/2)-year-old girl with a urogenital sinus defect associated with an enlarged phalluslike clitoris is described. The child had an anteriorly located anus, a single kidney with grade III vesicoureteric reflux, a 3.5-cm-long common urethrovaginal channel, a large vesical calculus, and a hamartoma in the pubic region. After a preliminary colostomy, definitive surgery consisted of urethral reconstruction, vaginal reconstruction using a colonic graft, and posterior relocation of the anus. The discussion centers on a review of the previously reported cases of female pseudohermaphroditism associa

[Anesthetic management of a patient with chronic high spinal cord injury (second report)].

We administered anesthesia twice to a 43-year-old male patient with complete sensory and motor disturbance below the upper thoracic nerves due to chronic high spinal cord injury. The first operation was the gluteus maximus musculocutaneous flap for closure of a sacral decubitus ulcer under general anesthesia. The second was the transurethral lithotripsy for the bladder stone under spinal anesthesia. Severe hypertension occurred probably due to autonomic hyperreflexia (AH) during both operations. Therefore, we must be careful in anesthetic management of patients with chronic high spinal cord in

[Patient-controlled sedation using propofol for a patient with von Gierke disease].

Patient-controlled sedation (PCS) using propofol under spinal anesthesia in transurethral lithotripsy was carried out in a 44 year old patient with von Gierke disease accompanied with liver dysfunction, chronic renal failure, hypoglycemia and metabolic acidosis. After administering spinal anesthesia PCS was started (0.2 mg.kg-1 intravenous bolus dose of propofol; infusion at 2 mg.kg-1.h-1; a three-minute lockout time interval following an initial doses of 0.4 mg.kg-1). PCS with propofol, throughout the operation, brought about adequate sedation level for this patient with 2 or 3 on Wilson's se

Personalizing BPH Management: Bladder Stone Removal Alone vs. With Concomitant Laser Enucleation-A Multicenter Perspective With Patient Reported Outcomes and Decisional Regret Analysis.

INTRODUCTION: The necessity of routinely combining bladder stone removal with surgical treatment for BPH remains debated. We compared the efficacy and safety of bladder stone removal with and without concomitant outlet surgery in patients with BPH. METHODS: We conducted a multicenter ambispective cohort study comparing bladder stone removal alone vs. removal with concomitant holmium laser enucleation of the prostate (HoLEP). BPH patients with bladder stones > 1 cm were included. Exclusion criteria ruled out alternative causes of bladder stones. The "stone removal-only" group (n = 63) underwen

Spontaneous appendico-renal fistula: clinical presentation and management of a previously unreported entity.

Entero-urinary fistulas are uncommon in urological practice and may have widely varying aetiologies ranging from benign to malignant or iatrogenic in nature. All permutations of entero-urinary fistulas have been reported in the literature except an appendico-renal fistula. Here, we present one such case, presenting with urinary tract infections and perineal urethrocutaneous fistulae. He was ultimately diagnosed to have a spontaneous appendico-renal fistula as underlying pathology behind the symptoms.

Obstructive uropathy and severe acute kidney injury from renal calculi due to adenine phosphoribosyltransferase deficiency.

BACKGROUND: Adenine phosphoribosyltransferase (APRT) deficiency is an uncommon genetic cause of chronic kidney disease due to crystalline nephropathy. METHODS: A case of a Chinese boy with APRT deficiency presenting with severe acute kidney injury secondary to obstructive uropathy from multiple renal calculi was reviewed. RESULTS: The patient underwent staged removal of the calculi. Infrared spectrometry of the renal calculi showed 2,8-dihydroxyadenine. APRT deficiency was confirmed with abolished APRT enzyme activity in red blood cells. He was started on allopurinol and low purine diet with

Epidemiology of bladder stone of children: precipitating events.

