Protean neuroimaging presentations in an adult with Klebsiella pneumoniae infection.

PURPOSE: To report on the findings of a series of cranial computed tomography (CT) and magnetic resonance (MR) imaging studies of a 57-years … rotean neuroimage features of a woman with K. pneumoniae infection and the confirmation of multiple brain abscesses was made by follow-up neuroimaging studies. These sequential neuroimage findings in one patient are uncommon but deserved special clinical note by the first-line, primary-care physicians including neurologists.

Spectra of central nervous system melioidosis.

Burkholderia pseudomallei infection of the central nervous system (CNS) is rare with less than 50 cases reported over the last 30 years. The retrospective melioidosis study at University Malaya Medical Centre has documented three cases of CNS melioidosis out of more than 160 cases of melioidosis since 1978. There were two patients with brain abscess and one with spinal epidural abscess. The predisposing factors were: one patient was an aboriginal farmer and the other two were diabetic. Their age ranged from 17 to 45 years. Prominent neurological features were limb weakness, cranial nerve palsy…

Disseminated subcutaneous Nocardia farcinica abscesses in a nephrotic syndrome patient.

We describe an unusual case of disseminated subcutaneous abscesses caused by Nocardia farcinica in a 49-year-old man with nephrotic syndrome. He had received systemic corticosteroid therapy for 5 months. He developed a submandibular abscess associated with sialoadenitis on the right submaxillary gland. Magnetic resonance imaging revealed connection between the submandibular abscess and the right submaxillary gland. The subcutaneous abscess spread from the submandibular triangle to the left axillary region, the left upper arm, the left hypochondriac region, the left scapular region, the right e

Disseminated aspergillosis in a renal transplant patient: Diagnostic difficulties re-emphasized.

An asymptomatic and radiographically occult lung abscess was the primary focus of infection in this case of fatal disseminated aspergillosis in a renal transplant recipient. Extensive neurological evaluation in response to a change in personality failed to reveal a brain abscess, which was the cause of death. This case illustrates the variability in presentations of aspergillosis and the continuing difficulties in diagnosing this infection in immunosuppressed patients.

Suspected ruptured medulla oblongata abscess treated conservatively with a favorable outcome: A rare case report from Vietnam.

Brainstem abscess is a rare and serious condition that requires intensive medical treatment whenever a suggestive diagnosis is made. This report presents the conservative treatment of a middle-aged female patient with a suspected ruptured medulla oblongata abscess. The patient reported a 7-day history of headaches and right-sided weakness. Her neurological symptoms gradually worsened, resulting in right hemiplegia, urinary retention, and swallowing difficulties. Brain magnetic resonance imaging confirmed a severe abscess in the medulla oblongata extending to the upper cervical spinal cord. Aft

Cerebral aspergillosis and facial acneiform lesions following initiation of ibrutinib in a patient with chronic lymphocytic leukemia.

A case of a 67-year-old male with CLL, presented with prolonged pancytopenia after his first cycle of fludarabine, cyclophosphamide, and rituximab (FCR) chemotherapy. He was then treated with ibrutinib oral monotherapy. Shortly after ibrutinib treatment initiation, he developed a brain abscess and pulmonary disease as a part of an invasive aspergillosis. The patient improved after brain abscess drainage and the anti-fungal therapy voriconazole. Upon resuming ibrutinib four months after his hospitalization, he developed extensive acneiform facial lesions. This case is the first to report on the…

Case report: Meningitis and intracranial aneurysm caused by mixed infection of oral microflora dominated by anaerobes.

INTRODUCTION: Meningitis caused by oral anaerobic bacteria is rare, especially when complicated with an infected intracranial aneurysm. This paper has described an extremely rare case of bacterial meningitis caused by a mixed infection of oral microflora dominated by anaerobes, which developed cerebral infarcts, brain abscess, intracranial aneurysm, and severe hydrocephalus during treatment. CASE REPORT: We describe a 65-year-old male patient who was presented with fever and headache as the initial symptoms and then developed left ophthalmoplegia, right hemiplegia, and disturbance of consciou…

Pulmonary nocardiosis associated with cerebral abscess successfully treated by co-trimoxazole: a case report.

