Leber hereditary optic neuropathy is a primary mitochondrial disease characterized by acute visual loss due to the degeneration of retinal ganglion cells. In this study, we describe a patient carrying a rare missense heteroplasmic variant in MT-ND1, NC_012920.1:m.4135T>C (p.Tyr277His) manifesting with a typical bilateral painless decrease of the visual function, triggered by physical exercise or higher ambient temperature. Functional studies in muscle and fibroblasts show that amino acid substitution Tyr277 with His leads to only a negligibly decreased level of respiratory chain complex I (CI)

OBJECTIVES: To determine the prevalence of cataract and pseudophakia/aphakia in the United States and to project the expected change in these prevalence figures by 2020.
METHODS: Summary prevalence estimates of cataract and of pseudophakia/aphakia were prepared separately for black, white, and Hispanic persons (for whom only cataract surgery data were available) in 5-year age intervals starting at 40 years for women and men. The estimates were based on a standardized definition of various types of cataract: cortical, greater than 25% of the lens involved; posterior subcapsular, present accord

OBJECTIVE: This review was conducted to determine the safety and efficacy of open-loop anterior chamber, scleral-sutured posterior chamber, and iris-sutured posterior chamber intraocular lenses (IOLs) in eyes with inadequate capsular support for posterior chamber implantation in the capsular bag or ciliary sulcus. It also attempted to determine whether there is a preferred IOL or fixation site of choice in eyes with inadequate capsular support.
METHODS: A literature search conducted for the years 1980 to 2001 yielded 189 citations related to IOL implantation in the absence of capsular support

The implantation of an artificial iris diaphragm is described in a case of aniridia and traumatic aphakia with a relapsed, tractional retinal detachment. The addition of heparin and dexamethasone to the infusion liquid is useful to reduce the fibrinous exudation which seems to be the main cause of diaphragm obliteration.

Glaucoma has been recognized as an infrequent but serious complication following surgery for congenital and acquired cataracts in childhood. Little is known concerning the influence of factors on the risk of glaucoma such as age at surgery, type of cataract, associated ocular abnormalities, and type of surgery. We reviewed records of 240 eyes of 155 patients on whom cataract surgery had been performed between January 1965 and July 1990 at Children's Hospital, Boston. After excluding those patients who had been followed up less than 5 years after surgery, and those who had had surgery after the

Fifty-two patients under the age of 16 years (68 eyes) received epikeratophakia grafts for the correction of aphakia. In 27 eyes, epikeratophakia was a primary procedure combined with lensectomy. Fifteen children underwent bilateral surgery. Overall, the success rate was 91%, and with repeated surgery it was 94%. The average change in refractive error was 15.9 diopters (D) and the average spectacle overcorrection was +0.3 D. Sixty-one percent (35/57) of the eyes were within 1 D of emmetropia and 97% (55/57) were within 3 D of emmetropia. Overall, the refraction was stable in the follow-up betw

We used argon laser trabeculoplasty to treat medically uncontrolled glaucoma after penetrating keratoplasty in ten eyes that were aphakic or pseudophakic. These patients were examined over an average of 22.8 months (range, 12 to 37 months) after treatment. We found an average decrease in intraocular pressure of 9.1 mm Hg (range, +8 to -19), from an average of 30.6 mm Hg before treatment to an average of 21.5 mm Hg after treatment. Eight of ten eyes had a reduction of greater than 5 mm Hg, and six of ten eyes had intraocular pressure of less than 20 mm Hg. Visual function remained stable, and c

After reviewing the definition, incidence and main etiologic factors of cystoid macular edema (CME) the authors analyze the structural and physiologic characteristics of the macular area, including the blood-retinal barriers, and the basic concepts of retinal edema. Personal observations on three different groups of diseases associated with CME, retinitis pigmentosa, pars planitis and aphakia, are presented. CME evaluation was made by slit-lamp microscopy, fluorescein angiography and vitreous fluorophotometry. In the situations examined, the main factors involved in CME formation appear to be

Neonatal aphakia is associated with retardation of the axial elongation of the neonatal eye. In contrast, form deprivation increases axial elongation, an effect that has been associated with decreased retinal dopamine metabolism. The present investigation was conducted to test the hypothesis that neonatal aphakia induces an effect on the levels of retinal dopamine opposite to form deprivation. Lensectomy and vitrectomy were performed on the right eyes of rhesus monkeys at approximately 1 week of age; their left eyes were unmanipulated. Axial length was measured by A-scan ultrasonography.
