The development of a polypoid intrapulmonary bronchogenic cyst in the bronchial lumen.

Intrapulmonary bronchogenic cysts are rare in adults and most are outside the trachea and bronchi. There are reports of clear link with the trachea, but the cyst itself occurs outside the trachea. Thus, bronchoscopy will not reveal the cause, which often leads to a diagnosis by surgical resection. We herein report an extremely rare case of an intrapulmonary bronchogenic cyst that was located entirely within the left main bronchial lumen. Bronchoscopy revealed a shiny and smooth surface mass with abundant blood vessels in the lumen that blocked the left main bronchus. The patient was successful…

Sleeve resection for bronchial carcinoid tumour in two children under six years old.

BACKGROUND: Paediatric tracheobronchial tumours are very rare, and pneumonectomy and lobectomy procedures are rarely indicated due to their surgical difficulties and high sequelae. Bronchoplastic techniques preserving lung parenchyma allow the resection and reconstruction of the main bronchi and carina. CASE PRESENTATION: Here, we present a 6-year-old boy suffering from a carcinoid tumour of the right main bronchus which was successfully managed with a right upper sleeve lobectomy and a 4-year-old girl with an endobronchial carcinoid tumour narrowing the left main bronchus that received a sle…

Automatic lung fields segmentation in CT scans using morphological operation and anatomical information.

Segmenting lung fields from CT (Computed Tomography) scans is an important task for the analysis, diagnosis and treatment of pulmonary disea … bar fissure, thin junction between the left/right lung fields, indentation of the blood vessels and bronchi-walls. This experiment is performed by employing the thoracic CT scans datasets, and it is proved to be an effective method.

Airway metastasis of small cell lung carcinoma: A rare presentation.

Airway metastasis from primary lung carcinoma is rare and typically associated with non-small cell histology. While small cell lung cancer is a particularly aggressive form of cancer, few cases of endotracheal or endobronchial metastasis have been reported. Airway involvement can go undetected because of the spread along the perilymphatic drainage system with mostly submucosal involvement causing significant airway compromise before onset of symptoms. We present a patient with recurrent small cell lung cancer, presenting with wheezing, cough, and dyspnea as a result of metastasis to the trache…

Coincidence of congenital agenesis of left lung and common atrium: a very rare case.

Pulmonary agenesis is characterized by undeveloped pulmonary vessels, bronchi as well as lung parenchyma and is a rare congenital anomaly with unknown etiology. It is usually diagnosed during childhood. Nearly, one third of these patients have concomitant congenital heart diseases. While more than 50% of these patients die before the age of 5 years, some other patients may be asymptomatic throughout their life. On the other hand, common atrium, another rare congenital anomaly, is characterized by complete absence of the atrial septum and is usually accompanied by atrioventricular valve malform…

[Case report: respiratory infection due to Alcaligenes xylosoxidans in a patient with Mounier-Kuhn syndrome].

Mounier-Kuhn syndrome is a rare entity characterized by abnormal dilatation of the trachea and main bronchi (tracheobronchomegaly). Alcaligenes xylosoxidans is a non fermenting gram-negative pathogen common in extra-and intra-hospital environment, which may be related to immunosuppression states. We describe the case of a 75 years old male, ex-smoker with moderate functional obstruction, chronic respiratory failure and chronic colonization by Pseudomonas aeuriginosa. He had an infectious exacerbation of his disease, reason that previously required several hospital admissions. The patient was t…

A case report of extralobar pulmonary sequestration in a dog.

Pulmonary sequestration is a rare congenital anomaly in the veterinary literature. This malformation is characterized by a cystic mass of non-functioning primitive lung tissue that does not communicate with the tracheobronchial tree or with the pulmonary arteries. This article describes gross and histopathological characteristics of extralobar pulmonary sequestration in a dog. Grossly, a mass was observed in the left side of the thoracic cavity, closed to the caudal lobes of the lung, without communication with the tracheobronchial tree and the pulmonary arteries that was separated by pleural …

Coccidioidomycosis presenting as a heart base mass in two dogs.

Two atypical cases of canine coccidioidomycosis presenting as heart base masses are described. An echocardiogram performed in one of the two dogs revealed a large mass at the base of the heart and a computed tomography scan showed that the mass compressed the bronchi, left atrium, aorta and pulmonary arteries. A firm, white or pale yellow mass was found at the base of the heart at necropsy examination in both cases. Microscopical examination of the masses revealed severe, chronic, locally extensive granulomatous or pyogranulomatous inflammation with intralesional spherules consistent with Cocc…

[Update on eosinophilic airway disorders].

Eosinophilic disorders of the airways affect the nose and paranasal sinuses, the bronchi and the lungs. Involvement of the nose includes the well known non allergic rhinitis with eosinophilia syndrome and new disorders characterized by co-participation of mast cells and neutrophils. Eosinophilic bronchitis must be distinguished from asthma, on the basis of some functional findings and a different cytokine profile. Eosinophilic pneumonia comprises acute and chronic forms, Churg-Strauss syndrome, hypereosinophilic syndrome, and eosinophilic granuloma, as well as secondary pneumonia such as Loeff…

[Low-intensity laser radiation in the combined treatment of patients with chronic obstructive bronchitis].

This study included 89 patients with chronic obstructive bronchitis (COB) that were exposed to harmful occupational factors. The control group consisted of 30 healthy subjects. Chronic obstructive bronchitis is characterized by serious disturbances in the cardiorespiratory system that manifest themselves in the form of bronchogenic pneumosclerosis with moderate lung emphysema and pulmonary hypertension associated with impaired general resistance. Simultaneously, activity of lipid peroxidation reactions decreases. Application of low-intensity laser radiation in combination with other therapeuti…

[Tracheal cartilaginous sleeve in craniosynostosis].

