Determinants of heart rate variability during deep breathing: basic findings and clinical applications.

The measurement of heart rate variation during forced breathing (HRDB) is a well-known clinical test of parasympathetic function. It is known that normal values of HRDB are strongly dependent on age. However, little is known about other physiological factors that may lead to reduced HRDB values that may mimic parasympathetic failure. Thirty-two normal subjects (age 56.7 +/- 12.4 years) and 32 neurological patients with pathological autonomic test findings (age 57.9 +/- 10.2) were studied. Oscillations in heart rate and in mean arterial blood pressure were recorded in the supine position during

Abnormal cardiovascular responses to postural changes and pharmacologic agents in a case of Shy-Drager syndrome.

A 64-year-old carpenter had an unsteady gait, severe dizziness, nocturia, and a loss of erection for more than 4 years. The neurological manifestations consisted of a wide-based ataxic gait, bilateral dysmetria with intentional tremor, staccato speech, rigidity, bradykinesia, and an iris-thinning. There was reproducible orthostatic hypotension. A sweat test revealed severe anhidrosis. Nicotine and methylbenzene sensitivity was absent, whereas norepinephrine infusion test showed a significant elevation of blood pressure. The resting plasma norepinephrine level on recumbency was low and a subnor

Association of pupillary parasympathetic hyperreflexia and systemic inflammation in patients with systemic lupus erythematosus.

In chronic inflammatory diseases, cytokines stimulate the hypothalamus pituitary adrenal axis and the hypothalamus autonomic nervous system (HANS) axis. The present study was performed to find autonomic nervous function parameters in patients with systemic lupus erythematosus (SLE) which are suitable to demonstrate the activation of the HANS axis during systemic inflammation. Thirty-four patients with SLE (age 35.3 +/- 1.9 yr) were investigated by seven standardized autonomic nervous function tests. The SLEDAI and laboratory parameters of systemic inflammation were assessed by standard techniq…

[Usefulness of heart rate variability for evaluating cardiac autonomic nervous function].

To investigate the effect of gastric ulceration on cardiac autonomic nervous function, 24-hour ambulatory electrocardiograms were recorded in patients with artificial gastric ulcers caused by endoscopic treatment of gastric tumors. Their cardiac autonomic nervous activity was assessed by both time- and frequency-domain analyses of heart rate variability obtained from electrocardiogram recordings. The results were compared with those from patients who had undergone endoscopic treatment of esophageal varices or colorectal tumors, or gastrointestinal endoscopy alone. Parameters of time-domain ana

[Neurogenic autonomic dysfunction in adults].

Neurogenic autonomic dysfunction (NAD) is underdiagnosed, and it is likely in patients, who have orthostatic hypotension and symptoms from multiple organ systems as well as abnormal results from a neurological examination. A clinical and neurophysiological examination of the autonomic nervous system combined with a standardised paraclinical evaluation should be performed. NAD may be present in neurodegenerative disorders, vitamin deficiency, toxicity, infection, and in paraneoplastic, metabolic, hereditary and immune-mediated conditions.

[Heart rate variability].

Heart rate variability (HRV) has become a useful parameter for the quantification of autonomic nervous function. HRV has been quantified, either by time domain or frequency domain analysis. Time domain measures, such as CVRR and RR50, are easy to calculate but they only provide information related to parasympathetic activity. The spectral analyses, on the other hand, give us information concerning 3 oscillatory components. The high frequency band (the frequency between 0.15-0.5 Hz) is known as the respiratory sinus arrhythmia (RSA), and the middle frequency band (0.88-0.15 Hz) is attributed to

Is cardiovascular disease risk in transgender, gender-diverse, and non-binary adults associated with autonomic imbalance?

