Development and validation of a nomogram model for predicting infarction after superficial temporal artery‒middle cerebral artery bypass in patients with intracranial atherosclerotic stenosis.

OBJECTIVE: This study aimed to develop and validate a nomogram model for predicting cerebral infarction risk after superficial temporal artery-middle cerebral artery (STA-MCA) bypass in patients with intracranial atherosclerotic stenosis (ICAS). METHODS: Patients with ICAS who received STA-MCA bypass were enrolled in this study. The independent risk factors for post bypass infarction were identified using univariate and multivariate logistic regression analyses. A nomogram model was developed and subsequently evaluated using receiver operating characteristic (ROC) curves, calibration curves,

Assessing superficial temporal artery-middle cerebral artery anastomosis patency using FLOW 800 hemodynamics.

OBJECTIVE: The objective of this study was to investigate the use of indocyanine green videoangiography with FLOW 800 hemodynamic parameters intraoperatively during superficial temporal artery-middle cerebral artery (STA-MCA) bypass surgery to predict patency prior to anastomosis performance. METHODS: A retrospective and exploratory data analysis was conducted using FLOW 800 software prior to anastomosis to assess four regions of interest (ROIs; proximal and distal recipients and adjacent and remote gyri) for four hemodynamic parameters (speed, delay, rise time, and time to peak). Medical rec

Long-Term Patency of Posterior Auricular Artery-Middle Cerebral Artery Bypass for Adult-Onset Moyamoya Disease: Case Report and Review of Literature.

BACKGROUND: Direct extracranial-intracranial (EC-IC) bypass is one of the fundamental techniques to prevent recurrent stroke in patients with adult-onset ischemic moyamoya disease. When the standard superficial temporal artery (STA) cannot be used for a graft, the posterior auricular artery (PAA) can be a potential surrogate graft. CASE DESCRIPTION: In this article, the authors reported a 34-year-old female patient suffering from ischemic moyamoya disease. To widely revascularize the anterior half of the hemisphere, direct double EC-IC bypass was considered beneficial; however, she had only a

Long-term follow-up results in 142 adult patients with moyamoya disease according to management modality.

BACKGROUND: To clarify the most beneficial treatment of the management modality based on our experience with adult moyamoya disease (MMD). METHODS: From 1998 to 2010, clinical results of 142 patients (ischemic, 98; hemorrhagic, 44) with adult MMD were investigated according to management modality. Revascularization surgery (direct, indirect, and combined bypass) was performed in 124 patients. We observed the clinical course of 18 patients who were treated conservatively. Clinical outcome, angiographic features, hemodynamic change, and incidence of recurrent stroke were investigated pre- and p

Emergency Mechanical Thrombectomy for Acute Middle Cerebral Artery Occlusion Accompanied by Adult-Onset Still's Disease.

Adult-onset Still's disease (AOSD) is a rare systemic inflammatory condition of an unknown etiology. Stroke is a rare complication associated with AOSD; most of these are cerebral infarctions due to the occlusion of small blood vessels. Here, we report the first case of mechanical thrombectomy in a patient with cerebral infarction due to a large vessel occlusion associated with AOSD. A 60-year-old man with no underlying disease was diagnosed with AOSD. Sixteen days after admission, he suddenly lost consciousness and was found to have right hemiplegia and aphasia. Head CT showed early signs of

Improved neurologic function after cerebrovascular accident with extracranial-intracranial arterial bypass.

This study, by use of direct measurement, sought to investigate the role of diminished cerebral blood flow on neurologic function after cerebrovascular accident (CVA). Twenty-seven patients had mean middle cerebral artery pressure (MCAP) measured intraoperatively prior to a superficial temporal artery to middle cerebral artery bypass. The ratio of MCAP to mean systemic blood pressure (BP) was less than 0.500 in 11 patients. Six of these 11 had preoperative neurologic deficit and four of these six had improved neurologic function after revascularization. In 16 patients the MCAP/BP was greater t

Endovascular treatment for severe cerebral venous sinus thrombosis in a patient with polycythemia vera and nephrotic syndrome: a case report.