Tracheal cartilaginous sleeve (TCS) is a congenital malformation involving fusion of the tracheal arches that may be isolated to a few tracheal arches, include the entire trachea, or extend beyond the carina into the bronchi. Tracheotomy was required in 9 of 23 craniosynostosis cases undergoing gradual distraction at Osaka City General Hospital from March 2002 to April 2006. TCS was diagnosed in 5 of 9 cases-four Pfeiffer patients and one Crouzon patient. Diagnosis was made intraoperatively during tracheotomy or at autopsy. 3D-CT was not useful in diagnosing TCS. Aggressive management of respi…

[Hilar adenopathy compressing the pulmonary arteries in the course of sarcoidosis].

INTRODUCTION: In pulmonary sarcoidosis, vascular involvement is usually limited to the small and medium-sized vessels. Enlarged hilar lymph nodes are usually considered to be soft and hence unlikely to cause pressure on adjacent bronchi or blood vessels. CASE REPORT: We report a rare symptomatic compression of a major pulmonary artery by lymphadenopathy in a case of sarcoidosis. Our patient presented with exertional dyspnoea, with enlarged hilar lymph nodes responsible for segmental pulmonary arterial hypertension, without other thoracic abnormalities. The symptoms and lymphadenopathy regress…

[Acute airway obstruction during chemotherapy-induced agranulocytosis with fever].

Acute airway obstruction caused by mucoid impaction can cause sometimes life-threatening respiratory distress. Bronchial plugging is usually observed in subjects with chronic diseases such as asthma, allergic bronchopulmonary aspergillosis, or cystic fibrosis. In children, it can be related to heart failure. Acute airway obstruction in a patient without a chronic respiratory disease is exceptional. We report the case of a patient who developed bronchial plugs obstructing the bronchi during a period of agranulocytosis induced by chemotherapy. The patient experienced acute respiratory distress w…

Endobronchial cryptococcosis in AIDS.

Although cryptococcosis is a common thoracic complication among patients with AIDS, endobronchial abnormalities have rarely been reported. A 45-year-old man presented with a productive cough, fever, and headache. His CD4+ cell count was 7/mm3 and testing for antibodies to HIV-1 was positive. Radiological examination revealed consolidation in the left lung, including cavitation. Bronchoscopic examination demonstrated white, slightly raised, plaque-like lesions in the trachea and left bronchi. Histopathological examination of endobronchial biopsy specimens revealed granulation tissue with abunda…

Prenatal diagnosis of tracheal obstruction: possible association with maternal pertussis infection.

A fetus with the sonographic appearance of echogenic and enlarged lungs and dilated trachea and bronchi, indicating laryngotracheal obstruction, is reported. Additionally, the fetus had ascites and subcutaneous edema and the amniotic fluid volume was reduced. Doppler flow investigation of the systemic venous circulation revealed signs of heart failure, and color Doppler visualized possible increased pulmonary flow. Following termination of pregnancy, autopsy confirmed the sonographic observations and revealed a hypoplastic thymus. During the present pregnancy the mother suffered from sustained…

[The characteristics of the humoral response to respiratory syncytial viral infection in adult patients with different forms of bronchitis].

The levels of specific IgG and IgM were determined in 72 patients with verified respiratory syncytial (RS) virus infection: 11 patients with acute delayed bronchitis (ADB), 26 with recurring bronchitis (RB) 35 with chronic obstructive bronchitis (ChOB), by indirect enzyme immunoassay; RS virus was detected by direct immunofluorescent technique. The activity of the inflammatory process in the bronchi was shown to depend upon RS infection activity in cases of ChOB and RB. RS infection was observed to be significantly dependent on obstruction and bronchospasms. The specific humoral response was r…

[Morphogenesis of cystic abnormalities of the lung].

The material used was the pathomorphological studies of cystic developmental lung defects with consideration of X-ray and clinical data. In total 126 cases have been studied including: 72 cystic lung hypoplasias, 38 solitary cysts, 8 sequestrations, 8 bronchogenic cysts of mediastinum. Cystic hypoplasia revealed branching of cystic cavities and sometimes their connection with respiratory compartments. The other above mentioned defects reveal sometimes the connection between cysts, bronchi and respiratory compartments. Consequently with the formation of cystic developmental lung defects the dev…

A new form of metaphyseal chondrodysplasia in two sibs: surgical treatment of tracheobronchial malacia and scoliosis.

We describe a 19-year-old male with a previously unrecognized form of disproportionate short stature, tracheobronchial malacia, and progressive scoliosis and his 28-year-old sister with the same but milder condition. The clinical characteristics were short limbs and digits and thoracolumbar scoliosis. Bone films showed progression from marked metaphyseal dysplasia of tubular bones in childhood to short and broad bones with mild dysplasia of the joints in adulthood. The vertebrae and the intervertebral plates were only mildly affected in spite of marked scoliosis. Trachea and bronchi were reinf…

Focal mineralization and nonspecific granulomatous inflammation of respiratory mucous membranes in pigs.

Discrete mineralized foci and granulomatous inflammation occurred in the lamina propria mucosae of respiratory mucous membranes of adult pigs. Lesions were present in clinically healthy pigs of both sexes, including castrated males, fed various pelleted or non-pelleted diets. They were mainly in longitudinally corrugated mucosae of the dorsal wall of the trachea. Identical mineralization and inflammation occurred in the nasal cavity and, in decreasing frequency and intensity, in the thoracic trachea and bronchi. The lesions in respiratory mucous membranes occurred in pigs with and without mine…