Urological surgery evolved from the ancient practice of removing primary bladder stones from young boys. Bladder stones, once ubiquitous, long ago disappeared from the developed world while pockets of disease still exist in developing countries. Two epidemiological studies identified as precipitating events of bladder stone formation the practice of substitutive carbohydrate feedings to newborns. In Southeast Asia, masticated rice is fed to newborns in stone-endemic villages while in England, during the eighteenth and nineteenth centuries pap or panada was used to hand-feed infants when breast

Pneumatic ureterolithotripsy in paediatric and adolescent patients: a ten-year experience at the Hospital Universiti Sains Malaysia.

INTRODUCTION: Two to three percent of stone disease cases occur in the paediatric age group. It is common in some parts of the world, such as in Turkey, India and Thailand. More than 50 percent of stones in children are still managed through open surgery. Ureteroscopic intervention for children remains a challenging treatment option. However, in contemporary urology practice, this mode of intervention is becoming more common. In this retrospective study, we reviewed our experiences with ureteroscopy and pneumatic lithotripsy in the paediatric and adolescent age group. METHODS: A total of 13 p

Urolith masquerading as severe acute radiation toxicity: case report.

INTRODUCTION: Postoperative radiotherapy after radical prostatectomy has been shown to improve numerous outcomes for selected patients in mature randomized controlled trials. During treatment patients usually experience mild to moderate urinary symptoms, lower gastrointestinal symptoms and tiredness. We describe an unusual case of a patient experiencing severe, culture negative urinary symptoms early in a course of salvage radiotherapy. MATERIALS AND METHODS: A patient's history was retrospectively derived from a single institution. RESULTS: A total radiation dosage of 66 Gy in 33 fractions

[Magnetic resonance urography: possibilities and prospects].

A new technique of urinary tracts visualization--magnetoresonance urography (MRU)--for a year (2000) was applied in examination of 25 patients aged 17 to 63 years with ureteral concrements (n = 11), ureteropelvic stenosis (n = 10), ureteral stenosis (n = 2), urinary bladder tumor (n = 1), Ormond's disease (n = 1). MRU provides the same information about the obstruction and dilatation of the urinary tracts as excretory urography (EU). In cases of EU contraindication (allergy, renal failure) or lack of contrast substance excretion, MRU allows to avoid additional invasive diagnostic procedures. I

Reliability and validity of the General Health Questionnaire (GHQ-12) among urological patients: a Malaysian study.

This present study was undertaken to validate the English version of the General Health Questionnaire (GHQ-12) in urological patients. Validity and reliability were studied in patients with lower urinary tract symptoms (LUTS) and patients without LUTS. Reliability was evaluated using the test-retest method and internal consistency was assessed using Cronbach's alpha. Sensitivity to change was expressed as the effect size in the pre-intervention versus post-intervention score in additional patients with benign prostatic hyperplasia (BPH) who underwent transurethral resection of the prostate (TU

[The use of the alpha 1-adrenoblocker doxazosin in the pharmacotherapy of disorders of urine outflow of spastic origin].

Our objective in this study was summing up of results of clinical studies on effectiveness of a selective alpha-adrenoblocker doxazosine in the pharmacotherapy of reflex and spastic disorders of outflow of urine. Doxazosine ("Cardura", Pfizer Inc.) was prescribed per os once per day in the morning after meal in a dose of 2-4 mg. The drug can be used not only as a hypotensive agent but also in conservative treatment of patients with prostate hyperplasia. It was found to be very effective in attempts to bring the disordered outflow of urine back to normal in patients presenting with calculi in t

Inhibition of calcium oxalate crystal growth in vitro by uropontin: another member of the aspartic acid-rich protein superfamily.

The majority of human urinary stones are primarily composed of calcium salts. Although normal urine is frequently supersaturated with respect to calcium oxalate, most humans do not form stones. Inhibitors are among the multiple factors that may influence the complex process of urinary stone formation. We have isolated an inhibitor of calcium oxalate crystal growth from human urine by monoclonal antibody immunoaffinity chromatography. The N-terminal amino acid sequence and acidic amino acid content of this aspartic acid-rich protein, uropontin, are similar to those of other pontin proteins from