Cerebral venous sinus thrombosis (CVST) is a rare cerebrovascular disease that can occur at any age and generally has a good prognosis. Polycythemia vera and nephrotic syndrome are uncommon risk factors for cerebral venous sinus thrombosis. A dilemma exists in the treatment of cerebral venous sinus thrombosis with polycythemia vera and nephrotic syndrome, as some cases are refractory to first-line therapy. Here, we report a patient with CVST who presented with a generalized seizure and was found to have bilateral frontal lobe hemorrhage and subarachnoid hemorrhage. Brain magnetic resonance ven

Total arch replacement and classic elephant trunk in a patient with an anomalous left vertebral artery.

This video tutorial explains how to treat an aortic aneurysm affecting the arch, descending, and thoracoabdominal segments, in the presence of an anatomical aortic arch variation. The procedure involves replacing the entire aortic arch and ascending aorta using the classic elephant trunk technique, creating a platform for a planned staged endovascular treatment. Special emphasis is placed on cerebral protection strategies, including preservation of an anomalous left vertebral artery and special reinforcement of a distal arch anastomosis. The presence of an anomalous artery arising directly fro

A3-A3 Bypass for Giant Anterior Cerebral Artery Aneurysm: 2-Dimensional Operative Video.

Large fusiform anterior cerebral artery aneurysms often require revascularization to allow for the treatment of the aneurysm and preservation of distal perfusion. The A3-A3 side-to-side anastomosis maintains ipsilateral distal perfusion. The inflow to the fusiform segment can then be clip occluded to treat the diseased vessel segment. This procedure is illustrated by the case in this video. The patient had a large right anterior cerebral artery fusiform aneurysm. An anterior interhemispheric craniotomy with the right side down was utilized for the approach. Postprocedural angiography demonstra

Ruptured Duplicated Middle Cerebral Artery Aneurysm Successfully Treated by Coil Embolization with Balloon Remodeling.

A duplicated middle cerebral artery (dMCA) is a rare anatomical variant that can be associated with aneurysm formation and rupture. The dMCA arises from the terminal portion of the internal carotid artery, with the caudal trunk supplying the temporal lobe and the cranial trunk supplying the frontal lobe and the lenticulostriate perforators. Previous reported cases were all treated with open surgical clipping with or without cerebral revascularization to reconstruct the dMCA. We present a rare case of a young woman with a ruptured dMCA (subtype B) aneurysm. We have demonstrated the unique anato…

Novel therapies and the best new device concepts for 2013 CAS techniques: the Mo.Ma® Proximal Occlusion System.

Recently carotid artery stenting (CAS) was upgraded in the class of recommendation among other revascularization treatments for symptomatic patients with carotid artery disease. This makes CAS potentially available to a broader range of patients. Significant evolution in CAS materials and protection devices contributed towards this improvement. Particularly in the setting of symptomatic carotid stenosis the most important cerebral protection devices available today, are the proximal protection systems. Comparing to the distal, filter-type protection devices, proximal protection systems can off

Occlusion of unilateral carotid artery in Down syndrome.

The association between moyamoya phenomena and Down syndrome (DS) is reported in the literature. This paper reports a case of DS, which at age 9 presented right hemiparesis, secondary to the occlusion of the left internal carotid artery; cerebral angiography (CAG) showed a collateral circulation that mimicks the moyamoya phenomenon. Clinical recovery was almost complete; a second CAG after 15 months showed a persistent occlusion of the left internal carotid artery and an opacification of the left middle cerebral artery from abnormal vessels; but the collateral circulation is not enhanced. This

Transcarotid arterial revascularization for symptomatic carotid web.

OBJECTIVES: Symptomatic carotid web is an increasingly recognized cause of acute ischemic stroke with a high risk of recurrent ischemic events despite aggressive medical interventions. Surgical interventions including transfemoral carotid artery stenting (TFCAS) and carotid endarterectomy have been described to reduce this risk, but transcarotid arterial revascularization (TCAR) has not been evaluated for this purpose. MATERIALS AND METHODS: Patients with cerebral ischemia from carotid web underwent TCAR with flow reversal. Patients were monitored for periprocedural complications and assessed

Carotid-carotid crossover bypass after mechanical thrombectomy for internal carotid artery occlusion due to plaque from stenosed innominate artery.

