The effect of treatment on the age-antibody relationship in children infected with Schistosoma mansoni and Schistosoma haematobium.

The effect of praziquantel treatment on the age-antibody relationship was studied in 174 children aged between 6 and 17 years from a schistosome endemic area in Zimbabwe. The children were co-infected with Schistosoma mansoni and S. haematobium with infection prevalences of 74% and 53% respectively. Antibody levels for the isotypes IgA, IgE, IgM, IgG1, IgG2, IgG3 and IgG4, directed against soluble egg antigen were measured using an indirect ELISA assay. Treatment resulted in a significant increase in levels of IgG2 and IgG3 while levels of IgA decreased significantly. In untreated children the…

Multiple imputation for non-response when estimating HIV prevalence using survey data.

BACKGROUND: Missing data are a common feature in many areas of research especially those involving survey data in biological, health and soc … es for the missing observations. In particular national and subgroup estimates of HIV prevalence in Zimbabwe were computed using multiply imputed data sets from the 2010-11 Zimbabwe Demographic and Health Surveys (2010-11 ZDHS) data. A survey logistic regression model for HIV prevalence and demographic and socio-economic variables was used as the substanti …

Poverty and human immunodeficiency virus in children: a view from the Western Cape, South Africa.

Sub-Saharan Africa is the region affected worst by human immunodeficiency virus (HIV), with the most southern countries, including Botswana, Zimbabwe, Swaziland, and South Africa, carrying the highest disease burden. This geographic distribution represents a complex interaction among virological, political, social, cultural, and economic forces. In South Africa the HIV epidemic is seemingly unchecked, with 18% of the adult population infected. Although South Africa is a mid-developed country, there is a large chasm between the wealthy and the poor, with many living in moderate to extreme pover…

The geographical distribution of Kaposi's sarcoma and of lymphomas in Africa before the AIDS epidemic.

Estimated incidence rates are presented for three human immunodeficiency virus (HIV)-associated cancers [Kaposi's sarcoma (KS), Burkitt's ly … rom the era of AIDS indicates 20-fold increases in the occurrence of Kaposi's sarcoma in Uganda and Zimbabwe. The highest rates for BL were three to four times the rates for leukaemia at young ages in Western populations, but the general incidence of other NHL was no higher than in the West and very low rates were indicated for much of southern Africa.

On estimating the basic reproduction number for Schistosoma haematobium.

Existing estimates of the basic reproduction number, Ro, for human schistosomes are mostly in the range 1-4, implying that schistosomes should be relatively easy to eliminate from endemic areas, which is contrary to practical experience. An estimate of Ro for a site in Zimbabwe is obtained here using a mathematical model explicitly incorporating two features believed to be epidemiologically significant; age-dependent exposure and acquired immunity. Parameter estimates are, as far as possible, obtained independently, but the coefficients representing man-snail and snail-man transmission, as wel…

Caesarean section rates in an African country.

The use of caesarean section (C.S.) has been analysed in the 12 hospitals of the Midlands Province of Zimbabwe during a 2-year period. Maternal mortality rate, perinatal mortality rate, low birthweight rate, percentage of high risk pregnancy, C.S. rate and instrumental delivery rate have been extracted for each hospital. The rate of C.S. delivery varied between 2.2 and 16.8 per 100 deliveries but was not correlated with the number of high-risk pregnancies. Increased use of C.S. was not linked to better perinatal results. An important determinant of the C.S. rate appears to be the physician and…

Maintenance payments for child support in southern Africa: using law to promote family planning.

This article presents some of the legal, social, cultural, and practical constraints facing women who attempt to enforce their right to maintenance (child support) payments in southern Africa. It is based on research by the Women and Law in Southern Africa Trust, a network of women who research women's legal rights in six countries: Botswana, Lesotho, Mozambique, Swaziland, Zambia, and Zimbabwe. Statutes in all countries in the region provide that a man must support his legitimate and illegitimate children; there are, however, weaknesses in the laws on the books. The social and cultural constr…

Rabies-related viruses.

Five viruses related to rabies occur in Africa. Two of these, Obodhiang from Sudan and kotonkan from Nigeria, were found in insects and are only distantly related to rabies virus. The other three are antigenically more closely related to rabies. Mokola virus was isolated from shrews in Nigeria, Lagos bat virus from fruit bats in Nigeria, and Duvenhage virus from brain of a man bitten by a bat in South Africa. The public health significance of the rabies-related viruses was emphasized in Zimbabwe where in 1981 a rabies-related virus became epizootic in the dog and cat population. It is postulat…

Establishing a Pharmacy-Based Pre-Exposure Prophylaxis Program for Young Women Who Sell Sex: Protocol for a Randomized Controlled Trial.

BACKGROUND: Sex workers (SWs) are at risk of acquiring HIV infections. Despite this, SWs have low uptake of relevant preventive measures, such as HIV pre-exposure prophylaxis (PrEP). Community-based programs that make PrEP convenient and accessible have the potential to drive PrEP continuation and reduce HIV risk. OBJECTIVE: This study aims to describe the co-development and piloting of the Together Optimizing PrEP Access in Zimbabwe (TOPAZ) intervention-where SWs have the opportunity to access PrEP refills through pharmacies in Zimbabwe. METHODS: Using a participatory process, the TOPAZ int…

Characterising progression and regression patterns across the spectrum of tuberculosis: a multistate modelling approach.

BACKGROUND: The conceptualisation of tuberculosis has undergone a paradigm shift from binary states to a spectrum, resulting in the International Consensus for Early TB (ICE-TB) framework. This study aimed to use data from a prospective, observational cohort study and multistate modelling to address the lack of contemporary data to quantify movement between ICE-TB states. METHODS: ERASE-TB was a prospective, observational cohort study evaluating novel diagnostic tests for earlier detection of tuberculosis. Household contacts aged at least 10 years in Zimbabwe, Tanzania, and Mozambique were fo…

Factors affecting the propensity of tsetse flies to enter houses and attack humans inside: increased risk of sleeping sickness in warmer climates.

BACKGROUND: Sleeping sickness, or human African trypanosomiasis, is caused by two species of Trypanosoma brucei that are transmitted to humans by tsetse flies (Glossina spp.) when these insects take a bloodmeal. It is commonly assumed that humans must enter the normal woodland habitat of the flies to become infected, but recent studies found that tsetse frequently attack humans inside buildings. Factors affecting human/tsetse contact in buildings need identification. METHODOLOGY/PRINCIPAL FINDINGS: In Zimbabwe, tsetse were allowed access to a house via an open door. Those in the house at suns…
