LYMFASIM, a microsimulation model for transmission and control of lymphatic filariasis, was used to simulate the effects of mass treatment, in order to estimate the number of treatment rounds necessary to achieve elimination. Simulations were performed for a community that represented Pondicherry, India, and that had an average precontrol microfilariae (MF) prevalence of 8.5%. When ivermectin was used, 8 yearly treatment rounds with 65% population coverage gave a 99% probability of elimination. The number of treatment rounds necessary to achieve elimination depended to a large extent on covera

Citing earlier advances in the treatment of lymphatic filariasis [particularly the effectiveness of single-dose diethylcarbamazine (DEC) in reducing microfilaraemia and its enhanced effectiveness when co-administered with single-dose ivermectin], Eric Ottesen, Mahroof Ismail and John Horton consider recent studies on the antifilarial activity of albendazole that have led to the current recommendations for its use in single-dose regimens in conjunction with either DEC or ivermectin for large-scale control/elimination programmes. Furthermore, the potential of albendazole as a macrofilaricide for

In India, lymphatic filariasis persists as a major cause of clinical morbidity and as an impediment to socio-economic development. The direct costs incurred for the treatment of adeno-lymphangitis (ADL) episodes and the consequent indirect costs due to loss of income were determined from selected agricultural labour-intensive rural endemic pockets in Tamil Nadu. Information on the occurrence of ADL, its frequency and duration were collected using semi-structured questionnaire from randomly selected patients afflicted with chronic manifestations of bancroftian filariasis. Direct (treatment) cos

OBJECTIVE: To study the regular pattern of growth and declination or the transmission potential of filariasis after the disease was basically (the microfilarial rate was lower than 1%) in Henan Province in 1987.
METHODS: According to the distribution of filaria species and original microfilarial rate, in 7 surveillance sites in 7 counties (cities) the etiology and mosquito vector surveys were carried out continuously during 1988-1995 and no control measurement for pathogen was taken.
RESULTS: Ten residual microfilaremias became negative gradually in the first 6 years and no new microfilaremi

Adverse reactions following treatment of onchocerciasis and bancroftian filariasis are common and frequently severe. They are generally caused not by direct drug toxicity but by host inflammatory responses to dying microfilariae. To define the responsible mechanism, serial blood levels of interleukin-6 (IL-6) and tumor necrosis factor (TNF) were studied in 15 microfilaria-positive patients (10 with bancroftian filariasis, 5 with onchocerciasis) and 4 microfilaria-negative persons after diethylcarbamazine treatment. Elevations in IL-6 correlated with the occurrence and severity of clinical symp

A resurvey conducted 8 years later in the Union Territory of Dadra & Nagar Haveli revealed the persistence of filariasis amongst its residents. Though no control measures have been in operation since the detection of filariasis in Dadra and Nagar Haveli in 1982, the parasitological indices, microfilaria (mf) and disease rates as well as mf count have remained static. The entomological studies revealed five times increase in density of Culex quinquefasciatus both in Silvassa and Naroli towns.

A differential equation model for bancroftian filariasis transmission dynamics is developed. Fluctuations of the mosquito vector population are related to environmental variables such as climate and vector control operations. Prevalence in the mosquito population and mean worm burden in the human population are considered. The behaviour of the model is compared with epidemiologic and entomological data from Pondicherry (South India). The model enables one to make predictions on the effect of future control operations.

Localized adverse events following mass drug administration for lymphatic filariasis elimination are often selflimiting. However, they are of significant concern to the participants and may affect community drug compliance if not managed appropriately. Here, we report a rather uncommon adverse event, the 'string sign' in a south Indian woman with bancroftian filarial infection treated with the triple drug regimen (IDA: ivermectin, diethylcarbamazine and albendazole). String sign is the manifestation of acute lymphangitis as a cordlike subcutaneous swelling due to the swollen and inflamed lymph

After detection of 2 clinical lymphatic filariasis (LF) cases in a postelimination context in 2023 on the island of Futuna (Wallis and Futuna archipelago), the Wallis and Futuna Health Agency conducted a LF prevalence survey in Futuna in May 2024. This cross-sectional study, carried out among schoolchildren <18 years of age, identified 5 children with antigenemia, indicating an estimated antigenemia prevalence in Futuna children nearing 2%. The study also confirmed a spatial cluster of cases in the village of Taoa, where the child antigenemia prevalence reached 7.5% (95% CI 2.1%-18.2%), and de

BACKGROUND: American Samoa successfully completed seven rounds of mass drug administration (MDA) for lymphatic filariasis (LF) from 2000-2006. The territory passed the school-based transmission assessment surveys in 2011 and 2015 but failed in 2016. One of the key challenges after the implementation of MDA is the identification of any residual hotspots of transmission.
METHOD: Based on data collected in a 2016 community survey in persons aged ≥8 years, Bayesian geostatistical models were developed for LF antigen (Ag), and Wb123, Bm14, Bm33 antibodies (Abs) to predict spatial variation in infe

Colorimetric NINA-LAMP assays targeting Brugia Hha I repeat, Onchocerca volvulus GST1a and Wuchereria bancrofti LDR each exhibit species-specificity and are also highly sensitive, detecting DNA equivalent to 1/10-1/5000th of one microfilaria. BACKGROUND: Annual mass treatment with ivermectin and albendazole is used to treat lymphatic filariasis in many African countries, including Tanzania. In areas where both diseases occur, it is unclear whether HIV co-infection reduces treatment success.
METHODOLOGY: In a general population study in Southwest Tanzania, individuals were tested for HIV and circulating filarial antigen, an indicator of Wuchereria bancrofti adult worm burden, before the first and after 2 consecutive rounds of anti-filarial mass drug administration.
PRINCIPLE FINDINGS: Testing of 2104 individuals aged 0-94 years be…
