In systemic diseases, brain biopsy is an exceptional diagnostic procedure which must be restricted to those rare forms involving only the central nervous system, at least clinically, and when a treatable disease is suspected the treatment of which requires a precise histological diagnosis. According to the clinical presentation and radiological appearance, an open biopsy or a stereotactic biopsy under radiological control will be proposed. In this paper, will be reviewed the indications and neuropathological findings in systemic amyloidoses extending to the central nervous system, cerebral ang…

To determine if superior health at old age protects against cognitive impairment (CI) and Alzheimer's disease (AD), we prospectively studied 100 optimally healthy oldest-old (> or =85 years) individuals. Initially, subjects represented the top 3% of the oldest old for health. During 5.6 +/- 0.3 years of follow-up, 34 subjects developed CI, and 23 progressed to AD. By age 100, probability of CI and AD were.65 +/-.09 and.49 +/-.10. Median onset age was 97 years for CI and 100 for AD. Clearly, superior health at old age does not guarantee protection against cognitive decline. Lifetime risks were

Brain damage in the premature or full-term fetus or newborn infant encompasses multiple patterns of injury, many considered to be anomalous in origin. However, there is increasing evidence that such congenital lesions arise as a consequence of hypoxia or ischemia (reperfusion failure). Animal models have been helpful in elucidating the underlying cellular and molecular mechanisms responsible for perinatal brain damage. Ultimately, neuroprotective strategies will prevent or minimize the occurrence of these lesions and their consequent functional deficiencies.

The fatal acardiac syndrome is a rare complication of monochorionic twinning and is reported in 1 of 35,000 births. It is caused by arterioa … as an edematous acardiac twin without peripheral reduction defects and a nearly normally developed brain. An acardiac twin with a nearly normal external appearance and an almost normally developed brain, nourished by a surviving twin brother, has not previously been described in the literature.

BACKGROUND: Brain death (BD) is the irreversible loss of all functions of the brain and brainstem. Spontaneous and reflex movements of the limbs have been described in this condition. However, facial myokymia (FM) in BD has not been previously reported. The origin of that motor phenomenon in alive patients is still uncertain, since supranuclear, nuclear and peripheral mechanisms have been proposed.
OBJECTIVE: We describe the presence of FM in a patient who fulfilled the criteria for BD. A 40-year-old-man had right-sided weakness and impaired consciousness. After 14 h admission, he fulfilled t…

The German Childhood Cancer Registry (GCCR) was established in 1980. From 1980 to 1997 4,447 CNS tumours in children below 15 years of age have been reported to the registry. From 1988 to 1997 the average annual incidence was 2.5/100,000 children. This corresponds to 19.2% of the registered diseases. Compared with incidence rates reported from other developed countries one can estimate that there is about 25% underreporting of CNS tumours in the GCCR. This is in contrast to the relatively complete ascertainment of other childhood malignancies (above 95%). Based on 3012 incident cases from 1988

BACKGROUND: Brain tissue acidosis can result from ischemia when cerebral blood flow reduction reaches a critical value. The portable, battery-operated Khuri pH monitor has been used previously in a large number of patients undergoing cardiopulmonary bypass surgery to monitor the intramyocardial pH during aortic clamping. It was found to be easy to use, reliable, and a strong predictor of myocardial preservation. The goal of this study was to evaluate the applicability of this monitor in the measurement of brain pH.
METHODS: Fifteen New Zealand white rabbits underwent general endotracheal anes…

BACKGROUND: Whether the human brain is nothing but an advanced computer is a matter of inconclusive debate. This paper contributes to that debate.
METHOD: Critical reasoning based on evidence provided by the history of a woman who complained of amnesia after each of two separate acts of attempted suicide.
FINDINGS: A life-threatening tendency (suicidal impulses) may be countered by a functional imperfection (selective amnesia) or a feigned malfunction (malingering).
INTERPRETATION: Some aspects of brain function may depend on operations that no hitherto invented computer can duplicate.

This article is focused on the mechanisms underlying primarily ischaemic/reperfusion brain injury in both the term and premature infant. Although the mechanisms involved include similar initiating events, principally ischaemia-reperfusion, and similar final common pathways to cell death, particularly free radical-mediated events, there are certain unique maturational factors influencing the type and pattern of cellular injury. We will therefore initially describe the physiological and cellular/molecular mechanisms of brain injury in the term infant, followed by the mechanisms in the premature …

Future brain imaging studies of dyslexia should have a sufficient number of males and females to detect possible gender differences in the neurological underpinning of this disorder. Detailed knowledge about such differences may clarify our understanding of the structural and functional impairments which lead to the phonological deficits that characterize dyslexia. Functional brain imaging studies have shown that males and females exhibit different patterns of brain activation during phonological processing. Further differences between the brains of males and females have been suggested by stu…

BACKGROUND AND PURPOSE: Our purpose was to document the nature and progression of brain abnormalities in Salla disease, a lysosomal storage disorder, with MR imaging.
METHODS: Fifteen patients aged 1 month to 43 years underwent 26 brain MR examinations. In 10 examinations, signal intensity was measured and compared with that of healthy volunteers of comparable ages.
RESULTS: MR images of a 1-month-old asymptomatic child showed no pathology. In all other patients, abnormal signal intensity was found: on T2-weighted images, the cerebral white matter had a higher signal intensity than the gray …
