[Addisonian crisis caused by metastatic lung cancer. Report of one case].

Addisonian crisis as a first manifestation of metastatic disease secondary to cancer is uncommon. We report a 63-year-old man with a history of one year of fatigue, weakness, weight loss and repeated symptomatic hypoglycemia. The cortisol stimulation test with ACTH confirmed primary adrenal insufficiency. While receiving adequate treatment with oral hydrocortisone, he presented an adrenal crisis that was treated properly. A CT scan of the lung demonstrated a nodule in the upper right lobe and bilateral adrenal tumors. The biopsy of the lesion revealed a lung adenocarcinoma. The staging with po

Can etomidate cause an Addisonian crisis?

The purpose of this study was to assess if it could have been possible to discover the side effects of etomidate by infusion in an earlier phase of its introduction on the market, using the routine parameters of everyday anesthesiological practice. The study was undertaken in twelve patients scheduled for major abdominal vascular surgery. The effect of etomidate-fentanyl and air anesthesia was compared with the effects of neurolept anesthesia. Arterial blood-pressure, central venous blood pressure, pulse rate, peripheral and central body temperature were recorded continuously. Blood samples we

Autoimmune disorders as predictive factors for oral lichen planus and malignancy risk: real-world data analysis.

OBJECTIVE: Oral lichen planus (OLP) is a T-cell-mediated mucosal disorder with malignant potential and ∼1% prevalence. Its associations with autoimmune diseases and oral squamous cell carcinoma (OSCC), especially among smokers, remain unclear. This study quantified OLP prevalence across 17 autoimmune disorders and evaluated OSCC risk in these cohorts. METHODS: Data from TriNetX research network were analyzed in a retrospective cohort of adults ≥18 years. Patients with one of 17 autoimmune diseases were identified via ICD-10 codes and compared to nonautoimmune controls. OLP prevalence and risk

PLAP -CAR T cells mediate high specific cytotoxicity against colon cancer cells.

UNLABELLED: Antiphospholipid syndrome (APS) is a systemic autoimmune disease characterized by the presence of antiphospholipid antibodies and venous and arterial thrombotic events, including obstetric complications. We describe the case of a 56-year-old female diagnosed with APS with triple antibody positivity and multiple disease-associated manifestations, namely recurrent purpuric lesions, adrenal insufficiency due to infarction, acalculous cholecystitis, and three spontaneous abortions. Her follow-up was marked by severe thrombotic and haemorrhagic events, notably splanchnic vein thrombosis

Prevalence of silent celiac disease in patients with autoimmune thyroiditis from Northern Sardinia.

Celiac disease (CD) is frequently associated with other autoimmune diseases such as Type 1 diabetes mellitus, autoimmune thyroiditis (AT), and Addison's disease. The frequency of these associations varies with the populations studied. We conducted this study to ascertain the prevalence of CD in patients with AT from Sardinia, an area with a very high prevalence of CD. To this aim, 297 consecutive patients with AT (as defined by elevated antithyroid antibody levels and a positive ultrasound scan) were studied. Immunoglobulin A and G-class antigliadin antibodies were assayed in serum; if either

[Differential diagnosis of space-occupying adrenal masses].

Computed tomography (CT) and magnetic resonance (MR) imaging are first line modalities in the evaluation of patients with adrenal gland masses, and have the potential to be very accurate for the localization of adrenal gland masses in patients with diseases associated with hyperfunctioning conditions of the adrenal gland. Both CT and MR imaging allow a specific diagnosis of acute adrenal hemorrhage, adrenal myelolipoma, and adrenal cysts. CT is also helpful in the assessment of patients with Addison's disease, particularly the subacute form secondary to granulomatous diseases. Quantitative eva

Susceptibility and resistance alleles of human leukocyte antigen (HLA) DQA1 and HLA DQB1 are shared in endocrine autoimmune disease.

Because particular human leukocyte antigen (HLA) DQ alleles are the major predisposing factors for type 1 diabetes mellitus (IDDM), we investigated whether they are shared by other endocrine autoimmune diseases. We, therefore, analyzed the HLA DQ genotypes of 171 patients with IDDM, 271 with Graves' disease (GD), 65 with Hashimoto's thyroiditis, 51 with postpartum thyroiditis, 53 with Addison's disease (AD), and 271 healthy controls. HLA DQA1 and DQB1 alleles were defined by polymerase chain reaction and sequence-specific oligonucleotide hybridization as well as by single strand conformational

Risk of invasive pneumococcal disease in people admitted to hospital with selected immune-mediated diseases: record linkage cohort analyses.

BACKGROUND: Invasive pneumococcal disease is a serious infection, and it is an important cause of morbidity and mortality in certain groups of 'at-risk' people. Those considered 'at-risk' in the UK include very young children, people aged 65 years and older and people with certain serious chronic diseases, asplenia or immunosuppression. There is little evidence about whether people with immune-mediated diseases are at increased risk of pneumococcal disease and therefore may benefit from pneumococcal vaccination. METHODS: Retrospective cohort studies, using linked hospital data, from the longs

[A Novel Pathogenic variant in NR0B1 gene associated with Congenital Adrenal Hypoplasia].

X-linked adrenal hypoplasia congenita is a rare cause of primary adrenal insufficiency. Mutations in the NR0B1 gene cause a loss of function in the DAX1 receptor, which activates genes involved in the development and function of the hypothalamic-pituitary-gonadal axis. Objective: To describe a case of adrenal hypoplasia congenita secondary to a mutation in the NR0B1 gene and identified the differential diagnoses of the pediatric patient with adrenal insufficiency and hypogonadotropic hypogonadism. Clinical Case: A 4-year-old male patient with no relevant history and from a rural area was admit

The coagulation system in endocrine disorders: a narrative review.

Endocrine disorders can influence the haemostatic balance. Abnormal coagulation test results have been observed in patients with abnormal hormone levels. Also unprovoked bleeding or thrombotic events have been associated with endocrine disease. The aim of the present review is to summarise the available evidence on the influence of common endocrine disorders on the coagulation system, and their possible clinical implications. We focus on thyroid dysfunction, hyper- and hypocortisolism and growth hormone disturbances, while other endocrine disorders are only briefly discussed. In the published

Endocrine Autoimmunity in Pregnancy.

Human gestation leads to a number of physiological alterations which peak at the development of placentta known for, among many other functions, being a transient but highly potent endocrine organ. Hormonal activity of placenta is marked by its ability to continuously produce and secrete high levels of progesterone. Progesterone guards the well-being of the fetoplacental unit throughout the gestation and one of the proposed mechanisms of this principle involves the development of local and systemic immune tolerance mainly due to impediment of CD4+ lymphocyte activation. However, though these a

Acute reversible cardiomyopathy with cardiogenic shock in a patient with Addisonian crisis: a case report.

A 42 year-old female, admitted to the ICU, with Addisonian crisis developed acute cardiopulmonary failure after hydrocortisone therapy was initiated. An echocardiogram showed severe reduction in the left-ventricular ejection fraction. Additionally, profound ECG abnormalities with diffuse ST-elevation and decreased QRS-amplitudes occurred, whereas Troponin-I was only moderately increased. Chest X-ray displayed bilateral pulmonary edema. The patient's condition culminated in respiratory failure and cardiogenic shock requiring catecholamines and ventilatory support. After a week, she had recovere