BACKGROUND: The treatment for internal carotid artery occlusion (ICAO) due to innominate artery stenosis is not well established. We herein describe a case of carotid-carotid crossover bypass and common carotid artery (CCA) ligation after mechanical thrombectomy for ICAO due to a plaque from the stenosed innominate artery. CASE DESCRIPTION: A 70-year-old man was transferred to our hospital because of left-sided hemiparalysis. Head magnetic resonance imaging/angiography showed a cerebral infarction in the right middle cerebral artery area and the right ICAO due to a plaque from the stenosed in

Recanalization of an acute middle cerebral artery occlusion using a self-expanding, reconstrainable, intracranial microstent as a temporary endovascular bypass.

BACKGROUND AND PURPOSE: Although self-expanding intracranial microstents have been used to treat acute middle cerebral artery (MCA) stroke, there are disadvantages associated with placing a permanent endovascular implant. We describe a technique in which a reconstrainable stent was used to provide a temporary endovascular bypass to achieve MCA recanalization without permanent stent implantation. METHODS: A 55-year-old male presented with acute onset left hemiplegia (National Institutes of Health Stroke Score (NIHSS) of 20. Moyamoya-Type Collaterals in ACTA2 p. Arg179 Related Arteriopathy: Challenging the Paradigm of Absent Neovascularization-A Longitudinal DSA Study.

BACKGROUND AND PURPOSE: ACTA2 p. Arg179 variants can cause a distinctive cerebrovascular arteriopathy historically described lacking classic moyamoya basal collaterals. Limited data on collateral architecture and surgical outcomes have led to uncertainty regarding revascularization strategies. The purpose of this study is to provide a systematic longitudinal DSA assessment of collateral patterns and postoperative findings in pediatric ACTA2-associated cerebrovascular disease. MATERIAL AND METHODS: Ten pediatric patients with genetically confirmed ACTA2 mutations underwent DSA imaging. Eight c

Urgent endovascular revascularization for symptomatic intracranial atherosclerotic stenosis.

BACKGROUND: Endovascular revascularization for intracranial atherosclerotic stenoses is being increasingly performed at major medical centers and has been reported to be technically feasible and safe. The authors report their experience with patients who underwent such a procedure for impending stroke and neurologic instability. METHOD: All 18 patients (21 intracranial lesions) treated between 1997 and 2002 at the authors' institution with endovascular revascularization were retrospectively reviewed. Each patient had failed maximal medical therapy and was thought to be at high risk for an imm

Asymptomatic moyamoya disease subsequently manifesting as transient ischemic attack, intracerebral hemorrhage, and subarachnoid hemorrhage in a short period: case report.

A 59-year-old man, who had incidentally been found to have asymptomatic moyamoya disease 4 months previously, suffered transient ischemic attack (TIA) in the left extremities. Three weeks later, he again suffered TIA, and neuroimaging examination revealed fresh subarachnoid hemorrhage in the interhemispheric cistern as well as an asymptomatic thalamic hemorrhage in the contralateral hemisphere, which was not evident at the first onset of TIA. Digital subtraction angiography confirmed the diagnosis of moyamoya disease, and single-photon emission computed tomography demonstrated significant hemo

Symptomatic ≤50% carotid artery stenosis is associated with higher rates of vulnerable plaque features and acceptable revascularization outcomes: A three-stroke-center cohort study.

OBJECTIVE: Current carotid revascularization paradigms are primarily stenosis-based, with optimized medical therapy alone generally recommended for symptomatic patients with (≤50%) mild-to-moderate degree of stenosis (MMDS) and no routine recommendation for revascularization, despite evidence of potentially high-risk plaque features beyond stenosis. We aimed to contrast clinical presentation, prevalence of vulnerable plaque features, and postrevascularization outcomes among symptomatic patients with MMDS and high-degree stenosis (HDS > 50%) across three stroke centers. METHODS: We performed a