Nocardiosis is an acute or chronic infectious disease caused by the soil-borne filamentous bacteria belonging to the genus Nocardia. The organisms opportunistically infect both immunocompromised and immunocompetent individuals. The lungs are the primary site of infection and brain abscess is, by far, the most common complication following nocardial metastasis from pulmonary lesions. Although surgical intervention must always be considered in the treatment of nocardial brain abscess, it can obviously be cured by antibiotic therapy alone. This report describes a case infected by Nocardia cyriaci…

Streptococcus oralis cerebral abscess following monkey bite in a 2-month-old infant.

Although cerebral abscesses caused by animal bites have been reported, they are extremely rare in infants and have not been described following monkey bite. A 55-day-old male infant presented with a multi-loculated Streptococcus oralis cerebral abscess following a monkey bite on the scalp. There was a clinical response to antibiotic therapy and repeated surgical aspiration followed by a ventriculoperitoneal shunt. This is the first report of a patient with a brain abscess following a monkey bite.

Temporal Lobe Abscess Secondary to Chronic Otitis Media: Evaluation and Management in a Medical Complex Patient.

Chronic otitis media is a persistent middle ear infection caused by various well-described bacteria that, in rare cases, can lead to severe complications including intracranial infections such as brain abscesses and meningitis. We present a case of a 42-year-old female with a complicated medical history including chronic otitis media, who developed a brain abscess and meningitis following the spread of the infection. The patient initially presented with altered mental status and concerns for acute intracranial processes were confounded by her significant polysubstance use history. Further work…

Haemophilus parainfluenza-Associated Aortic Root Abscess and Septic Embolic Stroke Post-transcatheter Aortic Valve Replacement: A Case Study.

We present a rare instance of aortic root abscess and septic embolic stroke due to Haemophilus parainfluenza in a 75-year-old male patient who had undergone a recent transcatheter aortic valve replacement. The patient initially presented with generalized weakness and altered mental status. Blood cultures grew Haemophilus parainfluenza. Brain imaging showed multiple infarcts with some areas of hemorrhagic conversion. Echocardiography demonstrated an aortic root abscess as well as vegetation on the tricuspid valve. Surgical correction was deemed high risk; hence, management involved intravenous

Intra-spinal abscess due to Bacteroides fragilis and Klebsiella pneumoniae co-infection in a child with spinal dysraphism.

Bacteroides fragilis (B. fragilis) is an uncommon cause of spinal abscess. We present a case of an 18-month-old child, with spinal dysraphism-Spina bifida occulta, who developed intra-spinal abscess infection with B. fragilis and Klebsiella pneumoniae. Magnetic resonance imaging (MRI) of the brain and spine showed multiple abscesses extending through the presacral fistula into the spinal cord. Patient was surgically treated along with administration of antimicrobial agents (ceftriaxone and metronidazole), resulting in an excellent clinical outcome.

Reprogramming of pathogenic strategies in Fusobacterium nucleatum: a study based on an in vitro infection model.

BACKGROUND: Fusobacterium nucleatum is a Gram-negative obligate anaerobic bacterium with trans-organ pathogenicity. It serves as a crucial pathogen causing intracranial anaerobic infections and is closely associated with the occurrence of brain abscess. However, the mechanism by which Fusobacterium nucleatum crosses the blood-brain barrier and induces intracranial infection remains unclear. METHODS: In this study, Fusobacterium nucleatum clinical strain isolated from the brain abscess was identified and characterized. Its growth characteristics and biofilm-forming capacity were compared with …

Conglomerate mediastinal mass of a different etiology.

Streptococcus intermedius is a Gram-positive commensal of the oral cavity with tendency to cause brain and liver abscesses. Mediastinal involvement from S. intermedius is very rare. We present a case of 22-year-old male with newly detected mediastinal conglomerate nodal mass. On his way for bronchoscopy, he developed seizures and was found to have three brain lesions. The aspirate from brain biopsy grew S. intermedius whereas the aspirate from mediastinum showed only acute inflammation. Follow up imaging after antibiotic treatment showed interval resolution of brain abscesses and the mediastin…