Blood pressure is a key indicator of cardiovascular health with chronically high levels increasing the risk of cardiovascular diseases (CVD) such as heart attack and stroke. Emerging evidence shows that transgender, gender-diverse, and non-binary (TGD) adults tend to have higher blood pressure than age-matched cisgender adults, corresponding to an increased CVD risk in this population. Yet, the mechanisms underlying elevated blood pressure in TGD adults remain unclear, posing challenges to TGD-affirming healthcare. Given the autonomic nervous system's role in CVD - wherein reduced parasympathe…

Autonomic imbalance in cardiovascular disease: molecular mechanisms and emerging therapeutics.

Autonomic imbalance is a key driver of cardiovascular disease progression, arising from disrupted interactions between sympathetic and parasympathetic signaling. This review explores the molecular mechanisms underpinning autonomic dysfunction, emphasizing the roles of β-adrenergic receptor (βAR) signaling, cyclic AMP (cAMP) compartmentation, and cholinergic regulation. Dysregulated cAMP nanodomain signaling, βAR desensitization, impaired vagal tone, and maladaptive autonomic nerve remodeling collectively promote structural, electrophysiological, and functional deterioration. Advances in high-r

[Autoimmune Autonomic Ganglionopathy and Acute Autonomic Sensory Neuropathy].

Autoimmune autonomic ganglionopathy (AAG) and acute autonomic sensory neuropathy (AASN) are immune-mediated neuropathies that affect the autonomic and/or dorsal root ganglia. Autoantibodies against the nicotinic ganglionic acetylcholine receptor (gAChR) detected in the sera of patients with AAG play a key role in the pathogenesis of this condition. Notably, gAChR antibodies are not detected in the sera of patients with AASN. Currently, AAG and AASN are not considered to be on the same spectrum with regard to disease concept based on clinical symptoms and laboratory findings. However, extra-aut

Reliability of ultra-short indices for autonomic dysfunction in dyslipidemia.

Dyslipidemia is associated with autonomic nervous system (ANS) dysfunction. Heart rate variability (HRV) is a powerful tool for evaluating the ANS and for cardiovascular risk stratification. Yet, the methodologies used are impractical in most clinical settings and therefore, are usually not applied. The current study aimed to evaluate the reliability of ultra-short HRV parameters, which are easily calculated from any standard ECG, as a practical method for ANS study, with a focus on patients with dyslipidemia. Fifty-nine volunteers with dyslipidemia underwent HRV study of parametric and power …

Autonomic testing: common techniques and clinical applications.

Laboratories able to test autonomic function are increasingly available and rely on batteries of well-accepted, noninvasive tests. Tests of parasympathetic cardiovagal, sympathetic vasoconstriction, and sudomotor (sweating) function are most commonly employed. Common examples include heart rate variability to various challenges, Valsalva maneuver, standing and tilt-table studies, and various sudomotor methods. New techniques and technical refinements continue to be described. Most studies rely on perturbations of complex systems and not direct assessment. Testing has helped to improve disease

Brainstem-type Lewy body disease presenting with progressive autonomic failure and lethargy.

The authors report an autopsy case characterized by progressive lethargy and autonomic failure with a distinctive pattern of occurrence of Lewy bodies. Autonomic dysfunction such as sleep apnea, orthostatic hypotension, dysuria, and hypohidrosis predominated with lethargy, whereas parkinsonism was not apparent. Numerous Lewy bodies were widely evident microscopically in brainstem nuclei and the intermediolateral cell columns of the spinal cord, as well as in the sympathetic ganglia, but were rare or absent in the cerebral cortex and other supratentorial structures. Marked neuronal loss was see

[Evaluation of autonomic nervous function by pupil dynamics recording].

Pupil is richly innervated by autonomic nerves. Therefore, its size or movement well reflects the autonomic nervous function. By analyzing dynamics of pupil movement for light stimulus followed by infrared video-pupillography, the abnormality of autonomic nervous system can be expressed in a quantitative manner. Clinical applications of pupillography and pharmacological examination to Horner syndrome, Adie syndrome, diabetes mellitus, pupillary involvement of Behçet disease, pupil of brain death and patients with chemical sensitivity have been described. Segmental spasm of iris in Horner and A…
